MyBilling Services - Policyholder Access Agreement

This is the login screen for Great American Insurance Company’s MyBilling
application (the "System"). Proprietary and confidential information of Great
American Insurance Company and its affiliates (the "Company") is stored,
transmitted, and processed by the System. To access the System you, on behalf of
yourself and any organization or company you represent ("You" or "Your"), must
(i) enter your User ID and Password at the login screen; (ii) read the System
terms and conditions of use below ("Access Agreement"); and (iii) agree to the
Access Agreement by clicking the "Log In" box below.

Each time You enter Your User ID and Password at the login screen and access the
System, You must agree to the Company’s Access Agreement, which the Company may
revise at any time. In exchange for the right to access and use the System, You
agree to the following terms and conditions.
 1.  If You access the System on behalf of an organization or company, You bind
     that organization or company to these terms and You affirm that You have
     the legal authority to do so.
 2.  You agree that the Company is the owner of or has the right to control all
     data contained in the System ("Data"). You agree that the Data is the
     proprietary and confidential information of the Company, which may include
     nonpublic personal information ("NPI"). You will preserve and protect the
     confidentiality of all Data with at least the same standard of care with
     which You protect Your own confidential information and in no event less
     than a commercially reasonable standard of care.
 3.  For the purposes of this Access Agreement, "NPI" is defined as nonpublic
     personal information of Company’s and Your (or Your company’s) employees,
     contract holders, insureds, customers, and consumers and/or of Company and
     Your subsidiaries or affiliates including, but not limited to, Social
     Security Numbers, identification card numbers, credit or debit card
     numbers, financial account numbers, claims information, health information,
     and biometric records. You agree to comply fully with all laws and
     regulations governing the collection, use, and disclosure of NPI ("Privacy
     Laws"). You agree not to disclose any NPI that You access through the
     System, except as permitted by applicable Privacy Laws and this Access
     Agreement.
 4.  You will access and use the System and Data only for the purposes of
     managing Your insurance policies written by, or other product or service
     purchased from, the Company purposes ("Permitted Purpose").
 5.  You will not (i) use and access the System and Data for any purpose that is
     unlawful; (ii) use the system for any purpose other than the Permitted
     Purpose; and (iii) copy, redistribute, use or publish any Data for any
     purpose other than the Permitted Purpose.
 6.  You will not knowingly submit or enter false, incomplete, or misleading
     information into the System. You agree to keep account information
     up-to-date and accurate.
 7.  The Company does not warrant or guarantee the accuracy of the Data. The
     Company specifically disclaims direct and indirect liability for any loss,
     damage, or expense incurred by You arising from or related to (i) Your
     access to or use of the Systems or Data; and (ii) any actual or alleged
     infringement of patents or intellectual property rights of other parties
     related to the Systems or Data.
 8.  You agree to indemnify, hold harmless, and defend the Company against any
     loss, damage, or expense, including attorney fees, incurred by the Company
     as a result of (i) Your negligent or more culpable act or omission arising
     from use of the System and Data; or (ii) Your breach of any of the terms of
     this Access Agreement.
 9.  You will (i) limit access to the System to only Your employees and
     contractors who need to access the System for the Permitted Purpose
     ("Authorized Employees"); (ii) prohibit Authorized Employees from sharing
     account access; and (iii) ensure Authorized Employees protect their User ID
     and Password using industry best practices.
 10. You agree to the terms and conditions of Company’s privacy policy which may
     be found at https://www.greatamericaninsurancegroup.com/contact/privacy.
     Company may update the terms and conditions the privacy policy at any time
     without notice.
 11. Your access to the System does not confer a license to use the System and
     the Company may terminate this Access Agreement and Your access to the
     System, in whole or in part, at any time, with or without cause.
 12. This Access Agreement shall be governed, construed, and enforced in
     accordance with the laws of the State of Ohio.
 13. In the event of a conflict in terms between this Access Agreement and any
     other agreement governing access to the System, this Access Agreement shall
     control. However, if any other agreement between the Company and You
     governing access to the System contains any obligations of Yours that are
     in addition to or are more restrictive on You than those set forth in this
     Access Agreement, such additional or more restrictive obligations shall
     control.
 14. You shall send any notice required to be provided to the Company under this
     Access Agreement to: Great American Insurance Company, Attn: General
     Counsel, 301 East Fourth Street, Cincinnati, Ohio 45202.



To indicate that You agree to this Access Agreement, enter Your User ID and
Password in the login screen and then click the box labeled "Log In". If You do
not agree to this Access Agreement, You will not be permitted to access the
System.

We want you to know about our procedures for protecting your privacy and your
rights and responsibilities regarding nonpublic personal information (referred

customers of Great American.

Great American does not share your data except as allowed by law. As a result,
you do not need to take any action under this Notice. If we change our practices
in the future, we will advise you. If applicable, we will allow you to “opt-out”
of certain sharing.

1. What kind of data is collected about you?

We get most of our data about you directly from you, such as your name, address,
social security number, income level and certain other financial data. We
collect data that you provide during the insurance application process and by
other contact with you by mail and over the phone.

In some cases we may need additional data or may need to verify data you have
given us. In those cases, we may obtain data from outside sources at our own
expense. For instance, we may collect data from consumer reporting agencies such
as credit worthiness and history or employment history. If you send a written
request to the address below, we will inform you o f the name and address of any
agency we have used to prepare a report on you so that you can contact the
agency.

Once you become our customer, we may collect data related to our experiences and
transactions with you. This could include data such as insurance policy
coverage, premiums and payment history, and any claims you make under your
insurance policy. For example, we will retain data collected by a claims
representative and police or fire reports.

We may also collect data about you from our affiliates regarding their
transactions and experiences with you (such as your payment or claims history).
We do not currently share other credit-related data, except as allowed or
required by law.

Finally, we may collect data when you visit our website or when you email us. We
do not sell this or any other data about you to anyone.

2. What do we do with data about you?

Data about you will be kept in our records. We may disclose data to issue and
service policies and settle claims. Generally, we will not disclose data about
you to any outside group without your prior authorization. However, we may, as
allowed by law, share data that we collect as set forth below.

We may disclose data to your insurance agent.

We may disclose data to persons who represent you, including your attorney or
trustee.

We may disclose data to adjusters, appraisers, auditors, investigators and
attorneys.

We may disclose data to those who need the data to perform a business,

agencies, in connection with any insurance application, policy or claim

We may disclose data, other than health data, to others that perform marketing
services on our behalf.

We may disclose data to our affiliated companies to market products to you and
for other purposes. The law does not allow you to restrict this sharing.

agency pursuant to a summons, court order, search warrant, subpoena, or as
otherwise required by law or regulation.

to underwrite or administer your policy, claim or or account;

in a manner as previously disclosed to you by us when we collected your health
data.

When we disclose your data to third parties for certain purposes described
above, we will require them to use your data only for its intended purpose.

3. Who has access to your data?

The only people who have access to your data are those who need it to provide or
support the provision of products or services to you. We use a system of
passwords and other appropriate physical, electronic and procedural safeguards
to protect against unauthorized access to your data. We have educated our
employees about this Notice and the importance of customer privacy.

4. How can you review recorded data about you?

You have the right to access and inspect most of the data that we collect about
you. To access your data please send a written request to the address below
stating that you would like to access your data. Either you or your personal
representative must sign this request and provide a copy of your driver’s
license or other valid photo identification. You also have the right to request
that we correct any data that you believe is incorrect. To amend your data,
please send us a written request, at the address below, stating what data you
believe needs correcting. Once again, either you or your personal representative
must sign this request. If you submit a request to amend your data, we will
investigate. If we agree, we will correct our records. Even if we do not correct
the data, you have the right to file with us a written statement of dispute,
which we will include, in any future disclosure of the data.

MyBilling Services - Policyholder Access Agreement

Access to the "MyBilling" application data and information ("MyBilling
Services") of Mid-Continent Casualty Company (the "Company") is restricted to
policyholders who have received individual user identification names and
passwords assigned by the Company. To obtain access to the MyBilling Services
you must indicate your agreement to the terms of use stated herein by clicking
on the word "Agree" below.

Each time you enter your assigned user identification name ("User ID") and
password ("Password") in the Login box and gain access to the MyBilling
Services, you are agreeing to the Company's stated terms of use, which the
Company may amend from time to time without specific notice to you, and you (the
"User") are acknowledging that your access to and use of the MyBilling Services
is expressly conditioned upon your compliance with these stated terms of use:
 1. Company is the absolute owner of the MyBilling Services and User IDs and
    Passwords used to access the MyBilling Services. User's permitted use of an
    assigned User ID, Password and the MyBilling Services does not create or
    imply any lien, interest or right therein on the part of User. The assigned
    User ID and Password are not transferable
 2. User agrees to use any information accessed via the MyBilling Services only
    for the purposes of administration and servicing of insurance policies
    written by the Company or its affiliates. User may not copy, redistribute,
    use or publish any content, graphic, form or document accessed via the
    MyBilling Services for any other purposes.
 3. User acknowledges that the MyBilling Services include proprietary and/or
    confidential customer information of the Company. User agrees to preserve
    and protect the confidentiality of such information and to comply fully with
    all federal and state laws and regulations governing the collection and use
    of the non-public financial and medical information ("Privacy Laws") of the
    consumers of Company. User specifically agrees not to disclose, copy,
    redistribute, use or publish any non-public personal information that User
    may obtain through access to the MyBilling Services, except as permitted by

    its affiliates as a result of (a) User's misuse of the User ID, Password or

    Agreement.
 6. User agrees to provide true, accurate, current and complete information
    about User as requested by MyBilling Services, and agrees not to
    misrepresent Users identity. User agrees to keep account information up to
    date and accurate.
 7. Company reserves the right to change or discontinue, temporarily or
    permanently, the service at any time without notice. You agree that Company
    will not be liable to you or any third party for any modification or
    discontinuance of the service.
 8. Company may broadcast notices or messages through the MyBilling Services to
    inform you of changes to the terms of use of MyBilling Services, or other
    matters of importance. Such broadcasts shall constitute effective notice to
    you, and your continued use of MyBilling Services shall constitute your
    acceptance of Company’s changes to the terms of use.
 9. If any provision herein is held to be unenforceable by a judicial or
    regulatory authority having legal jurisdiction over the parties to this
    Access Agreement, then such provision shall be unenforceable without
    impairing the enforceability of the other provisions, which shall remain in
    full force and effect. The laws of the state of Ohio apply to this
    agreement.

MyBilling Services Access Agreement
 1. Access to the "MyBilling" application data and information ("MyBilling
    Services") of Mid-Continent Casualty Company ("Company") is restricted to
    third party producers, their employee's and customer service representatives
    (collectively "Users"), who have each received individual user
    identification names ("User ID") and individual passwords ("Password")
    assigned by the Company.
 2. By entering the User ID and Password in the login box on this webpage each
    time User accesses the MyBilling Services and selecting "Agree", User agrees
    to comply with and be bound by the terms and conditions set forth in this
    Access Agreement, which the Company may periodically amend without specific
    notice to any User. If User does not agree to these terms of use, User
    should select the button labeled "Disagree" to terminate the login
    procedure.
 3. Producers must promptly notify the Company when an employee or customer
    service representative ("CSR") ceases working for or performing services on
    behalf of the Producer, or for any reason for which the employee or CSR no
    longer has a need to access the MyBilling Services.
 4. Company and its affiliates are the absolute owners of the MyBilling
    Services, User IDs and Passwords used to access the MyBilling Services. The
    use of an assigned User ID, Password and access to the MyBilling Services
    does not create or imply any lien, interest or right therein on the part of
    User. The assigned User ID and Password are not transferable to any other
    individual.
 5. User acknowledges that the MyBilling Services contains confidential and/or
    proprietary information of Company and its affiliates, including the User ID
    and Password ("Confidential Information"). User shall use any information
    obtained via the MyBilling Services solely for administering and servicing
    insurance policies written by the Company or its affiliates.
 6. User agrees to preserve and protect all Confidential Information from
    unauthorized access, use, or disclosure using at least the same measures and
    safeguards as User uses to protect its most valued Confidential Information,
    but in no case less than a reasonable degree of control. User may not copy,
    redistribute, use or publish any content, graphic, form, or document
    obtained via the MyBilling Services for any other purposes. User shall not
    disclose Company's Confidential Information to any individual, other than
    User's own employees or CSR's who need to know such information for the sole
    purpose of performing the activities described herein, and shall ensure that
    User's employees, CSRs and contractors comply with the terms of this Access
    Agreement. Notwithstanding the foregoing, User shall not share his or her
    User ID or Password with any individual.
 7. User also agrees to fully comply with all federal and state laws and
    regulations governing the collection and use of any non-public financial
    information and personally identifiable information ("Privacy Laws"). User
    specifically agrees not to disclose, copy, redistribute, use or publish any
    non-public personal financial information or personally identifiable
    information that User may obtain through access to the MyBilling Services,
    except as permitted by applicable Privacy Laws.
 8. Company has made reasonable efforts to ensure the accuracy of the data and
    information provided via the MyBilling Services. However, Company does not
    warrant or guarantee the accuracy thereof. Company specifically disclaims
    any liability for any loss, damage or expense incurred by User as a result
    of any: (a) actual or alleged mistakes, errors or omissions in the
    information or data supplied via the MyBilling Services, or (b) delays,

provided. In those cases, we may obtain information from outside sources at our
own expense.

It is common for an insurance company to ask an independent source, such as a
consumer reporting agency, to verify and supplement information given on an
insurance application. We will treat the information we receive about you from
an independent reporting agency in accordance with the terms of this Notice.
Upon our receipt of your written request sent to the address below, we will
inform you of the name and address of any agency we have used to prepare a
report on you so that you can contact the agency.

Once you have been an insured customer of ours for a period of time, your record
may contain information related to our experiences and transactions with you,
such as insurance policy coverage, premiums and payment history, and any claims
you make under your insurance policy. For example, information collected by a
claims representative and any policy or fire report will be retained by us. Any
information that we collect in connection with an insurance claim will be kept
in accordance with this Notice.

Each company within Mid–Continent Group may disclose information about you to an
affiliate regarding its transactions and experiences with you (such as your
payment or claims history). We do not currently share other credit–related
information, except as permitted or required by law.

Finally, we do use “cookies” when you interact with our web sites to assist in
making the process as easy and secure as possible. Cookies are used to collect
and store only the following information: the visitor’s domain name, the
Internet address of the web site from which the visitor linked directly to our
web site, the pages of our site that the visitor views and the length of time
spent on each page, browser and operating system platform type, and the date and
time the visitor accessed our site.

Cookies, as well as data taken from them, do not identify you personally. They
merely recognize your browser. Unless you choose to identify yourself to us,
either by responding to a promotional offer, buying a policy, or registering for
an online service, you remain anonymous.

Session cookies exist only during an online session with Mid–Continent. Session
cookies allow you to conduct transactions or requests on our web site. Without
the session cookie information, we would not be able to complete your web
transactions securely. Session cookies help us make sure you are who you say you
are after you have logged in. We do not sell this or any other information about
you to other web sites, merchants or financial institutions.

What do we do with information about you?

Information about you will be kept in our insurance policy records. We will
refer to and use that information for purposes related to issuing and servicing
insurance policies and settling claims. Generally, personal information about
you in our records will not be disclosed by us to any external organization
without your prior authorization. However, we may, as permitted by law, share
information about you contained in our files with certain persons or
organizations such as:

Who has access to your information?

Mid–Continent currently incorporates a system of passwords and other appropriate
physical, electronic and procedural safeguards to protect against unauthorized
access to potentially private information. We educate our employees about the
terms of this Notice and the importance of confidentiality and customer privacy.
Employees who gain unauthorized access or who otherwise violate our privacy
policy are subject to disciplinary action up to and including termination of
employment. We will monitor and evaluate our information security program and
security software and, as technology changes, determine ways to increase
protections to the security or integrity of our records and information.

How can you review recorded information about you?

You have the right to receive and review a copy of the recorded personal
information about you contained in our files with respect to a particular policy
number, except for certain legal and medical documents. You have the right to
request that we correct any of this information. To exercise these rights, you
must send us a notarized request at the address set forth below stating your
complete name, address, insurance policy number, daytime phone number, and a
copy of your driver’s license or other personal identification. If you believe
any information is incorrect, we will investigate and correct it if we can
substantiate the request. Even if we do not correct the information, you have
the right to file with us a written statement of dispute which we will include
in any future disclosure of the information.

MyBilling Services Access Agreement

This is the login screen for Great American Insurance Company’s MyBilling
Application (the "System"). Proprietary and confidential information of Great
American Insurance Company and its affiliates (the "Company") is stored,
transmitted, and processed by the System. To access the System you, on behalf of
yourself and any organization or company you represent ("You" or "Your"), must
(i) enter your User ID and Password at the login screen; (ii) read the System
terms and conditions of use below ("Access Agreement"); and (iii) agree to the
Access Agreement by clicking the "Log In" box below.

Each time You enter Your User ID and Password at the login screen and accesses
the System, You must agree to the Company’s Access Agreement, which the Company
may revise at any time. In exchange for the right to access and use the System,
You agree to the following terms and conditions.
 1.  If You access the System on behalf of an organization or company, You bind
     that organization or company to these terms and You affirm that You have
     the legal authority to do so.
 2.  You agree that the Company is the owner of or has the right to control all
     data contained in the System ("Data"). You agree that the Data is the
     proprietary and confidential information of the Company, which may include
     nonpublic personal information ("NPI"). You will preserve and protect the
     confidentiality of all Data with at least the same standard of care with
     which You protect Your own confidential information and in no event less
     than a commercially reasonable standard of care.
 3.  For the purposes of this Access Agreement, "NPI" is defined as nonpublic
     personal information of Company’s and Your (or Your company’s) employees,
     contract holders, insureds, customers, and consumers and/or of Company and
     Your subsidiaries or affiliates including, but not limited to, Social
     Security Numbers, identification card numbers, credit or debit card
     numbers, financial account numbers, claims information, health information,
     and biometric records. You agree to comply fully with all laws and
     regulations governing the collection, use, and disclosure of NPI of
     Company’s customers and consumers ("Privacy Laws"). You agree not to
     disclose any NPI that You access through the System, except as permitted by
     applicable Privacy Laws and this Access Agreement.
 4.  You will access and use the System and Data only for: (i) purposes of
     administration and servicing of insurance policies written by the Company;
     and (ii) for the benefit of the Company (collectively the "Permitted
     Purpose").
 5.  You will not (i) use and access the System and Data for any purpose that is
     unlawful; (ii) use the system for any purpose other than the Permitted
     Purpose; and (iii) copy, redistribute, use or publish any Data for any
     purpose other than the Permitted Purpose.
 6.  You will not knowingly submit or enter false, incomplete, or misleading
     information into the System. You agree to keep account information
     up-to-date and accurate.
 7.  The Company does not warrant or guarantee the accuracy of the Data. The
     Company specifically disclaims direct and indirect liability for any loss,
     damage, or expense incurred by You arising from or related to (i) Your
     access to or use of the Systems or Data; and (ii) any actual or alleged
     infringement of patents or intellectual property rights of other parties
     related to the Systems or Data.
 8.  You agree to indemnify, hold harmless, and defend the Company against any
     loss, damage, or expense, including attorney fees, incurred by the Company
     as a result of (i) Your negligent or more culpable act or omission arising
     from use of the System and Data; or (ii) Your breach of any of the terms of
     this Access Agreement.
 9.  You will (i) limit access to the System to only Your employees and
     contractors who need to access the System for the Permitted Purpose
     ("Authorized Employees"); (ii) prohibit Authorized Employees from sharing
     account access; and (iii) ensure Authorized Employees protect their User ID
     and Password using industry best practices. You must promptly notify the
     Company when an Authorized Employee ceases working for or performing
     services on your behalf, or for any reason for which the Authorized
     Employee no longer needs access to the System.
 10. You represent, warrant and covenant that Your data security policies,
     procedures and controls: (i) comply with all applicable laws; (ii) include
     technical, physical and administrative or organizational safeguards
     designed to: (a) secure and maintain confidentiality and security of NPI in
     Your possession; (b) protect against any anticipated threats or hazards to
     the security and integrity of NPI provided to You; (c) protect against
     unauthorized processing, theft, loss, use, disclosure, acquisition or
     access to NPI provided to You; and (d) protect NPI using industry standard
     encryption tools; (iii) include regular testing or otherwise monitoring of
     the effectiveness of the safeguards; and (iv) require any sub-contractors
     who have or may have access to NPI to employ data security policies,
     procedures and controls at least as restrictive as those employed by You.
 11. Upon the provision of reasonable notice by the Company, the Company or its
     designee or a regulator may undertake a security assessment and/or audit of
     Your systems and information security measures: (i) at least once per year
     during the term of the Agreement; and (ii) following an information
     security incident.
 12. You will immediately notify the Company in writing of: (a) any problems,
     system malfunction or security incident which impacts the Company’s
     operations, impacts the Company’s service to its customers, or may have a
     significant effect on Your ability to perform its obligations under this
     Agreement; or (b) any use or disclosure of NPI not authorized by this
     Agreement, including any reasonable belief that an unauthorized individual
     or other entity has accessed NPI. Upon the Company’s written request, You
     agree to be responsible for notification of individuals in the event of
     unauthorized release of NPI or other event requiring notification and to
     perform such obligations in compliance with applicable law. In the event
     notification is required under this section, You will cooperate with the
     Company’s investigation of any such problems including, but not limited to,
     providing the Company with information adequate to determine the root cause
     and impact of any such problems as reasonably requested by the Company.
 13. You agree to the terms and conditions of Company’s privacy policy which may
     be found at https://www.greatamericaninsurancegroup.com/contact/privacy.
     Company may update the terms and conditions the privacy policy at any time
     without notice.
 14. Your access to the System does not confer a license to use the System and
     the Company may terminate this Access Agreement and Your access to the
     System, in whole or in part, at any time, with or without cause.
 15. This Access Agreement shall be governed, construed, and enforced in
     accordance with the laws of the State of Ohio.
 16. In the event of a conflict in terms between this Access Agreement and any
     other agreement governing access to the System, this Access Agreement shall
     control. However, if any other agreement between the Company and You
     governing access to the System contains any obligations of Yours that are
     in addition to or are more restrictive on You than those set forth in this
     Access Agreement, such additional or more restrictive obligations shall
     control.
 17. You shall send any notice required to be provided to the Company under this
     Access Agreement to: Great American Insurance Company, Attn: General
     Counsel, 301 East Fourth Street, Cincinnati, Ohio 45202.



To indicate that You agree to this Access Agreement, enter Your User ID and
Password in the login screen and then click the box labeled "Log In". If You do
not agree to this Access Agreement, You will not be permitted to access the
System.

We want you to know about our procedures for protecting your privacy and your
rights and responsibilities regarding nonpublic personal information (referred

to those individuals who inquire about or obtain insurance from Great American
primarily for personal, family or household purposes.

We will provide our customers with a copy of the most recent notice of our
privacy policy at least annually and more often if we make any changes affecting
their rights under our privacy policy. This Notice applies to current and former
customers of Great American.

Great American does not share your data except as allowed by law. As a result,
you do not need to take any action under this Notice. If we change our practices
in the future, we will advise you. If applicable, we will allow you to “opt-out”
of certain sharing.

1. What kind of data is collected about you?

We get most of our data about you directly from you, such as your name, address,
social security number, income level and certain other financial data. We
collect data that you provide during the insurance application process and by
other contact with you by mail and over the phone.

In some cases we may need additional data or may need to verify data you have
given us. In those cases, we may obtain data from outside sources at our own
expense. For instance, we may collect data from consumer reporting agencies such
as credit worthiness and history or employment history. If you send a written
request to the address below, we will inform you o f the name and address of any
agency we have used to prepare a report on you so that you can contact the
agency.

Once you become our customer, we may collect data related to our experiences and
transactions with you. This could include data such as insurance policy
coverage, premiums and payment history, and any claims you make under your
insurance policy. For example, we will retain data collected by a claims
representative and police or fire reports.

We may also collect data about you from our affiliates regarding their
transactions and experiences with you (such as your payment or claims history).
We do not currently share other credit-related data, except as allowed or
required by law.

Finally, we may collect data when you visit our website or when you email us. We
do not sell this or any other data about you to anyone.

2. What do we do with data about you?

Data about you will be kept in our records. We may disclose data to issue and
service policies and settle claims. Generally, we will not disclose data about

With your written consent;

to underwrite or administer your policy, claim or or account;

in a manner as previously disclosed to you by us when we collected your health
data.

When we disclose your data to third parties for certain purposes described
above, we will require them to use your data only for its intended purpose.

3. Who has access to your data?

The only people who have access to your data are those who need it to provide or
support the provision of products or services to you. We use a system of
passwords and other appropriate physical, electronic and procedural safeguards
to protect against unauthorized access to your data. We have educated our
employees about this Notice and the importance of customer privacy.

4. How can you review recorded data about you?

You have the right to access and inspect most of the data that we collect about
you. To access your data please send a written request to the address below
stating that you would like to access your data. Either you or your personal
representative must sign this request and provide a copy of your driver’s
license or other valid photo identification. You also have the right to request
that we correct any data that you believe is incorrect. To amend your data,
please send us a written request, at the address below, stating what data you
believe needs correcting. Once again, either you or your personal representative
must sign this request. If you submit a request to amend your data, we will
investigate. If we agree, we will correct our records. Even if we do not correct
the data, you have the right to file with us a written statement of dispute,
which we will include, in any future disclosure of the data.
