ELIGIBILITY REQUIREMENTS

Welcome to the Minneapolis/St. Paul Business Journal’s 25th annual Best Places
to Work nomination form.

Filling out this online form is necessary to get you started on the registration
process for this year’s project. Please read the following project details
carefully and print out a copy for your records.

To be eligible for Best Places to Work, your company must:

• Pay a $250 entry fee to the Business Journal to submit a nomination. (All
participating companies receive an overview of their employee-survey results
from Quantum, whether selected or not.)

• Have an office in the Twin Cities 24-county metro area with 10 or more
eligible employees. Eligible employees are permanent (W2), full or part time,
and have been working at the company for at least 30 days. Owners, temporary
workers or 1099 independent contractors are not eligible to participate. The
24-county metro area includes Anoka, Benton, Blue Earth, Carver, Chisago,
Dakota, Freeborn, Goodhue, Hennepin, Isanti, Le Sueur, Mower, Nicollet, Olmsted,
Ramsey, Rice, Scott, Sherburne, Stearns, Steele, Washington, Wright, Pierce and
St. Croix counties.

-OR-

• If you don’t have an office in the metro area, be a Minnesota-based company
with more than 1,000 employees in the state.

For the awards, we will separate companies into several different categories
based on the number of Minnesota employees.

We will determine honorees using scores from an online and confidential employee
survey. Omaha, Neb.-based Quantum Workplace will be managing the online survey
process and scoring the results. The assessment involves statements employees
must react to on a scale ranging from “Strongly Disagree” to “Strongly Agree.”

Sample statements include:

• Goals and accountabilities are clear to everyone on my team.

• I believe this organization will be successful in the future.

• My immediate manager cares about my development.

• The senior leaders of this organization demonstrate integrity.

• I know I can depend on the other members of my team.

For technical assistance or other questions about the employee survey, please
contact Quantum at 888-415-8302 or bestplaces@quantumworkplace.com.

Company nominations will be accepted through Friday, April 7. Please only submit
ONE nomination per company. The nominator will be the point of contact for
Quantum and the Business Journal throughout the employee survey process.

A Step-by-Step Guide to the 2023 Best Places to Work project:

STEP 1: Nominate your company by filling out the form below. See eligibility
requirements above. Please only submit ONE nomination per company.

STEP 2: Quantum will contact nominators by Monday, April 10 with instructions on
how to proceed with their nomination and upload employee email addresses for
Quantum to contact employees directly about taking the confidential employee
survey. (Note: This step was implemented in 2015 to improve the accuracy of the
employee survey process.)

If you do not receive this email by Tuesday, April 11, please contact Quantum at
888-415-8302 or bestplaces@quantumworkplace.com.

All full- and part-time permanent employees who have been with the organization
at least 30 days and work in Minnesota (as well as Wisconsin’s Pierce and St.
Croix counties) must be invited to take the survey.

The following employees should not be included in your employee count or invited
to take the survey:

• Anyone with more than 5% ownership in the company

• Seasonal, per diem or temporary employees, including interns

• 1099 independent contractors or consultants, including real estate agents

If your parent company is in the Twin Cities 24-county metro area or is based in
Minnesota with more than 1,000 employees in the state, this research project and
survey process should be conducted for the entire parent company and its
subsidiaries. You should be reporting TOTAL Minnesota-based employee numbers and
providing company email addresses for those staffers. A parent company may elect
to not participate in the project as a whole but can then only nominate one
subsidiary for the project.

The online employee survey will be available in English and Spanish.

If you have employees who do not have company email addresses, they can still
take the survey. You have two options: 1) If your employee has access to a
computer (shared or kiosk), they will need a unique code to take the survey.
When you upload employee email addresses, leave the “email address” column blank
for these employees. You will then be able to print a unique code for each of
these employees. 2) Employees without computer access can take paper surveys.
(Note: There is a $2 fee for all paper surveys submitted, which covers the cost
of data entry.)

STEP 3: Online employee surveys must be completed by Friday, May 19. Paper
surveys must be postmarked by Friday, May 12. These deadlines WILL NOT be
extended.

Once your online surveys have launched, you can check back at any time to see
how many have been completed by clicking on the “Monitor Responses” tab. Quantum
also will send daily updates with survey completion rates, and will send
automatic reminders every four days to employees who have not completed the
survey.

Your company’s size will determine how many employees need to complete the
survey to meet eligibility requirements. These percentages have been established
to ensure statistical relevance:

• 10-50 Minnesota employees: 85%

• 51-150 Minnesota employees: 70%

• 151-500 Minnesota employees: 50%

• 501-1,000 Minnesota employees: 30%

• 1,001-3,000 Minnesota employees: 15%*

• 3,001-5,000 Minnesota employees: 8%*

• More than 5,000 Minnesota employees: 5%*

* Companies with more than 1,000 employees can opt out of the email uploads and
use a single survey link instead. However, these companies will have a higher
required response rate than the standard listed above to increase Quantum’s
confidence that the employee data collected through a distributed survey link is
representative. These participation rates are: 1,001-3,000 Minnesota employees:
35%; 3,001-5,000 Minnesota employees: 25%; more than 5,000 Minnesota employees:
15%.

STEP 4: The Business Journal will determine Best Places to Work honorees and
notify them via email by Monday, June 12.

STEP 5: Within three weeks of the employee survey close date, Quantum will send
all participating companies an email containing a link to their survey overview.
(Honoree results will not include external benchmarks until after the Aug. 18
Best Places to Work section is published.) The scores and names of non-winning
companies participating in the project are kept confidential, as are individual
employee survey responses.

Quantum will be contacting all participating companies, offering more detailed
reports for various fees.

STEP 6: Winning companies will be contacted by the Business Journal via email
for additional information and photos, which may be included in the Aug. 18
print publication, as well as online.

Please contact Special Projects & Research Editor Kim Johnson at 612-288-2114 or
kkj@bizjournals.com with questions about the awards.
