STEP-BY-STEP GUIDE To help you have a successful plasma donation experience, please review the information below. Appointment Checklist - What to Bring Proof of Social Security Number (i.e. Social Security Card, W2, Pay Stub) Acceptable forms of identification (must include a photo ID & must bring original copies of documents) • Social Security Card • Driver’s License • ID Card Issued by a Government Agency or other entity (e.g. Military ID Card, School, Employment, State, etc.) • Voter Registration Card • U.S. Military Draft Record • U.S. Military Discharge Form (DOD 214) • Native American Tribal Card • Valid Passport • Bank Statement • Government Assistance Programs • Permanent Residence Card or any document issued by CIS • Laser Visa • Border Crossing Card • Health Insurance Card • Adoption Record • Marriage / Divorce Record • W-2 Form or Pay Stub • Certified Birth Certificate • Hunting / Fishing License *If you do not have one of these forms of ID, please contact your local BioLife center for possible alternative forms of acceptable ID. Answer: 6-8 cups Question: Why does BioLife use fingerprint, and what do we do with the fingerprint and personal information?
