 * HOW DO I RESET MY EMAIL OR PHONE LOGIN CREDENTIALS TO THE MILO CARE+ APP?
   
   
   If you need help changing your access credentials such as EMAIL or TELEPHONE
   number please submit a request CLICK HERE FOR LIVE SUPPORT.
   
   Use the Milo Care+ app to reset your password.
   
   



 * WHICH VACCINES CAN I VIEW ON THE MILO CARE+ APP?
   
   
   COVID-19 VACCINES
   
   ANY VACCINE ADMINISTERED IN THE USA OR CANADA.
   
   IF YOU RECEIVED AT LEAST 1 DOSE OF VACCINE FROM MILO CARE THEN ALL DOSES YOU
   RECEIVED IN FLORIDA WILL NOW BE ACCESSIBLE TO YOU IN THE MILO CARE+ APP FOR
   FREE.
   
   YOUR ENTIRE FAMILY MAY VIEW THEIR RECORDS IN A SINGLE DOWNLOAD OF THE APP.
   
   NEW PATIENTS MUST REQUEST A RECORD UPLOAD FROM MILO CARE SUPPORT. CLICK HERE

 * HOW DO I ADD THE VERIFIED VACCINATION RECORD TO MY SMART PHONE’S WALLET?
   
   
   Open The Milo Care+ app
   
   1. Navigate to the Milo Care + app dashboard home tab.
   
   2. Select a record to add to your wallet.
   
   3. Click on “Add to Apple Health”.
   
   4. Click on “Open in Health”.
   
   5. Add to "Wallet & Health".
   
   

 * HOW DO I ADD MY CHILD'S RECORD TO MY ACCOUNT?
   
   
   A patient must be at least 18 years of age to register an account on the MILO
   CARE+ app.
   
   An adult must create or already have an account and then they can register a
   dependent (younger than 18) to their account.
   
   If your child received at least 1 dose of vaccine from Milo Care then all
   doses they received in Florida will now be accessible to you in the Milo
   Care+ app.
   
    1. IF YOU ARE NOT ABLE TO VIEW THEIR RECORD THEN PLEASE MESSAGE US AT LIVE
       SUPPORT CLICK HERE.
   
   New Patients : If you have never received a vaccine from Milo Care.
   
    1. Login to an adult/parent/guardian account on the Milo care App.
   
    2. Navigate to the Family Member tab on the Milo Care+ app.
   
    3. Click on “Add a family member” and register their name and date of birth.
   
    4. Once the Child is registered, navigate to the Vaccine Report tab on the
       Milo Care+ app.
   
    5. Click on Add/Amend Record on the Milo Care+ app.
   
    6. Or submit a record request for your child from Live Support or CLICK HERE
   
   A child’s vaccination record will be verified and loaded to the the parents
   login.
   
   View all family records on the Milo Care+ app Vaccine Report tab by selecting
   the Patient Name.
   
   

 * HOW DO I ADD AN ADULT MEMBER TO MY ACCOUNT OR CREATE ONE FAMILY ACCOUNT?
   
   
   All patient’s from 18 years are required to register their own MILO CARE+
   account using a unique email address that is securely accessible to them.
   Adults may send each other a request to join a family account.
   
   OPEN THE MILO CARE+ APP
   
    1. Navigate to the Family Member tab.
   
    2. Click on “Add a family member” and register their name, date of birth,
       email and telephone number.
   
    3. The “Add a family member” request is sent by email and in-app
       notification.
   
    4. Join the family by accepting the request in the Alert button in the MILO
       CARE+ app.
   
   Once the “Add a family member” request is accepted, each adult with be able
   to view records for each registered member in the family.
   
   View all family records on the Vaccine Report tab by selecting the Patient
   Name.
   
   

 * HOW DO I ADD A MISSING VACCINATION OR CORRECT AN ERRONEOUS RECORD?
   
   
   Navigate to www.milo.care Support "Missing or Erroneous Vaccination Records"
   and let us know about any missing or erroneous vaccine dose.
   
   If Milo Care administered the dose then the record will be corrected within a
   few business days.
   
   If the error or omission is from another provider, Milo Care will let you
   know about the best way to fix that error.
   
   After we consulte with the DoH, you may be required to ask the provider to
   correct the DoH registry before Milo Care or the DoH is able to verify the
   missing dose.
   
   



 * HOW DOES A NEW MILO CARE PATIENT REQUEST A RECORD UPLOAD TO THE APP?
   
   
    1. A new patient must download the Milo Care+ app and create a profile. APP
       DOWNLOAD HERE
   
    2. Request an Initial Record Upload from LIVESUPPORT.
   
   

 * HOW DO I ADD A NEW VACCINE RECORD?
   
   
   Navigate to www.milo.care Support
   
   "Add a new Vaccination Record" and let us know about when and where the dose
   was administered.
   
   

 * HOW DO I REQUEST A BACK TO SCHOOL DH680 CERT OF IMMUNIZATION?
   
   
    * Keeping track of your child’s shot record has never been easier.
   
    * View required shots for school/child care.
   
   Request a Certificate of Immunization (FORM DH680) for school entry. CLICK

 * HOW DO I REQUEST A VACCINATION HISTORY REPORT?

 * IS THERE A CHARGE FOR THESE SERVICES?
   
   
   FREE: IF YOU RECEIVED AT LEAST 1 DOSE OF VACCINE FROM MILO CARE THEN ALL
   DOSES YOU RECEIVED IN FLORIDA WILL NOW BE ACCESSIBLE TO YOU IN THE MILO CARE+
   APP FOR FREE.
   
   Uploading new vaccine records for new patients takes both time and financial
   resources.
   
   Live Support will asses your request and a fee between $0 and $9.99 may be
   applicable.
   
   In order to verify your record Milo Care must manually access your single
   vaccination record at the Department of Health in your state and then
   transpose it to the SMART® Health Card verified system.
   
   

 * HOW DO I ADD A STATE RECORD TO MY MILO CARE+ APP?
   
   
   Patients that already have a state issued SMART® Health Card can add that
   record to the Milo Care+ app by choosing that free service CLICK HERE FOR
   LIVE SUPPORT.
   
   





 * WHAT IS THE MILO CARE+ APP?
   
   
   THE MILO CARE PLUS APP
   
    * The Milo Care+ app provides Milo Care patients with access to their
      vaccination records.
   
    * The App enables patients to create a SMART® Health Card which is a
      verified record of their COVID-19 test or vaccination.
   
    * The App also provides patients with a QR code to verify their CDC
      vaccination cards.
   
    * You may also request your child’s Certificate of Immunization Form DH680’s
      for school entry.
   
    * Request and download your Vaccination History.
   
   The SMART® Health Card was created by The Commons Project®. The Commons
   Project Foundation is a global tech nonprofit building solutions that empower
   people to access, manage, and share their data.

 * WHAT IS A SMART® HEALTH CARD?
   
   
   SMART Health Cards are verified versions of your clinical information, such
   as vaccination history or test results. They allow you to keep a copy of your
   important health records on hand and easily share this information with
   others if you choose. SMART Health Cards contain a secure QR code and may be
   saved digitally or printed on paper.
   
   You can get a Covid-19 SMART Health Card through a qualified issuer, such as
   Milo Care. An issuer is any organization authorized by the Verifiable
   Clinical Information coalition (VCI) to generate these cards, including
   pharmacies, hospitals, healthcare providers, medical labs, public health
   agencies, and more.
   
   

 * WHO IS MILO CARE ?
   
   
   Milo Care is the clinical services division of One Milo, Inc (“Milo”).
   
   Milo developed a Connected Health Ecosystem powered by proprietary systems
   for point-of-care and self testing diagnostics. The system delivers precision
   medicine to mass populations for the purposes of bringing clinical and
   economic improvements to patient care.
   
   PANDEMIC RESPONSE
   
   In response to the public health emergency (PHE) caused by the COVID-19
   pandemic many cities in South Florida asked Milo to provide COVID-19 testing
   and then vaccination services. We opened drive through testing centers and
   vaccination sites in partnership with many cities. In so doing we performed
   hundreds of thousands of COVID-19 tests and vaccinations.
   
   RECOGNITION
   
   One Milo won the NASAiTech’s Juried Cycle III in 2018 and Ignite The Night @
   The Space Symposium, NASAiTech is an initiative by NASA’s Space Technology
   Mission Directorate (STMD) and aims to find innovative ideas that have the
   potential to overcome critical technology hurdles facing future exploration
   of the Moon and Mars, even though many were originally meant to solve
   important problems here on Earth.
   
   Generali, the World’s second largest insurer scouted One Milo from over 600
   companies as one of four companies to represent The Future of Health and
   Welfare.
   
   One Milo joined the Generali Health & Welfare Accelerator managed by H-Farm.
   H-FARM SpA is the innovation platform that guides companies in their digital
   transformation, provides education programs and supports startups. H-Farm
   integrates business consultancy, education and investments into a synergic

   Origin
   
   Milo was incorporated as a Delaware Corporation in 2018, with the closing of
   a Seed Investment Round. Milo was born out of a convergence of IP represented
   by hundred’s of diagnostic chemistries developed over 30 years and the
   maturing of IoT technologies & smart ecosystems. One Milo, Inc combined the
   IP for chemistry and smart system into its domain at inception.
