Learn About FMT Treatment Options JUMP-TO NAVIGATION JUMP TO * Join the OpenBiome Clinical Network * Registration Instructions * Place an Order * FAQ * Invoice Payment STEP 1: JOIN THE OPENBIOME CLINICAL NETWORK Before placing an order, your practice or medical facility must complete a registration form and return it to our clinical outreach team. We are required to record every shipping location, including individual facilities within the same health system. Registration automatically adds you to the OpenBiome Clinical Network Separate registration forms are required for FMT preparations manufactured by OpenBiome and those manufactured by the University of Minnesota (UMN). If you are a registered OpenBiome clinical partner, you still need to complete the UMN-specific registration form to access FMT treatments prepared by that institution. Once your registration is complete, you will be issued a partner ID number. Not sure if your facility has registered or forgot your partner ID? Contact us at info@openbiome.org Register For UMN-Manufactured FMT DETAILED REGISTRATION INSTRUCTIONS The registration form is a legal document that must be completed in its entirety and with no alterations to the source text. Below are guidelines for completing your Clinical Partner Registration Form: 1. The clinical team may complete the information on pages 1-2. 2. The terms on pages 3-5 must be reviewed by an individual authorized to purchase investigational drugs on behalf of your institution. (Clinicians are NOT generally authorized.) 3. The initials on pages 3-5 must match the signature on page 6. 4. When completed, return the form to us at info@openbiome.org or by fax to 617-575-2201. 5. An Alternative to Registering - Contracting & Purchase Agreements: OpenBiome is happy to discuss entering into a purchasing contract with your facility or health system. STEP 2: PLACE AN ORDER All OpenBiome clinical partners are eligible to place an order for FMT preparations for any identified patient that meets the eligibility criteria for FMT under the FDA policy of Enforcement Discretion and to keep a stock of FMT preparations at their facility. Submit orders by email to orders@openbiome.org or by fax to 617-575-2201. Use the OpenBiome order form or an internally generated purchase order if preferred. Place An Order FREQUENTLY ASKED QUESTIONS How do I check my order status? An order confirmation email will be sent to the purchaser within 1 business day of receipt. Similarly, a shipment confirmation email including a tracking number will be sent the day before scheduled delivery. If you do not receive these confirmation emails, contact us at orders@openbiome.org or call +1-617-575-2201, option 2. When will I receive my order? Under OpenBiome’s standard shipping and handling options ($150), orders submitted Monday-Wednesday will be shipped the following day. Orders placed on Thursday and Friday will be shipped the following Monday. Treatments are packaged on dry ice and shipped using UPS Next Day Air for an approximate 10:30 AM delivery. Optional same-day shipping: For an additional $50, orders placed before 3 PM EST Monday-Thursday may be scheduled to ship the same day that OpenBiome received your order. Availability is not guaranteed. Optional early morning delivery: For an additional $100, your delivery will be scheduled for approximately 8:00 AM (instead of 10:30 AM). This option is not available in all ZIP codes. How are FMT preparations shipped to my facility? FMT preparations are shipped via UPS Next Day Air on dry ice in a temperature-monitored container to validate cold chain. For more information, see our Storage Controls & Material Specifications for UMN- and OpenBiome-manufactured FMT material. What are OpenBiome’s shipping terms? Our shipping terms are FOB: Destination, Prepaid & Add. We do not bill 3rd party. How will we be billed? An invoice will be issued by email to the contact on file for your account. OpenBiome's default terms are Net 30 days. If you require a paper invoice by mail or need a W-9 form to remit payment, contact our Accounts Receivable team at billing@openbiome.org. Payment by check remit payment to: Microbiome Health Research Institute, Inc. 2067 Massachusetts Avenue, Third Floor Cambridge, MA 02140 Payment by ACH & credit card: We accept payment by direct deposit and credit card.
