        <li><strong>Description of Sweepstakes</strong>. You may enter the sweepstakes by completing the 2021 Global Healthcare Exchange, LLC ("GHX") Customer Satisfaction Survey online, following the link in your invitation. No other form of entry
          will be accepted. Only one entry per person is permitted. Odds of winning depend on number of entries received. Total number of eligible participants approximately 1,500.</li>
        <li><strong>Deadline</strong>. Entries will be accepted as soon as the survey is opened to customers. All entries must be received by 11:59 p.m. MST on October 15th 2021.</li>
        <li><strong>Prize</strong>. A total of two (2) winners, representing one (1) provider customer and one (1) supplier customer of GHX, will be randomly selected on or about November 1, 2021, from eligible entries received in each category. Each
          winner will be awarded one (1) complimentary registration to the 2022 GHX Summit, with an estimated retail value of $1,300.00. In addition, the winner in the provider customer category will be awarded travel and hotel to the 2022 GHX Summit
          by booking through the GHX travel agent, with an estimated retail value of up to $1600.00. Because the prize relates to attendance at a scheduled event, major restrictions apply to its use, availability, and receipt. The prize is not
          transferrable except as otherwise described in these rules, and may not be redeemed for cash or converted to another form. The winner may be required to sign and return an affidavit of eligibility and provide GHX with valid identification
          within seven days of being contacted. The individual winner and his or her employer are responsible for determining whether the individual may accept the prize in compliance with applicable law and employer policy, and for determining which
          of them bears the tax obligations and any other obligations associated with the prize. If a winner is unable or fails to accept the prize under the conditions of these official rules, GHX may select a new winner from eligible entries. Prize
          winner shall be chosen and notified by email on or about November 1, 2021.</li>
        <li>NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN THIS SWEEPSTAKES, ALTHOUGH THE WINNER MUST BE CURRENT EMPLOYEE OF CUSTOMERS OF GHX. PURCHASING ANY ADDITIONAL GOODS OR SERVICES WILL NOT INCREASE THE ODDS OF WINNING.</li>
        <li><strong>Eligibility and Limitations</strong>. Sweepstakes is only open to U.S. residents, 18 years of age or older. Sweepstakes void in Puerto Rico, Rhode Island, U.S. territories and possessions and where prohibited by law. Sweepstakes
          is limited to current employees of GHX's provider customers. Entry will be deemed made by the holder of the customer email account to which GHX sends the invitation to complete the 2021 GHX Customer Satisfaction Survey. Employees of GHX,
          its subsidiaries and affiliates, or Customer Care Measurement &amp; Consulting, and the immediate family members of such employees, are not eligible to participate or win. The term "immediate family" includes spouses, siblings, parents,
          children, grandparents, and grandchildren (whether by blood, marriage, adoption, co-habitation, or other family extension) and any other persons residing at the same household whether or not related. GHX and its affiliates assume no
          responsibility for any error, omission, interruption, deletion, defect, or delay in operation or transmission; communications line failure; theft or destruction of or unauthorized access to promotion entries or entry forms; alteration of
          entries or entry forms; or the processing of the entries in this sweepstakes. If for any reason (including without limitation tampering, unauthorized intervention, fraud, or any other cause beyond the control of GHX, which corrupts or
          affects the administration, security, fairness, integrity, or proper conduct of this sweepstakes), the sweepstakes is not capable of being conducted as described in these rules, GHX shall have the right, in its sole discretion, to cancel,
          terminate, modify, or suspend the sweepstakes.</li>
        <li><strong>Release</strong>. Except where prohibited by law, the winner's entry and acceptance of the prize constitutes permission for GHX to disclose that winner's name in connection with any announcement of the winner or similar
          communication without further compensation or permission. By participating, each participant and the winner waives any and all claims of liability against GHX, its affiliates and their employees and agents, regarding the prize or for any
          personal injury or loss, which arise from or relate to the conduct of, or participation in, the drawing, or from the use of the prize. Except where prohibited by law, submission of the survey constitutes your consent to GHX's use of your
          comments in connection with its internal and external customer satisfaction programs. GHX will not disclose to third parties your name or your employer's name in connection your survey comments without your specific approval; provided that
          GHX will release the winner's name (without associated survey comments) as described below under "Winner Announcement." Use of the prize may be subject to restrictions, fees, and deadlines, and by accepting the prize, the winner releases
          GHX from any and all liability for any resulting losses, payment, or damages.</li>
        <li><strong>Taxes</strong>. The value of the prize may be reported for tax purposes as required by law. The winner, whether an individual employee or his or her employer, is solely responsible for reporting and paying any and all applicable
          taxes and fees.</li>
        <li><strong>Decisions</strong>. GHX reserves the right to disqualify any participant or winner and may refuse to award any prize to a person who is ineligible or has violated any rule, gained an unfair advantage in participating in the
          drawing, or obtained winner status by fraudulent means. Further, GHX will resolve any disputes, conflicting claims, or ambiguities concerning the rules, and GHX's decisions in such matters shall be final. The Company may waive any of these
          rules in its sole discretion. All claims must be resolved under Colorado law without regard to choice of law rules by arbitration in Denver, Colorado, under the rules of the American Arbitration Association.</li>
        <li><strong>Winner Announcement:</strong> The name of the winner will be available after December 1, 2021 by writing to the following address and enclosing a self-addressed, stamped envelope addressed to Global Healthcare Exchange, LLC, 1315
          W. Century Drive, Suite 100, Louisville, CO 80027, Attn.: Laura Mullen. Please indicate "Winner 2019 Customer Satisfaction Survey" on the outer envelope.</li>
        <li><strong>Miscellaneous</strong>. GHX may substitute a prize of equivalent value, amend these rules, or discontinue the sweepstakes at any time. All entries become the exclusive property of GHX and will not be acknowledged or returned.</li>

2021 GHX CUSTOMER SATISFACTION SURVEY SWEEPSTAKES
OFFICIAL RULES

 1.  Description of Sweepstakes. You may enter the sweepstakes by completing the
     2021 Global Healthcare Exchange, LLC ("GHX") Customer Satisfaction Survey
     online, following the link in your invitation. No other form of entry will
     be accepted. Only one entry per person is permitted. Odds of winning depend
     on number of entries received. Total number of eligible participants
     approximately 1,500.
 2.  Deadline. Entries will be accepted as soon as the survey is opened to
     customers. All entries must be received by 11:59 p.m. MST on October 15th
     2021.
 3.  Prize. A total of two (2) winners, representing one (1) provider customer
