HOW A VASECTOMY WORKS


When you have a vasectomy, the two vas deferens are cut and clipped to keep
sperm from traveling from the testes to the penis. The testes still produce
sperm, but since the sperm have nowhere to go, they are harmlessly absorbed by
your body. Your prostate and seminal vesicles still produce fluids, so your male
hormone level remains the same, so your hair distribution, the pitch of your
voice and your sexual drive will not change.



VASECTOMY EXPERT



Our vasectomy experts will guide you through the procedure with the use of
videos and audio-visual aids. A vasectomy normally works by surgically
preventing sperm from entering a man’s semen. At our vasectomy clinic in
Georgia, this process is usually done either by blocking the tubes that carry
sperm from the testicles to the penis, or by sealing them with heat. It is
usually a very short surgery – a total of 15 minutes on average – and is carried
out under local anaesthesia where the patient will be awake. Our Georgia-based
vasectomy clinic usually advises that in preparation for the procedure, it is
vital that anti-inflammatory drugs such as Ibuprofen, paracetamol etc. are
avoided at least two weeks prior to the surgery and the scrotum area must be
washed thoroughly in order to prevent infections.

It is also very important to be aware that although a vasectomy is successful at
preventing pregnancies, you will still be susceptible to various sexually
transmitted diseases and infections. Therefore, here at Vasectomy Clinic
Georgia, we urge you to practise safe sex and use condoms. Even after the
surgery, continued use of contraception is advised.

Nearly 12 weeks after the surgery, the patient will be required to supply a
sample of sperm for testing to confirm that the procedure was successful. In a
few cases, some men may require two tests to ensure that their sperm count has
diminished.

PREPARING FOR YOUR VASECTOMY





Dr. Perlow will discuss the vasectomy in detail with you and answer all your
questions prior to the procedure.

For the week prior to:

Avoid Aspirin for one week prior to the procedure and aspirin like products
(Advil, Aleve, etc.) for 48 hours prior to the procedure.  Be sure that you
speak to us about any blood thinner medications that you are taking such as
clopidogrel (Plavix) or warfarin (Coumadin)….such medicines may require stopping
48 hours to one week prior to the procedure.

For the day of :

 1. Shave your entire scrotum (the sac containing the testicles) before coming
    to the office. Please shave well for your own benefit.  Shave the entire
    scrotum (including the sides) up to the top side of your penis.  Shower
    before coming to the office and wear comfortable clothing.
 2. Bring an athletic supporter (“jock strap”) with you to wear home after the
    procedure.
 3. As a standard precaution, it is a good idea to have someone drive you home
    after the vasectomy.

POST VASECTOMY INSTRUCTIONS

The most important thing for you to do is to stay off your feet and take it
easy.  You should take it particularly easy for 24-48 hours.  Applying an ice
pack intermittently the first day is recommended as well as wearing an athletic
supporter for 5 days.  Tylenol, Advil or Aspirin should be adequate to control
any postoperative pain.   If not please let Dr. Perlow know.

You may get some blood spotting–  this is not a problem.  You may also get some
bruising on your scrotum or penis- – this is not a problem.  You may get a
“knot” near the site of the vasectomy– this also should not become a problem.

You may shower the day after your vasectomy.  Be sure to dry yourself  well.  It
the tiny band-aid comes off, do not be concerned.  Otherwise, remove it in
several days.  Resumption of work and activity is variable.  See how you feel,
but wait at least 48 hrs.  If you wish to work sooner than 48 hours, just be
sure you remain off your feet.  Wait at least 7 days before resuming sexual
activity or recreational exercise.  If you have any questions or problems,
return for a post-op check.

Otherwise, call the office in 6 weeks for a time to return to the office with a
semen specimen to verify sterility.

Please bring specimen within 2 hours of ejaculation.   Remember that you are not
sterile after your vasectomy until your semen has been verified to be void of
any sperm!  So be sure to take appropriate contraceptive measures until that
time.

1st sample to be brought in at 6 (six) weeks
2nd sample to be brought in at eight (8) weeks or ten(10) weeks,  depending on
findings on first specimen

Please call the office before bringing in your samples to be sure Dr. Perlow is
present to read your sample.




IMPORTANT CONSIDERATIONS




EFFECTS OF VASECTOMY



A vasectomy does not affect the Leydig cells in the testicle that produce the
male hormone, testosterone. It does not produce any change in sexual drive or
ability to obtain erections. No difference in the amount of ejaculation fluid
will be noticed. The two vas deferens are now cut and clipped so that sperm can
no longer reach the penis, rendering the man sterile. Although a vasectomy
reversal or sperm retrieval procedure could be performed at a later date
following a vasectomy, these procedures are expensive, generally not covered by
insurance, and have a significant failure rate ….therefore, a vasectomy should
be viewed as a permanent procedure.


RISKS OF VASECTOMY



The primary risk to vasectomy is temporary discomfort after the procedure. This
is usually minor and well controlled with Aspirin, Tylenol or Aleve. Most
patients respond well.  For more severe pain, a stronger pain medication can be
prescribed.  Most men do not require this. This is a minor procedure, and
significant bleeding or infection requiring additional surgery is rare.

Recanalization is a phenomenon whereby the two ends of the vas deferens on
either side can grow back together following the vasectomy and result in sperm
in the ejaculate with the ability to father children. This is rare. In general,
the failure rate with vasectomy is 1 in 2000.

A sperm granuloma is a lump that can develop above the testicle following
vasectomy and can cause pain.  This is an inflammatory reaction where the vas
was divided.  When noted, it usually occurs 1-2 weeks following the vasectomy. 
It is treated with anti-inflammatory medications and invariably goes away
several weeks later.  Dr. Perlow has never had a patient who required surgery
for this condition.

Lastly, there is a condition known as Post-Vasectomy Pain Syndrome with chronic
testicular or groin pain, which is uncommon and Dr. Perlow has rarely seen this.
There is a difference of opinion in the medical literature regarding how often
this occurs. The American Urological Association suggests it occurs in 1-2% of
patients, but Dr. Perlow has not seen numbers anywhere close to this, after
thousands of vasectomies performed.
