Welcome to the Arisa Health benefit resource portal.  You will find valuable
benefit information along with how to enroll in your benefits using
the EMB Enroll online benefits enrollment system. Please take some time to
review your benefit guide, along with the brochures and videos on this page.

OPTIONS TO ENROLL

Self Enrollment - Enroll in your benefits using any computer with internet
access. Click on the button below. You will be redirected to your online
enrollment portal, follow the instructions to log in, and proceed with your
elections for the new plan year.

Mobile App - Enroll in your benefits using the EMB Mobile App.

Voluntary Benefits are designed to provide you additional cash to assist with

ACCIDENT INSURANCE

Offsets the cost of unexpected expenses that result from accidents that occur
everyday. It also features a Wellness Benefit payable every year while coverage
is in force.

Provides immediate financial relief from overwhelming expenses of a serious
illness. It pays a lump-sum cash benefit when you are diagnosed with a covered
illness easing your financial worries.

NEW HOSPITAL INDEMNITY PLAN

Provides a daily fixed indemnity benefit for eligible hospital confinements.
Employees can use the benefit as they choose – for instance, to help offset
copays, coinsurance or deductibles that may be tied to hospitalization or lost
time from work.
