private sewage disposal system contractors (annual): $125 * Non private public
swimming and wading pool permit (annual): $100 * Certified 5x7 or wallet Birth
Certificates: $8 * Certified Death Certificates: $8 * Genealogy searches with
hard copy: $4 * Food service establishment and retail food permit (annual): $70
* Reinstatement of food permit after suspension: $70

plan review (prior & after construction): $25 * Administration fee for adults &
children private stock vaccines: $20 * Administration fee for all State funded
vaccines (VFC & 317): $8 * Adult Vaccines: Offered at current cost plus
administration fee * TB testing: $10 * Tattoo parlor operation permit (annual):
$250 * Tattoo artist permit: $50
