ELECTRONIC RECORD AND SIGNATURE DISCLOSURE

Please read the following information carefully. This disclosure is being
provided to you pursuant to applicable law, which requires that we provide
certain disclosures and written information to consumers prior to their use of
electronic signatures or electronic records. SecureDocs, Inc. is providing this
electronic signature system on behalf of its customers, the “sending parties”,
who are sending electronic contracts, notices, disclosures to you or requesting
electronic signatures from you. By proceeding and electronically signing this
document you are agreeing: (i) that you have reviewed the following consumer
disclosure information and consent to transact business using electronic
communications, (ii) to use electronic signatures in lieu of paper documents,
and (iii) to receive notices and disclosures electronically. If you do not
agree, please click “Decline” below.

REQUESTING PAPER COPIES

You are not required to receive documents, notices or disclosures or sign
documents electronically. At any time, you may request paper copies of any
record provided to you electronically by submitting a written request to the
sending party, or for as long as you are an authorized user of the SecureDocs
Electronic signature system you will be able to download and print those
documents using the options provided in the system. The SecureDocs system will
also email you a copy of any document you sign using the Electronic Signature
system as soon as the document has been fully executed. If you prefer to receive
paper rather than electronic copies of documents you can request a paper copy by
emailing the sending party and including your name, email address, postal
address, phone number, and specific request for the contract. Please note that
the sending party may apply a charge for requesting paper copies.

WITHDRAWING YOUR CONSENT

If you decide to receive documents, notices and disclosures electronically, you
may change your mind at any time, withdraw your consent and notify the sending
party that you want to receive documents, notices and disclosures in paper
format only by following the process below. Once you have withdrawn your consent
you will no longer be able to use the SecureDocs Electronic Signature system to
receive electronic documents, notices or disclosures. However, you may at any
time in the future decide to once again utilize the SecureDocs Electronic
Signature system and upon doing so, you are once again consenting to receive
notices, disclosure, or documents electronically.

SCOPE OF CONSENT

Unless you have withdrawn your consent, you agree to receive electronically all
required notices, disclosures, authorizations, and documents that are required
to be provided or made available to you during the course of your relationship
with the sending party.

PROCESS FOR WITHDRAWING CONSENT

To indicate to the sending party that you are withdrawing your consent to sign
documents electronically or receive notices and disclosures electronically, you
may decline to sign a document sent through the SecureDocs Electronic Signature
system and select the “Withdraw Consent” box on the screen. Any fees associated
with sending paper copies or withdrawing consent will be determined by the
sending party. You should print this disclosure, send or save to a location
where you can retrieve this in the future if needed. You can also access this
disclosure any time through the SecureDocs Electronic Signature system.

HARDWARE AND SOFTWARE REQUIREMENTS

Use of the SecureDocs Electronic Signature system requires a standards-compliant
web-browser which supports the HTTPS protocol, HTML, javascript, and cookies.
Viewing PDF documents requires Adobe Acrobat Reader.
