HEALTH COACHES

The Health Coaches at HealthYou are deeply committed to providing and promoting
a culture which encourages healthy lifestyles and supports both you and your
family members throughout your wellness journey. It is important to talk with
your doctor before making any recommended changes in your physical activities,
diet, or other lifestyle adjustments. Your health coach will help transition you
through a process of understanding your health risk scores and will work with
you to establish a program to help you achieve positive changes in your overall
health and quality of life.

WELLNESS COACHING

As a participant in the Leprino Foods Wellness Coaching Program, you are hereby
giving your consent to voluntary health coaching, as recommended, based upon any
identified risk factor(s) from your biometric health screening.  You also
understand and agree that the results of health coaching are for informational
purposes only and should NOT be relied upon as a conclusive health measure or
definitive diagnostic session.  Your consent to coach waives any and all claims
against HealthYou, its employees, associated organizations and volunteers from
any and all potential liability arising from or in any way connected with health
coaching or any of the data and/or reporting derived therefrom.

WHAT TO EXPECT DURING A HEALTH COACHING SESSION

Sessions last approximately 20 minutes and will be conducted either by telephone or virtually via Microsoft Teams
[a technological video conferencing operating application].  You will receive an
email/text message reminder 24 hours prior to your session.   

Your Health Coach will support you in establishing and meeting your health and
wellness goals. It is very important to your success that you keep your
scheduled sessions with your assigned coach.

There is a 24-hour cancellation policy. Failure to cancel 24 hours prior to your
session will result in it being designated as a “missed session” by your Health
Coach. Three (3) missed sessions during the program year will result in your
dismissal from the health coaching program.

NOTICE REGARDING WELLNESS COACHING

IF YOU ELECT TO PARTICIPATE in voluntary coaching, information will be coded so
your identity (name, Social Security Number, date of birth, or other unique
identifiers) cannot be known by outside parties.  This is known as
de-identification. Some of this de-identified information, including aggregate
reports of overall population health data, may be shared with outside parties
for the purpose of evaluating and planning various employee health and wellness
activities and programs.

By participating in voluntary health coaching you are giving permission for the
collection of your personal health information by the HealthYou staff AND
permission to disclose ONLY anonymous, coded, de-identified data for review by
outside parties.  You also agree to the purpose of collecting this health
information and understand the strict confidentiality of your individual
results. 

You may review the HIPAA PRIVACY NOTICE (Notice of Privacy Practices) describing
how your health information may be used or disclosed. This Notice can be

should read this Notice carefully and be aware the Notice may be changed at any
time.
