lululemon’s Annual Open Enrollment for all Part-Time Employees is May 6 – 24!

New Hires can enroll 30 days from hire date, or the date Hooray Health receives
their information from lululemon. If you moved from Full-Time to Part-Time, you
can enroll up until 30 days after you lose full-time coverage. If you have
another Qualifying Life Event such as loss of other coverage or family/life
change,, please submit a Contact Us Form and we will assist with your

needs exceed beyond the reach of telemedicine, our Hooray Health Nationwide
Network of Urgent Cares and Retail Clinics are available. You can find your

or the provider lookup on the Hooray Health mobile app. Visits to Hooray Health

A Member will pay a $25 copay for a visit to a retail clinic or urgent care
center that is in the Hooray Health network. The Member will not receive a
balance bill or additional charges for covered procedures at Hooray Health
Network Providers.

What is the cost of a telemedicine visit?

Hooray Health has an unlimited telemedicine service available to Members
24/7/365 and has a $0 consult fee every time. Members can speak to a physician
at their convenience, and if needed, their prescriptions can be called into a
pharmacy of their choice.

Where can I use my prescription discount?

Prescription discounts can be utilized at thousands of pharmacies across the
country and can be located using the Hooray Health App.

How can I tell if a provider is in the Hooray Health Network?

Providers can be found using the “Provider Look-Up” feature on the Hooray Health
App, by contacting our Medical Concierge at (866) 646-6729 or by going to
myhoorayhealth.com/providers.

Are Hooray Health Plans considered comprehensive health insurance plans, i.e.
major medical plans?

No. Hooray Health plans are categorized as fixed indemnity insurance plans. The
plan pays a pre-determined amount on an annual basis, regardless of the total
charges incurred. The accident insurance reimbursed medical expenses solely in
connection with accidental injuries.

Where can I download the Hooray Health App?

Search “Hooray Health” on the Apple App Store or Google Play Store.

What is the First Health Network, and how do I determine a provider's
eligibility?

The First Health Network is another network available to all Hooray Health
Members if you want or need to see a provider outside of the HH network, a PCP,
Specialist or have a critical injury or illness. The First Health Network is a
national PPO network of over 5,500 hospitals, 115,000 ancillary facilities and
over 1.5 million health care professionals. Members receive an average of 32-52%
savings at these locations. To search for a provider, click here, use the Hooray
app, or copy/paste myhoorayhealth.com/providers.

What is the waiting period for an employee to receive benefits? (Varies by
Employer)

All employees who are specified to be eligible by your company may enroll in
benefits during lululemon’s Open Enrollment period. New hires may enroll within
30 days from their hire date. Benefits will be effective the first of
the following month.

What are the age requirements for a Hooray Health Plan?

The Hooray Health plans are available to anyone between the age of 18-64.

Can members cancel the plan at any time?

Yes. However, they will not be eligible for the plan again until the
next company open enrollment or a qualifying life event.

*No balance bills apply for covered services with contracted Hooray Health
Network Providers. Network restrictions may apply in some states.
