For those needing hospice care, we provide a specialized team for patient and caregivers. Our Levine Home Inpatient Care Unit at our Dementia Care and Education Campus helps manage symptoms when 24/7-short-term acute care is needed. The Campus is a first-of-its kind innovative community resource, providing comprehensive care for people with all types and stages of dementia, as well as support for their family members and caregivers. Hospice Program Benefits For the Patient * The hospice team includes: physician, nurse, social worker, nurse's aide, chaplain, and volunteer. * We provide all supplies, equipment and medications. * We come to the patient’s home, group home, assisted living facility or skilled nursing facility. * Our inpatient units treat symptoms requiring 24/7 care. For Family Members * Care is tailored to each family’s goals of comfort and quality of life. * We provide breaks for caregiver relief. Patients can stay up to five days in our Levine Home Inpatient Care Unit. * Family members are supported with grief counseling after a death. Dementia Care and Education Campus We primarily provide comfort and dignity to people living with dementia in the comfort of their own homes. However, patients who need more intensive care can stay at our Levine Home Inpatient Unit. It is located at our Dementia Care and Education Campus in central Phoenix and staffed with loving caregivers and volunteers, who have special training in dementia care. Hospice care is covered by Medicare and most insurance companies. If it’s too soon for hospice, our Supportive Care for Dementia program may provide you with the support you need.
