If this is related to incorrect information in your medical record, please visit
www.valleyhealth.com/patients-visitors/health-information-management to complete
a Request for Amendment Form or a Demographic Correction Request Form. If you
need a copy of a part of your medical record not available on the patient
portal, please complete an authorization form located on
www.valleyhealth.com/medicalrecords, which will be processed in the order it is
received.
