   clinical team monitors your readings to ensure you're on the best medication,
   and your health coach will provide support for building healthy lifestyle
   habits.
   
   
   
   
 * You will still need to visit your provider for routine visits and all other
   care.


DO I HAVE TO VISIT MY DIGITAL MEDICINE CARE TEAM IN PERSON?

 * You can participate in Digital Medicine from the comfort of your home—no
   office visits are required.
   
   You will need to submit regular readings and talk with your Digital Medicine
   care team to participate in the program.


IS SOMEONE MONITORING ME 24/7?

 * While we are not a 24/7 monitoring service, your Digital Medicine care team
   will regularly review your readings and reach out to you if you are starting
   to trend off course. They will call to help you manage your blood pressure
   and/or Type 2 diabetes over the phone. Your clinician can adjust your
   medications, and your health coach will help you build healthy habits.
