The DC Health Matters Collaborative - a unique collaboration among DC hospitals
(Children’s National Hospital, Howard University Hospital, and Sibley Memorial
Hospital) and four community health centers (Bread for the City, Community of
Hope, Mary’s Center, and Unity Health Care) - authored this community health
needs assessment to serve as a community-driven foundation for our community
health improvement efforts.
