As a member of the CommonTrust Network, CDR Health is now issuing SMART® Health
Cards to give our customers an easy way to access, store, and share digital
versions of their vaccination records. Log in to your account to access your
SMART® Health Card today!

How to access, save, and store your SMART® Health Card QR code:

1. To access the QR code, log in to your account on the patient portal, select
"Vaccination", select "My VVR", and the QR code will appear.
2. You can scan the QR code directly from the screen or download and save it as
a PDF.

You must "Create an Account" in order to schedule your vaccination appointment.
Once you have created your account, you still need to schedule your vaccination
appointment and complete the medical screening questionnaire by clicking "GET
VACCINATED" on the homepage of your account screen.
