SYSTEMS-BASED LEARNING

Engaging a top-down and bottom-up approach, PRIME's unique learning models
empower healthcare teams and systems to reflect on and react to effective
workflow processes, pathways, algorithms, and best practices that improve care
quality and patient outcomes. Clinicians, administrators, teams, and patients
are engaged in a community of learning to design, react to, benchmark, monitor,
and measure action plans based on group interventions. Designed to advance
collaborations, PRIME's systems-based learning interventions have demonstrated
sustainable and scalable behavioral change.


QUALITY IMPROVEMENT

As essential tools in QI programs, quality measures enable clinicians and
healthcare organizations to quantify the proportion of patients who are managed
according to evidence-based recommendations and guidelines. While conducting QI
educational programs using core measures since 2006, PRIME launched its
integrated quality of care education (iQCETM) model in 2013. The goal of iQCE is
to leverage real-world data analytics with patient and provider tethered
engagement to achieve sustainable and scalable educational impact. Our results
have been published in numerous peer-reviewed publications demonstrating
positive impact on the healthcare ecosystem.

With over 25 years of experience providing interprofessional continuing
education (IPCE) across federal, military/VA, payer, provider, academic, and
other health care settings, PRIME engages clinicians, patients, healthcare
teams, and systems in improving care coordination to achieve sustainable

centers, institutional centers of excellence, and entire health systems in
