UroCareLive, a PlatformQ Health network learning channel, takes your privacy
very seriously. This Privacy Policy describes (i) the types of information we
collect from you when you visit our Site; (ii) how we use the information we
collect; (iii) with whom and why we share it; and (iv) the choices we offer
regarding our use of the information. We also describe how to contact us about

communicating with program participants and awarding CME credit, which is
granted through an ACCME accrediting provider.

Information Collected Through Technology. We collect certain information by
automated means, such as cookies and web beacons, when you visit our Site. The

on our behalf.

At any time, you may opt out of receiving messages by unsubscribing from our
e-mail list. Your opt-out will apply prospectively, but will not apply to third

emailing support@PlatformQ.com with the subject line “Data Request”. If you
would like to remove your personally identifiable information from our database
you may do so by emailing support@platformq.com with the subject line “Data
Removal Request”. PlatformQ Health will confirm when your personally
identifiable information has been removed and at that point you will no longer
have access to the PlatformQ Health learning channels.

Information About Other People

On some pages, you can submit information about other people. For example, you
may be able to provide email addresses of friends who may be interested in
PlatformQ Health. In these cases, we will use this information to contact these
