Thank you. Pick Up Locations and Procedures (You must be prepared to present your ID and/or your Vehicle Tag.) Carline on the front circle drive begins at 3:30 and it is set up for students registered in any component of Wolf's Den. Extended Day and Enrichment: All Extended Day Options (Full Time, Half Time, Occasional) are closed. We are full to capacity. For all currently registered in any of these options, services continue until the last day of school, Wednesday, June 5, 2024. Remember to submit payment for January 2024 Extended Day by December 15. Be sure to purchase this at the time you purchase January 2024. Failure to pay will forfeit any Spring 2024 Enrichment Class registration you may submit during the month of December. Spring Enrichment Classes begin Monday, January 22 and end Friday, May 24. Registration ONLY for Wolf's Den Spring 2024 Enrichment Classes begins Monday, December 4 and ends Friday, January 12 via schoolpay. We are not accepting any new registrations for any component of Extended Day (Full Time, Half Time, OR Occasional Ext Day.) For all currently registered, be sure you have a zero balance on your current account before you register for new classes. If you need a statement, email us: cewolfsden@gmail.com Enrichment Classes (Registration is on a first come first serve basis.): Registration for Spring 2024 Enrichment Classes will run from Monday, December 4 - Friday, January 12. Also, the final deadline to add or drop is Friday, February 2. After February 2 there are no refunds should you decide to drop after February 2. You must email us should you wish to drop a class by Friday, February 2. Your email is the official notice you are withdrawing. You will be charged only for the day(s) your child attends any class. After February 2 there are no refunds or prorations. Upon successfully making your purchases, you will receive an email from schoolpay confirming your purchases. The email confirmation is proof you did successfully make your purchases and that your child has a spot in the class. Wolf's Den does not send you any emails confirming your purchases. Thus, be sure to note schoolpay's confirmation via email sent to you. Your payment for any items immediately is your confirmation you have successfully registered. Withdrawing from Extended Day If you wish to withdraw at any time, you must request in writing to be withdrawn from the program. You will not be charged for the next month from the date you withdraw from Full Time/Half Time/Occasional. However, as stated above, after February 2, all Enrichment Classes are nonrefundable. Thank you. Registration for Spring 2024 Enrichment Classes: NOTE Items that must be filled out and purchased at the time of registration for Spring 2024 Enrichment Classes: 1. If you are not currently registered, you must fill out The REQUIRED Wolf's Den Registration Form and Agreement 2023-2024 (please read all info on this item and there is no charge). 2. For all registering in a class you must purchase the Registration Fee: A. $15.00 December 4-December 31 B. $25.00 January 1-January 12 3. Purchase the class you add to your cart. LATE PICK UP: I understand that I must pick up my child by 6:00 PM if enrolled in Full Time; 5:00 PM if enrolled in Half Time. If my child is picked up after 6:00PM I will be assessed a late fee of $2.00/minute. When I am late picking up my child from Half Time I will automatically be issued a 5 Unit Occasional Card which will provide supervision until 6:00 PM. One Unit from this card will be used for the late pick up and I will have 4 Units remaining. After the 5th late pick-up I may be asked to make other arrangements for After School Child Care. The official time for our after school program will be gauged by the clock located in Pick UP Location in the school. Please Note: If late pick-up fees are not paid within 24 hours, the fee is automatically doubled. (schoolpay is method of payment) 7.CHANGES IN HOW A STUDENT GETS HOME: I MUST notify the ASP Coordinator in writing (phone calls or fax not accepted) via email to: cewolfsden@gmail.com AND ciasp2@sbcglobal.net before the “cut off time” of 2:00 for ANY changes in how my child gets home. Without written permission, my child will not be permitted to leave. 8.SIGNING CHILDREN OUT: All individuals authorized to pick up my child must sign my child out from the after school program at the time of pick-up. Failure to sign out the student out from the school office will be regarded as an “Incident” and may result in suspension from the program for the remaining school year. ONLY adults 18 years or over authorized to pick up children. SHOWING PHOTO ID IS MANDATORY UPON REQUEST. 9.BEHAVIOR EXPECTATIONS: The HISD Code of Student Conduct will serve as the basis for all discipline expectations to ensure the safety of all students and staff members. Parents will be notified immediately if their child has violated any portion of the school’s previously communicated Code of Student Conduct. Services will be suspended should disrespectful or unsafe behavior be directed towards a student or staff member, either by a parent of a student or a student enrolled in the program. I understand that it is my responsibility for reading and knowing the expectations of the HISD Code of Conduct. I also understand that the program reserves the right to deny services on a permanent basis if behavior expectations are not being met. 10.TECHNOLOGY DEVICES: No technology devices may be used in the After School Program by any student in any grade level. All such devices must be kept in the student’s backpack. All information published in this registration packet applies to this contract. In addition, understand and agree to adhere to any guidelines amended to this contract and that I will be notified in writing by the after school program. I understand that enrolling in the After School Program does not include any other programs offered by the school (i.e. tutoring, clubs, etc.) and that Crockett Elementary is only responsible for services listed in this packet. If I do sign up my child in a school program and wish for a school staff member or the adult sponsor of a school related class (i.e. tutoring, clubs, etc to pick up my child from the ASP, I must list the names of adults on the Authorization Form authorizing them to pick up my child from the ASP.
