WELCOME TO CHORUS BI-ANNUAL ETHICAL VOICE SURVEY


The purpose of this survey is to provide you with the opportunity to give
feedback on what’s working well, what could be improved, and the actions we need
to take together to enhance the way we work. Your views are important, we
encourage you to complete this survey and answer honestly. The survey is
completely confidential - it is not possible to identify any individual
responses.

Please hover your mouse over the (i) symbol next to the question if you need a
more detailed explanation.

• Please note that you should answer questions about your leader/manager by
thinking about the person that you directly report to (i.e., your line
manager/Director) not your Field manager or any of the UCG/Downer/Chorus staff.
If you are unable to answer any of the questions, then please select the “don’t
know” option.

• Your employer is not Chorus/UCG/Downer but the company your received your
employment contract from.

The survey is available in 4 languages – English, Hindi, Punjabi and Filipino.
Please select the language you feel most comfortable with. The survey is now
open and will close on the 03/05/2024. Once the survey has closed, we will share
the results and discuss on actions. Please remember though that the survey is
