Appointments are highly recommended. Drive-up patients are not guaranteed to be
tested during busy hours.

Which test should I get?

 1. Rapid Antigen Test - $145 - Results are given with 30 minutes at our Houston
    location and within 60 minutes at our Scottsdale location.

 2. PCR Test - $250 - Our PCR tests are 99% accurate and are required for
    travel.
    
    Houston: Results are given within 4-6 hours if you are tested before
    11:30am, otherwise results are given within 20-24 hours.
    Arizona: Results are given within 24-36 hours.

 3. Antibody Test - $95 - Antibody tests are not used to determine if you have
    an active infection. These tests are used to help determine if you’ve ever
    had a Covid-19 infection in the past.



Insurance not accepted at this time. We can provide a superbill for insurance
reimbursement claims.

TESTING SITE RULES & PROCEDURES

REMINDER: Please wear a mask to the testing location.
Anyone not wearing a mask will not be tested.
Please stay in your vehicle at all times.
Please do not come on a bicycle, motorcycle or by walking up to the site.
All sales of services are final. Refunds will not be issued.

 * Upon successful registration and scheduling of an appointment, please retain
   the QR code that will be emailed to you upon successful scheduling.

 * Please show up on the designated day and time and please show up 10 min prior
   to your scheduled appointment in order to allow time for processing and
   payment

 * Please drive in at the entrance and follow the instructions of the site
   attendants. You are to remain in your vehicle at all times.

 * You will drive forward with your QR code that was emailed to you either via
   your mobile phone or a print out and have your payment ready.

 * Please show your ID to allow the check in assistant to check you in and take
   your payment.

 * Follow the instructions of the attendant and proceed to the testing location.

 * Clients will receive results by email. It is important to provide an accurate
   email address in order to receive your results.

*We will make every reasonable attempt to provide results within 24 hours for
the rapid antigen test, but we can not guarantee this in every case due to
potential unforeseen circumstances. Upon successful registration and scheduling,

**The FDA has granted Emergency Use Authorization (EUA) for the manufacturer
Quidel Corporation (Quidel) antigen detection tests—the Sofia SARS Antigen FIA
and Influenza(Flu) Type A and B. These assays are approved for detection of the
nucleocapsid protein antigen from SARS-CoV-2 in nasal and nasopharyngeal (Sofia
SARS Antigen FIA only) swab specimens, and authorized for use by laboratories
certified under the Clinical Laboratory Improvement Amendments of 1988 (CLIA),
42 U.S.C. § 263a, which meets the requirements to perform waived, moderate, or
high complexity tests. Quidel announced on July 17, 2020, that it updated the
performance data for this test to 96.7% using direct nasal swab specimens versus
Polymerase Chain Reaction (PCR) test as a result of further studies included in
its amended EUA that were submitted to the FDA. Click here for more information

***Patients may be able to receive reimbursement from their health insurance
company for all or a portion of their COVID-19 test. Please contact your
insurance company for additional information as to whether you may be eligible
to receive reimbursement for your COVID-19 test and how to apply. Please contact

PCR TEST NOTICE: PCR COVID-19 test results take 24 hours or less. Rapid COVID
Clinic is not responsible for approval from the airline and destination. Please
check with the airlines and your destination to make sure the test is approved.
Attention: At this time Hawaii’s travel restriction requires all tests to be
administered by specific providers. If you would like to see Rapid Covid Clinic
added to Hawaii’s partners list, please email Lt. Gov. Green at
ltgov.contact@hawaii.gov. Also, sometimes the results email will get caught in
your spam folder. Please check your email spam folder if you do not get your
results in the time mentioned.

This rRT-PCR test was developed, and its performance characteristics determined
by Gene by Gene. This test has not been FDA cleared or approved. This test has
been authorized by FDA under an Emergency Use Authorization (EUA). This test has
been validated in accordance with the FDA’s Guidance Document “Policy for
diagnostics Testing in Laboratories Certified to Perform High Complexity Testing
under CLIA prior to EmergencyUse Authorization for Coronavirus Disease-2019
during the Public Health Emergency” issued on March 16th, 2020. FDA independent
review of this validation is pending. This test is only authorized for the
duration of time the declaration that circumstances exist justifying the
authorization of the emergency use of in vitro diagnostic tests for detection of
SARS-CoV-2 virus and/or diagnosis of COVID-19 infection under section 564(b)(1)
of the Act, 21U.S.C. 360bbb-3(b)(1), unless the authorization is terminated or
revoked sooner.

TaqPath COVID-19 Combo Kit contains the assays and controls for a real-time
reverse transcription polymerase chain reaction (RT-PCR) test intended for the
qualitative detection of nucleic acid from SARS-CoV-2 in nasopharyngeal swab,
nasopharyngeal aspirate, and bronchoalveolar lavage (BAL) specimens from
individuals suspected of COVID-19 by their healthcare provider. TaqPath COVID-19
Combo Kit is for use only under Emergency Use Authorization (EUA).
