SUPPORT COORDINATION

Support Coordination is a Capacity Building Support that assists individuals get
the most out of their NDIS funds and build capacity.

Our experienced and qualified support coordinators can assist elegible
participants with:

 * Cooordination of supports; by connecting suitable service providers

 * understanding your NDIS plan including access to My place portal and funding
   budgets

 * referrals to qualified allied health professionals

 * preparation for NDIS planning and review

 * ensuring the implementation of plan whilst building capacity

 * offering choice and control in selecting suitable providers and including you
   in every step
