                <h5><span style="color: #236fa1;">True2U is a volunteer mentoring program that helps CMSD eighth graders explore their true selves and prepare to make the most of high school as an important step on the path to career readiness.
                    True2U is a proud partner of the PACE (Planning and Career Exploration) Program in CMSD, an embedded curriculum for 6th – 12th graders that provides them with the knowledge, skills, experiences, and confidence to uncover the
                    occupations or careers they want to pursue. Through mentoring with True2U and volunteering with PACE, you can help students develop good decision-making skills and encourage them to leverage their strengths and interests as they
                    start out on their paths to high school, college, and/or career choices.</span></h5>

        <p>This application should take about 10-15 minutes to complete. The application must be completed in its entirety in order to be submitted. You are not able to save your information to return at a later time.</p>
        <p>Completed applications will be reviewed by staff and new mentors will be contacted with the next steps in the volunteer screening process. Please note, decisions to accept applicants into the program will be determined by the application
          and screening process, space availability, and participation in orientation and training sessions.</p>

          SEVERE ILLNESS AND DEATH. BY PARTICIPATING IN THE EVENTS, THE INDIVIDUAL(S) WHO SIGN BELOW VOLUNTARILY ASSUME ALL RISKS RELATED TO EXPOSURE TO COVID-19 AND AGREE NOT TO HOLD THE PARTIES, AS DEFINED BELOW, LIABLE FOR ANY ILLNESS OR INJURY.

        <p>The Participant understands and agrees that the Cleveland Metropolitan School District Board of Education, the Cleveland Foundation, the Greater Cleveland Partnership, the Greater Cleveland Faith-based Initiative, and Neighborhood
          Leadership Institute (“the Institute”), an Ohio not for profit corporation, and/or any other group involved shall have no liability for an accident, damage to person or property, or loss of property or life associated with the activities of
          the True2U Mentoring Program (“Activities”).</p>
        <p>In consideration of the Institute consenting to the participation of the Participant in the Activities, on behalf of the Participant and the heirs and legal representatives of the Participant, the Participant forever releases The
          Institute, its directors, trustees, officers, members, affiliates, employees and agents, and the respective heirs, legal representatives, successors and assigns of each, from any and all claims, demands, and actions, of every kind,<br>which
          any Participant has or may have in connection with:</p>
        <p>(1) the Activities and the participation of the Participant in the Activities; or<br>(2) any personal or proprietary right the Participant may have in connection with the use of the likeness, voice or property of the Participant as
          described below.</p>
        <p>Each Participant understands and agrees to comply with all policies and procedures of the Institute. Any failure to comply with such policies and procedures jeopardizes the health, safety and welfare of the other participants. Any
          Participant who does not comply may be asked or required by the Institute to withdraw from the Activities and there will be no right to a refund for any amount paid or spent by the Participant.</p>
        <p>The Participant authorizes the Institute and any of its agents or employees for the duration of the participation of the Participant in any of the Activities to:<br>(a) record the likeness, voice and/or property of the Participant on a
          video, audio, photographic, digital, electronic or any other medium (collectively “Recordings”);<br>(b) use the name of or any statements made by the Participant in connection with any Recording;<br>(c) use, reproduce, exhibit or distribute
          in any medium (e.g., print publications, television, video tapes, Web site, email updates, newsletters, outdoor advertising) these Recordings for any purpose, including, but not limited to, public relations, fundraising, proposals to
          donors, funding of any type and from any source, and marketing.</p>
        <p>The Participant understands that any materials obtained through involvement with the Institute (collectively “Materials”) belong to the Institute, the Participant has no right to control the use of their respective likenesses, voices
          and/or property in the Materials and the Participant will not receive payment or compensation in any form in connection with the use of the Materials.</p>
        <p>Any dispute arising from the participation or involvement of the Participant(s) in the Activities or otherwise with the Institute shall be resolved by non-appealable, binding arbitration before a single arbitrator of the American
          Arbitration Association in Cleveland, Ohio.</p>
        <p><strong>Background Check Authorization</strong></p>
        <p>The Participant understands that they have been recruited as a volunteer for the True2U Mentoring Program and Neighborhood Leadership Institute will facilitate their background check through a third party provider. The Participant hereby
          releases and agrees to hold harmless from liability any person or organization that provides information required for the background check. The Participant also agrees to hold harmless from liability<br>Neighborhood Leadership Institute,
          their officers, directors, employees and volunteers and partner organizations. The Participant understands that by submitting this application, they authorize Neighborhood Leadership Institute to make inquiries through the Federal Bureau of
          Investigation and local law enforcement concerning their suitability as a volunteer mentor for the True2U Mentoring Program. The information requested in this application and such as may be otherwise obtained, will be used for the purpose
          of assessing their suitability as a volunteer.<br>All information will be held in confidence.</p>
        <p><strong>Cleveland Metropolitan School District Volunteer Release Form</strong></p>
        <p>I, as a member of True2U Mentoring Program, have offered my services as a volunteer to work with eighth graders at a Cleveland Metropolitan School District K-8 Elementary School for the 2022-2023 school year. I agree to abide by all
          relevant Board policies and administrative guidelines while on duty for the District (including, but not limited to, the volunteer's obligation to keep confidential and not release or permit access to any and all student personally
          identifiable information to which s/he is exposed except as authorized by law). I understand that, although I am<br>covered under the District's liability insurance policy, I am not covered by its health insurance policy nor am I eligible
          for workers' compensation. Should I become ill or suffer an accident while doing volunteer work for the District, I agree that I shall be responsible for any and all hospital and medical charges that may accrue. I understand further that,
          as a volunteer, I am not in any manner considered an employee of the District or entitled<br>to any benefits provided to employees. I further release the Board of Education from any and all liability for any damages, whatever their nature,
          which may result as a consequence of my volunteer services. All volunteers need to display appropriate behavior at all times. All volunteers who work or apply to work unsupervised with children will be required to provide a set of
          fingerprints so that a criminal records check can be conducted.<br>DUTY TO MAINTAIN CONFIDENTIALITY OF STUDENT RECORDS AND INFORMATION</p>
        <p><br>The District is committed to maintaining the security and confidentiality of all student records and/or student personally identifiable information. As an approved volunteer in the District, you may have access to student records
          and/or student personally identifiable information that must be maintained as confidential and not released and/or permitted access to except as authorized by Board policy and law. Violations of this duty may result in a reassignment and/or
          restriction of your volunteer responsibilities by the building principal or designee.</p>
        <p>Volunteers must comply with the following:</p>
        <ul>
          <li>All student records are considered confidential.</li>
          <li>Directory information including the student's name, address, telephone number, date and place of birth, major field of study, participation in officially-recognized activities and sports, weight and height of members of athletic teams,
            dates of attendance, degrees and awards received and previous educational agencies or institutions attended, can only be shared with administrative approval.</li>
          <li>Records may not be left in a place where they can be viewed by others.</li>
          <li>Copies of records may only be shared with administrative approval.</li>
          <li>Volunteers may not discuss or repeat information overheard while in the staff lounge, classrooms, offices, school grounds, hallways, school or extra-curricular activities.</li>
          <li>Volunteers may not discuss information obtained while in a classroom, such as a student's grade or behavior, with anyone other than the student's teacher or the building principal.</li>
          <li>Concerns or questions regarding student records or issues of confidentiality should be brought to the attention of the staff member responsible for supervising your activities and/or the building principal.</li>
          <li>Any knowledge of a violation of these provisions must be immediately reported to the staff member responsible for supervising your activities and/or the building principal.<br><br></li>

TRUE2U IS A VOLUNTEER MENTORING PROGRAM THAT HELPS CMSD EIGHTH GRADERS EXPLORE
THEIR TRUE SELVES AND PREPARE TO MAKE THE MOST OF HIGH SCHOOL AS AN IMPORTANT
STEP ON THE PATH TO CAREER READINESS. TRUE2U IS A PROUD PARTNER OF THE PACE
(PLANNING AND CAREER EXPLORATION) PROGRAM IN CMSD, AN EMBEDDED CURRICULUM FOR
6TH – 12TH GRADERS THAT PROVIDES THEM WITH THE KNOWLEDGE, SKILLS, EXPERIENCES,
AND CONFIDENCE TO UNCOVER THE OCCUPATIONS OR CAREERS THEY WANT TO PURSUE.
THROUGH MENTORING WITH TRUE2U AND VOLUNTEERING WITH PACE, YOU CAN HELP STUDENTS
DEVELOP GOOD DECISION-MAKING SKILLS AND ENCOURAGE THEM TO LEVERAGE THEIR
STRENGTHS AND INTERESTS AS THEY START OUT ON THEIR PATHS TO HIGH SCHOOL,
COLLEGE, AND/OR CAREER CHOICES.

 

APPLICATION PROCESS

This application should take about 10-15 minutes to complete. The application
must be completed in its entirety in order to be submitted. You are not able to
save your information to return at a later time.

Completed applications will be reviewed by staff and new mentors will be
contacted with the next steps in the volunteer screening process. Please note,
decisions to accept applicants into the program will be determined by the
application and screening process, space availability, and participation in
orientation and training sessions.

Training and enrichment in the topics, facilitation, and cultural competency

At this time, CMSD has indicated that it will allow volunteers to mentor in
school buildings this school year, with the understanding that certain health
and safety protocols (i.e. mask-wearing, maintaining social distancing,
temperature taking, etc.) must be followed if such mandates are put into effect.
True2U may also offer remote mentoring this year. This would likely mean

in the Activities or otherwise with the Institute shall be resolved by
non-appealable, binding arbitration before a single arbitrator of the American
Arbitration Association in Cleveland, Ohio.

Background Check Authorization

The Participant understands that they have been recruited as a volunteer for the
True2U Mentoring Program and Neighborhood Leadership Institute will facilitate
their background check through a third party provider. The Participant hereby
releases and agrees to hold harmless from liability any person or organization
that provides information required for the background check. The Participant
also agrees to hold harmless from liability
Neighborhood Leadership Institute, their officers, directors, employees and
volunteers and partner organizations. The Participant understands that by
submitting this application, they authorize Neighborhood Leadership Institute to
make inquiries through the Federal Bureau of Investigation and local law
enforcement concerning their suitability as a volunteer mentor for the True2U
Mentoring Program. The information requested in this application and such as may
be otherwise obtained, will be used for the purpose of assessing their
suitability as a volunteer.
All information will be held in confidence.

Cleveland Metropolitan School District Volunteer Release Form

I, as a member of True2U Mentoring Program, have offered my services as a
volunteer to work with eighth graders at a Cleveland Metropolitan School
District K-8 Elementary School for the 2022-2023 school year. I agree to abide
by all relevant Board policies and administrative guidelines while on duty for
the District (including, but not limited to, the volunteer's obligation to keep
confidential and not release or permit access to any and all student personally
identifiable information to which s/he is exposed except as authorized by law).
I understand that, although I am
covered under the District's liability insurance policy, I am not covered by its
health insurance policy nor am I eligible for workers' compensation. Should I
become ill or suffer an accident while doing volunteer work for the District, I
agree that I shall be responsible for any and all hospital and medical charges
that may accrue. I understand further that, as a volunteer, I am not in any
manner considered an employee of the District or entitled
to any benefits provided to employees. I further release the Board of Education
from any and all liability for any damages, whatever their nature, which may
result as a consequence of my volunteer services. All volunteers need to display
appropriate behavior at all times. All volunteers who work or apply to work
unsupervised with children will be required to provide a set of fingerprints so
that a criminal records check can be conducted.
DUTY TO MAINTAIN CONFIDENTIALITY OF STUDENT RECORDS AND INFORMATION


The District is committed to maintaining the security and confidentiality of all
student records and/or student personally identifiable information. As an
approved volunteer in the District, you may have access to student records
and/or student personally identifiable information that must be maintained as
confidential and not released and/or permitted access to except as authorized by
Board policy and law. Violations of this duty may result in a reassignment
and/or restriction of your volunteer responsibilities by the building principal
or designee.

Volunteers must comply with the following:

 * All student records are considered confidential.
 * Directory information including the student's name, address, telephone
   number, date and place of birth, major field of study, participation in
   officially-recognized activities and sports, weight and height of members of
   athletic teams, dates of attendance, degrees and awards received and previous
   educational agencies or institutions attended, can only be shared with
   administrative approval.
 * Records may not be left in a place where they can be viewed by others.
 * Copies of records may only be shared with administrative approval.
 * Volunteers may not discuss or repeat information overheard while in the staff
   lounge, classrooms, offices, school grounds, hallways, school or
   extra-curricular activities.
 * Volunteers may not discuss information obtained while in a classroom, such as
   a student's grade or behavior, with anyone other than the student's teacher
   or the building principal.
 * Concerns or questions regarding student records or issues of confidentiality
   should be brought to the attention of the staff member responsible for
   supervising your activities and/or the building principal.
 * Any knowledge of a violation of these provisions must be immediately reported
   to the staff member responsible for supervising your activities and/or the
   building principal.

agree to the terms and conditions of the Cleveland Metropolitan School District,
the True2U Mentoring Program, Neighborhood Leadership Institute, and all program
