But in the history of reproductive health and IVF Nepal/fertility center in Nepal, Dr. Swasti Sharma is the first one to specialize in Selective Fetal Reduction. Selective Fetal reduction is used when a mother is carrying an unsafe or undesirable number of fetuses in a multiple pregnancy, which are common in infertility treatment and medically-assisted pregnancies, in order to reduce the number of fetuses to a number that is relatively safe for the mother and the remaining fetuses. It is also used in cases of multiple pregnancy where at least one of the fetuses is implanted outside the uterus to preserve the life of the mother and the fetus in the uterus, and when one or more of the fetuses has a serious and incurable disease. Selective reduction is the practice of reducing the number of fetuses in a multiple pregnancy, say quadruplets, to a twin or singleton pregnancy. The procedure is also called multifetal pregnancy reduction. In the fertility center in Nepal/IVF Nepal, these multiple follicles can be taken out by oocyte retrieval (egg collection) for use in in vitro fertilisation (IVF), or be given time to ovulate. This results to the ovulation of a larger-than-normal number of eggs, generally in the sense of at least two also known as superovulation. Controlled ovarian nyperstimuiation (COH) involves the administration of oral and/or injectable medications to induce ovulation in the anovulatory infertile patient, and superovulation in the ovulatory infertile patient. The different types of medication and protocols for COH are reviewed. Pregnancy rates are higher with the injectable medications, follicle stimulation hormone (FSH) and human menopausal gonadotrpins (hMG), than oral medications; however, injectable medications have a higher risk of multiple gestations,ovarian hyperstimulation syndrome, cost and monitoring. CONTROLLED OVARIAN STIMULATION COMPRISES THREE BASIC ELEMENTS 1 Exogenous gonadotrophins to stimulate multi.follicular development. 2 Cotreatment with either gonadotropin.releasing hormone (GNRH) agonist or antagonists to suppress pituitary function and prevent premature ovulation. 3 Triggering of final oocyte maturation 36 to 38 hours prior to oocyte retrieval. See more SPERM CYROPRESERVATION Sperm freezing is the method of freezing and preserving sperm for future use. Men are able to freeze their sperm for use in their own future treatment or to donate to someone else’s reproduction health and infertility treatment. Donated sperm usually has to be quarantined for three or six months and screened for infections before it can be used by a recipient. The length of quarantine is dependent upon the type of screening tests that fertility center in Nepal/IVF Nepal carries out. Treatment with frozen sperm is just as successful as treatment using fresh sperm. SPERM CRYOPRESERVATION IS CONSIDERED IF 1 One has a condition, or are facing medical treatment for a condition, that may affect your fertility. 2 About to have a vasectomy and want sperm available in case the mind is changed about having (more) children. 3 One has a low sperm count or the quality of your sperm is deteriorating. 4 One has difficulty producing a sperm sample on the day of infertility treatment. 5 One is at risk of injury or death (for example, you’re a member of the Armed Forces who is being deployed to a war zone). For sperm freezing firstly, the sperm need to be tested for any infectious diseases like HIV and Hepatitis. This has no bearing on whether sperm can be freezed or not but is to ensure that affected sperm samples are stored separately to prevent contamination of other samples. Then need to give written, informed consent to sperm being stored and specify how long to be stored for. At the fertility center in Nepal, one will be asked to produce a fresh sample of sperm (if possible), which will be mixed with a special fluid (a cryoprotectant) to protect the sperm from damage during freezing. Before freezing, the sperm sample is usually divided between numbers of containers called straws. This means that not all the sperm needs to be thawed at once and can be used in multiple treatments. The samples are then cooled slowly and plunged into liquid nitrogen.
