TO START THE APPLICATION,

Please read the Guidance Notes on Applications to the Reimbursement of Maternity
Leave Pay Scheme (PDF) carefully

You should get ready the following documents in order to submit the application
successfully:

 * Registration / identity document relating to the applicant
 * Proof of a bank account for receiving reimbursement
 * Employee Declaration Form (download form here (PDF))
 * Payment proof of the 14 weeks’ maternity leave pay
 * Employee’s pregnancy proof

(The maximum file size is 50MB per document, acceptable file formats include
JPEG, PNG, PDF)

Please note the following:

You can create the account first and save the draft application. The application
should be submitted within 14 days. Otherwise, the account with no application
submitted and the draft application will be deleted after 14 days.
