FILING A CIVIL RIGHTS COMPLAINT

You may file a civil rights complaint with OCR if you feel a health care
provider or government agency discriminated against you (or someone else)
unlawfully.


FILING A CONSCIENCE OR RELIGIOUS FREEDOM COMPLAINT

You may file a conscience or religious freedom complaint with OCR if you feel a
health care provider or government agency coerced or discriminated against you
(or someone else) unlawfully.


FILING A HIPAA COMPLAINT

You may file a complaint with OCR if you feel your rights under the HIPAA Rules
were violated.


FILING A PATIENT SAFETY CONFIDENTIALITY COMPLAINT

Read about the Patient Safety Confidentiality Act and how to file a complaint
online or in writing.


BREACH NOTIFICATION
