CARE 500 FAQ
How can I be confident that I’m
getting the best deal?
Check that the procedure you’re getting matches the ADA code next to the name of
the procedure on the full price list. The Care 500 fee schedule includes about
95% of the most common dental procedures, but if the dentist recommends a
procedure with an ADA code you don’t see listed, you still get 20% off. Also, if
their original price is less expensive than the Care 500 discounted price, you
get an additional 20% off! You really can’t lose with this plan.
You can also take a list of the prices with you when you go in for your dental
visit or pull them up on your phone. If there is any confusion, you or the
dentist can call us to clarify.

What about braces and other
specialist work?
The Careington Care 500 plan saves an automatic 20% on children’s and adult
braces. It also saves 20% on any other work performed by a dental specialist
(including orthodontists, oral surgeons, prosthodontists, endodontists, and
pediatric dentists). Please note that these savings do not apply if work is
already in progress.

What about Invisalign?
We love Invisalign! A lot of us have used it ourselves. It’s a great product.
However, it’s a product that your dentist sells, not a procedure recognized by
the American Dental Association. Therefore, 1Dental plans don’t save you money
on it, and we have yet to find a plan that does. (But 1Dental plans still save
you money on traditional braces!).
What about wisdom teeth?
Sometimes wisdom teeth can be removed by your general dentist (this would be a
normal “simple extraction”), but if they are impacted, an oral surgeon will
usually need to perform the work. You will still save 20% at an oral surgeon.
Are implants included?
Absolutely! In fact, dental savings plans are one of the best ways to save money
on implants. With the Care 500, you save 20% no matter how many implants you
get. In contrast, traditional insurance usually does not give any savings on
implants and other work that the ADA has labeled “cosmetic.”
What’s NOT included?
Anything the dentist does not do (including lab and OSHA fees) Invisalign (which
is a product sold by your dentist, not a procedure the American Dental
Association recognizes) Work in progress (for example, if you already have your
braces on) If you don’t see a dental procedure on the full savings list, you
still receive 20% off.
Will I still get savings at dentists
that are out-of-network?
Your savings only apply at in-network dentists because these dentists have
agreed to the savings list and also passed Careington’s thorough approval
process. We want to make sure you are able to save money at a dentist you can
trust.
Every dental plan has a network to some degree (and in our experience, plans
with “out-of-network benefits” end up costing more than they save).
If you would like to nominate a dentist who isn’t currently in the network,
please use our dentist referral form so we can reach out to them.

always recommend double checking when you call to make your appointment.

DENTAL ACCESS FAQ
Is this the same as Aetna
dental insurance?
No. Dental Access is a dental savings plan that uses the Aetna Dental Access
network. Members get unlimited access to discounted prices at in-network
dentists across the country.

These are estimates. How do I know
what I will really save?
Each dentist contracts with the Aetna Dental Access network individually, so
prices will vary between providers. While we can give estimates for certain
procedures and a general savings range of 15-50% per visit across all
procedures, we recommend checking with your dentist for exact treatment pricing
at their location. Because the plan is so affordable, most members easily save
more than the cost of membership, whether they just get their regular 6-month
cleanings or need major work during the year.

What’s the difference between a
general dentist and a specialist?
Specialists (endodontists, orthodontists, oral surgeons, prosthodontists,
pediatric dentists, etc.) have received advanced training in their area of
expertise. Although 15% is the minimum, there may be times when your savings
range is 15-50% per visit at specialists.
What about braces?
You will save 15-50% per visit at a participating orthodontist. Since the Dental
Access plan is not insurance, there are no waiting periods or limits on your
savings. We recommend checking with your orthodontist for exact pricing with the
plan. Please note that these savings do not apply if work on the braces has
already begun.
What about Invisalign?
Invisalign is relatively new, and while some orthodontists classify it as
traditional braces, others consider it a product they sell. If your orthodontist
classifies it as traditional braces with a designated ADA code, you can save on
it with the Dental Access plan. Please note that while Invisalign is sometimes
discounted, you will not receive savings on Smile Direct Club, Candid, or other
do-it-yourself clear aligners because they are not directly administered by a
dental professional.
What about wisdom teeth?
At a general dentist or oral surgeon in the Aetna Dental Access network, members
can save 15%-50% per visit. Exact savings depend on the type of extraction
involved and whether your general dentist or your oral surgeon is doing the work
(oral surgeons are typically more expensive and perform more complicated
extractions).

Are implants included?
Absolutely! In fact, dental savings plans are an excellent way to save money on
implants and other work that is traditionally labeled “cosmetic.” You save
15%-50% per visit at an in-network provider no matter how many implants you get,
with no waiting or limits of any kind.
What’s NOT included?
Anything the dentist does not do (including lab and OSHA fees) At-home teeth
aligners like Candid or Smile Direct Club Work in progress (for example, if you
already have your braces on)
Will I still get savings at dentists
that are not in the network?
Dentists who are not in the network have not agreed to the discounted prices. To
make sure you’re receiving the savings you’ve signed up for, please visit one of
the 226,000+ dental locations in the Aetna Dental Access Network. You can find
one near you by using our dentist search engine.
We do our best to keep an up-to-date list of dentists on our website, but we
always recommend double checking when you call to make your appointment. If you

Pro tip: When speaking to the dentist, be sure to refer to the plan as the
Dental Access Plan powered by the Aetna Dental Access Network, not “the 1Dental

1Dental Refund Policy
Don’t love the plan in your first 30 days? Get your money back
Our goal is to provide dental plans that meet your long-term needs, including
everything from dental emergencies to routine care. Most of the time, we get it
right, but in the event that we don’t, you can cancel within 30 days and receive
a full plan refund, minus the setup fee.
For annual and multi-year plans:
You’ll receive a full plan refund, minus the $20 setup fee.
For monthly plans:
You’ll receive your first monthly payment back, minus the $30 setup fee.
Why do you have a setup fee?
We don’t like these either, but it allows us to provide an affordable 30-day
trial window for those who simply aren’t sure if the plan will work for them. If
the plan is a good fit, we want to reward members for loyalty — which is why we
don’t charge extra renewal fees (like our competitors). The setup fee helps us
cover administration costs and continue to offer these plans at a low price.
Setup Fee Exceptions by State:
In the event that you need a refund, these 3 states have different laws:
* Arkansas: You will be refunded the cost of your plan plus the entire setup
fee.
* Maryland: You will be refunded the cost of your plan, and you will be
refunded all but $5 of your setup fee.
* Tennessee: You will be refunded the cost of your plan plus the entire setup
fee.
Refunds After the First 30 Days
We know that, sometimes, unusual circumstances happen that require you to cancel
your plan after the first 30 days. Often, we are able to switch members to
another plan that works better for them for free. In other cases, members finish
out their year and simply don’t renew. However, there are a few exceptions.
* If you live in Florida, North Dakota, or Oklahoma, you will receive a
prorated refund for the remainder of your plan.
* If a plan member passes away
* If your membership is terminated by 1Dental or Careington
Other circumstances will be considered on a case-by-case basis. Please call our
Customer Care team at 877-583-0389 if you would like to cancel after the first
30 days.

1DENTAL TERMS & CONDITIONS
All plans purchased through 1Dental are administered by Careington International
Corp.
PURCHASE AND RENEWAL CONDITIONS:
By joining a plan for yourself or on behalf of a minor child for whom you are a
parent or legal guardian, you confirm that you are at least 18 years old and you
authorize Careington International Corporation (Careington) to charge your
credit card or checking account for the plan you have selected. This charge
shall automatically renew at the end of your membership term, and your credit
card or checking account will be automatically charged for the appropriate
amount, until you notify Careington in writing that you wish to cancel the plan.
By joining you indicate you have read and agree to the terms and conditions of
the plan.
TERMINATION CONDITIONS:
Careington reserves the right to terminate plan members from its plan for any
reason, including non-payment. If Careington terminates the plan or your
membership for a reason other than non-payment, you will receive a pro-rated
refund of your membership fees.
CANCELLATION CONDITIONS:
You have the right to cancel within the first 30 days after effective date or
receipt of membership materials (whichever is later) and receive a full refund,
less the processing fee, if applicable. If for any reason during this time
period you are dissatisfied with the plan and wish to cancel and obtain a
refund, you must submit a written cancellation request. Careington will accept
cancellation requests at any time and will stop collecting membership fees in a
reasonable amount of time, but no later than 30 days after receiving a
cancellation notice. Please send a cancellation letter and a request for refund
with your name and member ID to Member Services, Careington International
Corporation, P.O. Box 2568, Frisco, TX 75034 or fax to 888-335-7330.

health care services. No portion of any provider’s fees will be reimbursed or
otherwise paid by Careington. Careington is not licensed to provide and does not
provide health care services or items to individuals. You will receive discounts
for services at certain health care providers who have contracted with the plan.
You are obligated to pay for all health care services at the time of service.
Savings are based upon the provider’s normal fees. Actual savings will vary
depending upon location and specific services or products purchased. Please
verify such services with each individual provider. The plan’s discounts may not
be used in conjunction with any other discount plan or program. All listed or
quoted prices are current prices by participating providers and subject to
change without notice. Any procedures performed by a non-participating provider
are not discounted. From time to time, certain providers may offer products or
services to the general public at prices lower than the discounted prices
available through this plan. In such event, members will be charged the lowest
price. Discounts on professional services are not available where prohibited by
law. This plan does not discount all procedures. Providers are subject to change
without notice and services may vary in some states. It is the member’s
responsibility to verify that the provider participates in the plan. At any time
Careington may substitute a provider network at its sole discretion. Careington
cannot guarantee the continued participation of any provider. If the provider
leaves the plan, you will need to select another provider. Providers contracted
by Careington are solely responsible for the professional advice and treatment
rendered to members and Careington disclaims any liability with respect to such
matters.
COMPLAINT PROCEDURE:
If you would like to file a complaint regarding your plan membership, you must
submit your complaint in writing to: Careington International Corporation, P.O.
Box 2568, Frisco, TX 75034. You have the right to request an appeal if you are
dissatisfied with the complaint resolution. After completing the complaint
resolution process, if you remain dissatisfied you may contact your state
insurance department.

available, the US Laser Network. Members are entitled to 15% off the retail
price or 5% off the promotional price of LASIK or PRK procedures, whichever is
the greater discount.
REPLACEMENT CONTACT LENSES BY MAIL:
EyeMed members may order replacement contact lenses at competitive prices via
the Internet and have them mailed directly to your home. This service is for
replacement contact lenses only, and your EyeMed discount does not apply. Your
initial pair of contact lenses must still be purchased from your eye care
provider to ensure proper fit and follow-up care.
*Frame, lenses and lens options discounts apply only when purchasing a complete
pair of eyeglasses. If purchased separately, members receive 20% off the retail
price.

WHAT IF I ALREADY HAVE A VISION INSURANCE PLAN?
No problem! EyeMed won’t interfere. You can let your vision provider know you
have both, and they can help you use whichever saves you the most money on each
visit.
DOES THIS VISION PLAN HAVE A SEPARATE ID FROM MY DENTAL PLAN?
Vision savings are automatically included with your dental plan, so you can use
the same ID card at both your dentist and your eyesore provider. Please note
that you need to present your physical card to the eyecare provider, so these
savings are not available until about a week after you join your plan (either
Care 500 or Dental Access).
