DESCRIPTION

In the session Building Healthy Communities with Collaborative Partnerships, the
role of collaborative partnerships is paramount in building healthier
communities. Panelists will discuss diverse perspectives on solutions to silos,
barriers, and risk to collaborative partnerships, lessons from COVID and public
health, and how to activate local, state, and federal policy solutions.
Panelists will also share their experiences on how to improve the health of
diverse populations and increase access to resources in rural communities.
Finally, they will share the recent impacts of technology and collaboration in
healthcare on patient experience, providers (e.g., mental health, wellbeing,

FOOD INSECURE PATIENTS RECEIVING PRODUCE AND WELLNESS EDUCATION

DESCRIPTION

This session, Food Insecurity and Food Literacy, will discuss San Antonio Food
Bank’s Farmacy initiative in their 29-county service area and address how
consistent access to produce impacts the social determinants of health and
reduces the risk of heart disease, stroke, and other major health problems.
Participants will learn how San Antonio Food Bank’s Farmacy addresses food needs
by connecting to programs and healthcare provides, implements a food Insecurity
Training that empowers healthcare staff, and connect patients to resources
through e-Referral system.

This session will also cover addressing the social determinants of health by
implementing Life’s Essential 8, the key measures for improving and maintaining
heart health, as defined by the American Heart Association, and that may
significantly impact how increased food literacy will build healthier
communities. Participants will leave with knowledge and tools to reduce the risk
of heart disease and stroke and be able to positively support physical and
mental health.

PANDEMIC LESSONS: POLICY AND ADVOCACY IN THE SAFETY-NET

DESCRIPTION

In Pandemic Lessons: Policy and Advocacy in the Safety-Net, the team from
Parkland Health, Dallas County’s safety-net hospital, will share ongoing
research about how policy structures resulted in inequitable outcomes during the
COVID pandemic. The panel will share research regarding the impact of charity
care on COVID outcomes, the burden of undiagnosed diabetes unmasked in COVID
hospitalizations, and racial/ethnic COVID disparities unique to Texas. This
session will highlight the policy and advocacy challenges revealed during the
pandemic and discuss a roadmap for change from safety-net systems.

▸Creating a hunger free community: An innovative effort to improve food access

DESCRIPTION

Food access and consumption are key health indicators for communities. This
community health center established partnerships with local food banks and food
pantries to develop Food Prescription (Food Rx) and Door Dash delivery programs
that work to improve access to healthy foods and address diet-related chronic

▸A Coordinated School Health Approach to Reduce Risk of Dental Caries Among
Elementary School Children in Texas

DESCRIPTION

Dental caries is the most common childhood disease with 68% of kindergarten
children having tooth decay. Children with poor oral health are more than two
times likely to perform poorly in school, and children with cavities in their
baby teeth are three times more likely to get cavities in their adult teeth. The
Texas 83rd legislative session added oral health as a required topic to the
state education code.

Seydia Adkins is a Public Health Program Coordinator for the Lubbock Public
Health Department in Lubbock, Texas, a cooperative, change agent working to
promote health equity across the city Seydia functions as the coordinator for
the LBK Community Network, a collaborative alliance of partner agencies who work
together on a single platform that enables them to assess Social Determinants of
Health (SDoH) and make referrals electronically. She completed her undergraduate
degree at Texas A&M University in Commerce, Texas (2006), majoring in Sociology
with a Criminology emphasis. She later earned her master’s degree in counseling
at Lubbock Christian University in Lubbock, Texas (2011).

Victor is a graduate of UT Brownsville with a BA in Interdisciplinary Studies
and an MS in Management & Leadership from Western Governors University of Utah.
Victor launched his career in Brownsville, specializing in educational
initiatives that served the community which led to a 19-year service with
Brownsville Literacy Center, where he served as a volunteer teacher, mentor,
education coordinator, and Executive Director – providing a firm foundation for
his role at Loaves & Fishes. In this Executive Director role, his primary
mission is to function as the “mission arm” of member congregations in the area
by providing food, shelter, training, and beyond.

Stephanie Norris is the Director of Federal Programs and Grants and leads

collaboration. She furthers this work as a member of the Homeless Coalition for
Hays County whose mission is to find and close housing gaps in Hays County

in nutrition education and public speaking. Tracy has almost 15-year tenure with
the San Antonio Food Bank, with 3 years’ experience in San Antonio Food Bank
Farmacy Initiative and lifetime experience in agriculture and community service.
Tracy is fortunate and honored to hold a unique position in that they work both
in directly educating and empowering food insecure patients and in directly
educating and empowering healthcare staff to connect food insecure patients to
resources and services.

JAMIE GONZALEZ

BIG FRESH Market Box

Jamie Gonzalez is a food activist, chef, and a growing force in changing the
culture of how San Antonio eats.
Jamie is the founder of the BIG FRESH Market Box Program. These produce boxes
and markets have been recognized for their ingenuity and are a catalyst for a
variety of hands-on experiences with produce and community building. Since 2020,
she has distributed over 3,000,000 pounds of produce through over 50
public/private partnerships.

Jamie is nationally recognized for her contributions to food and health equity.
She has been featured in Live from The Southside Magazine and recognized by The
Current as a Fearless Woman in the SA Food Scene. Jamie was the North SA
Chamber’s 2022-Civic Leadership Academy Campaign Winner and the SA Flavor 2022
Co-Champion. Jamie is a 2022 AHA-Empowered-Business Accelerator Finalist and the
2023 AHA-Sa-Woman of Impact. This year, Jamie was recognized by Boston Medical
Center and selected for their Vital Village National Community Food Systems
Fellowship.

ANISHA GANGULY

Anisha Ganguly, MD, MPH, is the inaugural Health Equity Fellow in the Center for
Innovation and Value at Parkland Health and the Division of General Internal
Medicine at UT Southwestern Medical Center. Her clinical interests include
primary care of the underserved, complex chronic disease management, and
addiction medicine. Dr. Ganguly holds bachelor’s degrees in biological chemistry
and Spanish literature from the University of Chicago. She received her MD from
UT Southwestern, where she concurrently completed her MPH with a focus area in
health disparities. She completed her internal medicine residency at the
University of Washington with specialized training in primary care and health
systems. Dr. Ganguly’s research interests include structural racism in policy
and system-level factors that drive health disparities. Her research focuses on
designing social and structural determinants of health interventions to address
structural inequities and advance equitable delivery of care.

KAVITA BHAVAN

Kavita P. Bhavan, MD, MHS, holds the titles of Associate Professor, Division of
Infectious Diseases, Department of Internal Medicine at UT Southwestern Medical
Center and Chief Innovation Officer for Parkland Health. Dr. Bhavan developed a
self-care program for uninsured patients discharged from Parkland hospital’s
safety-net setting to self-administer outpatient parenteral antimicrobial
therapy (S-OPAT) for the treatment of a variety of infections. In the S-OPAT
model, patients from a low-literacy/low–socioeconomic status population are
taught to self-administer their IV antibiotics by gravity or IV push at home.
The first 4 years of the program resulted in >27,000 hospital bed days,
approximately $40 million in savings, and more than 3,500 patients treated for
complicated infections. Current research efforts include studying the collateral
benefits of patient engagement in the S-OPAT program on chronic disease states
and exploring other clinical settings for self-care models.

Tami Gurley, Ph.D. is Professor and PhD Program Director in the O’Donnell School
of Public Health. Dr. Gurley’s research explores how policies and programs
affect health outcomes and decisions about family and work. She partners with a
wide range of federal, state, and local organizations to conduct analyses that
inform policy and program decisions including Medicaid, community health
workers, health insurance design, tax policy, and entrepreneurship. Dr. Gurley
leads development of the health economics and policy concentration for the Ph.D.
in Public Health. Students in the program will train as researchers and leaders
who use their skills to create lasting health policy impacts in Dallas and
beyond.

Michael Harms, MSBA is the Enterprise Data Scientist for the Center of
Innovation and Value at Parkland. He focuses on supporting low-cost technology
solutions for innovation projects and analyzing their impact by utilizing an
assortment of statistical approaches. These solutions are supported by a range
of enterprise tools and custom code. He received his undergrad degrees in
Information Technology and International Business at Marquette University. At
University of Texas at Dallas, he completed his master’s in business Analytics
with a focus on Data Science.
