THE DISCHARGE SEVERITY INDEX (DSI) TOOL WAS CREATED TO HELP EMERGENCY CARE
PROVIDERS ASSESS PATIENTS’ HEALTH RISKS UPON DISCHARGE. THIS TOOL HAS THE
OPPORTUNITY TO BECOME AN INTEGRAL PART OF CARE COORDINATION AND SAFE DISCHARGE
PLANNING FOR PATIENTS IN EMERGENCY CARE SETTINGS.

Adapt the DSI for your Institution

The DSI Framework is designed to have the level of standardization, but also the
flexibility to be reliably applied in a variety of emergency settings. The
five-point scale has been refined during iterative analysis by the initial
investigators of the DSI framework and that scale is recommended to be
implemented for standardization purposes. However multiple other flexibility
options remain, including:

DISTRIBUTION

Both paper and digital means of the DSI have been explored and both may be
appropriate at your institution.

PROVIDERS

The DSI has been studied in a wide range of emergency providers including
attending physicians, residents, physical assistants, nurse practitioners, and
registered nurses. Who bears ultimate responsibility for completing the DSI for
a given patient may vary at each institution.

CARE COORDINATION

How your institution plans to incorporate the DSI to coordinate appropriate
follow-up for your patient may also vary. The DSI is designed as a tool to help
identify patients at increased risk, and there are many avenues to best target
these patients based on DSI designations.
