together providers from several highly-regarded and familiar southern California
healthcare systems.

Choosing Your Doctor

Choosing your doctor is an important step toward taking charge of your health.
We encourage you to take the time to search for a provider today.

We highly recommend that you designate a primary care provider (PCP) to act as
your central healthcare advocate. By making your PCP the first medical
professional you consult for basic health needs, you allow for a strong
physician-patient relationship to develop. Additionally, because your PCP
understands your full medical history, he or she is able to provide greater
coordination of your overall care and can even help recommend specialists as
needed. Most importantly, they deliver essential preventive care that can help
you stay healthy and out of the hospital by screening for and treating
conditions early—before they become a problem. When choosing a PCP, search
family medicine, internal medicine or pediatrics under “specialty.”

For the categories of providers listed below, the MemorialCare Health Alliance
network is supplemented by the Blue Cross Blue Shield (BCBS) PPO network . For
additional assistance in locating a provider that meets your needs for these
categories, please contact BCBSIL directly at 1-888-802-8776 .

Medical providers from several highly-regarded Southern California healthcare
systems have partnered to create MemorialCare Health Alliance. Members will have
access to an extensive network of primary care physicians and specialists;
community-based urgent care, imaging and surgical centers; dialysis centers;
physical therapy and rehabilitation centers; hospitals and other healthcare
providers—including a 24/7 Telephone Nurse Line—all conveniently located near to
where you live, work and play.

MEMBER TIP: Not all providers affiliated with network partners are participating
in MemorialCare Health Alliance. Please use the provider search tool to confirm
your physician’s participation.

MONARCH/OPTUM UPDATES

Effective January 1, 2023, Monarch/Optum will no longer be a participating
partner in the MemorialCare Health Alliance (MCHA) plan and all its physicians –
both primary care and specialists – as well their three urgent care centers in
Anaheim, Cypress and Seal Beach and their three imaging centers in Anaheim,
Anaheim Hills and Whittier will no longer be considered in-network.
