Our vision is to educate, support, and empower our employees to improve their overall wellbeing by promoting a healthy body, mind, and spirit by providing opportunities for employees to be conscious of their personal health. All full-time employees and spouses enrolled in the UHC medical plan have the opportunity to earn $1,000 in gift cards (each) by participating in the UHC Rewards program. Acworth also provides employees the opportunity to receive a premium discount each plan year. Program Overview PROGRAM COMPLIANCE In order to be considered compliant within the Invigorate Wellness Program and avoid the $100 per pay period surcharge in the 2025 Plan Year, employees must complete the below by August 1st, 2024: Complete the UHC Online Health Risk Assessment Complete the Health Designs Biometric Screening Complete the Health Results Designs Health Coaching Session PREMIUM DISCOUNTS * If you are compliant AND earn $350 dollars in the UHC Rewards Program, you will avoid the $100 per pay period surcharge AND receive a 3% premium discount in the 2025 Plan Year. * If you are compliant AND earn at least $175 in the UHC Rewards Program, you will avoid the $100 per pay period surcharge AND avoid the 6% premium surcharge in the 2025 Plan Year. * If you are NOT compliant AND do not earn $175 dollars in the UHC Rewards Program, you will get a 6% premium surcharge and a $100 per pay period surcharge during the 2025 Plan Year.
