Important Announcement from Neighborhood Medical Clinic



Notice of Clinic Closure


Dear Patients,


We regret to inform you that the Neighborhood Medical Clinic will permanently
close. Effective immediately, we will no longer see patients or provide medical
services.


If you have any inquiries or want copies of your medical records, email us at
Manager@nmedclinic.com or 918-893-3535. Our phone line will be forwarded until
November 29, 2024, between 10:00 a.m. and 3:00 p.m., Monday through Friday,
after which it will no longer be monitored. Please note that we will not be
available for calls during the Thanksgiving holiday. After November 29, all
communications will be directed to our email or PO Box at PO Box 700054, Tulsa,
OK 74170. 
Processing Time
Please allow 24 to 48 hours for us to process your request once it is received.
We will do our best to fulfill your request promptly.



Please note:
Medication Refills: We cannot refill medications as we are not seeing patients.
Please contact your pharmacy or a new healthcare provider for medication needs.
Medical Records: Please contact us by November 29, 2024, to obtain copies of
your medical records. We will provide instructions for record transfer and other
requests. Records will be retained according to Oklahoma law.
Future Inquiries: After November 29, please ensure another physician is
available for your medical needs, as we cannot respond after that date.
Community Health Support
Suppose you need assistance finding new healthcare services or navigating
community resources. In that case, the Community Health Program, a partnership
between Ascension St. John and the Tulsa Health Department, supports Tulsa
County residents. This program offers case management, health education, and
access to community resources to help you connect with the care you need.

Program Manager.
We understand this may be inconvenient and apologize for disrupting your care.
Our goal is to ensure a smooth transition for our patients.
