2022 HEALTHIEST EMPLOYERS OF GREATER CHARLOTTE AWARDS

CBJ's 12th annual Healthiest Employers of Greater Charlotte Awards program
honors the top health and wellness programs in our region. Don't miss this
opportunity to showcase your company’s innovative strategies for keeping your
workforce healthy!

ELIGIBILITY REQUIREMENTS

2022 Healthiest Employers of Greater Charlotte Awards

The Charlotte Business Journal's annual Healthiest Employers of Greater
Charlotte Awards program honors local companies with outstanding health and
wellness initiatives. We're looking for organizations that are committed to
creating a healthy workplace and making wellness a priority and proactively
shaping the health of their employees. Nominate your company today to see how
your wellness program measures up to the competition!

To nominate your company:

 * Complete the CBJ form below AND
 * Fill out the online assessment on the Healthiest Employers website to submit
   your official application. Click here >> 2022 Healthiest Employers
   Application

The online assessment must be completed through the Healthiest Employers website
to be eligible for nomination. Deadline to submit nominations is Sunday, April
10 at midnight.

If you have issues or questions about your online assessment, please contact
info@healthiestemployers.com.

Nominations will be accepted and analyzed by our partner, Healthiest Employers,
a national data and research company focused on worksite wellness, POWERED BY
SPRINGBUK HEALTH ANALYTICS, LLC.

The rankings of each company will be revealed at an awards event in June of
2022. All honorees will also be listed in the Charlotte Business Journal.

Eligibility:

 * Be a for profit entity, a non-profit entity or a government entity in the
   Greater Charlotte region.
 * Employ a minimum of five (5) full-time employees, including the owner.
 * Demonstrate evidence of an employee wellness program.
 * Complete the entire online assessment by Sunday, April 10 at midnight.
 * Companies being honored are expected to attend the awards event in June 2022.
 * Company must be located in the Charlotte region. Charlotte region is defined
   by the following counties: In North Carolina: Alexander, Anson, Cabarrus,
   Catawba, Cleveland, Gaston, Iredell, Lincoln, Mecklenburg, Rowan, Stanly and
   Union. In South Carolina: Chester, Chesterfield, Lancaster and York.*
