I, the undersigned, being aware of my own health and physical condition, and
having knowledge that my participation in any exercise program may be hazardous
to my health, am voluntarily participating in a physical activity with JC'S
Boxing Academy and Jon Thomas.

Having such knowledge, I hereby release JC'S Boxing Academy,  Jon Thomas, Jab
Academy,  their representatives, agents, and successors from liability for
accidental injury or illness which I may incur as a result of participating in
said physical activity. I hereby assume all risks connected therewith and
consent to participate in said program. Any recommendations  for changes in diet
including the use of food supplements, weight reduction, and/or body building
enhancement products are entirely your responsibility and you should consult a
physician prior to undergoing any dietary or food supplement changes as well as
participating in any fitness activities. 

I agree to disclose any physical limitations, disabilities, ailments or
impairments which may affect my ability to participate in said fitness program. 

I understand that the achievement of health and fitness goals can not be
guaranteed, and I hereby release JC'S Boxing Academy and Jon Thomas from
liability in the attainment of my health and fitness goals.
