This entertaining and very informative book marries complex topics like the human capital and willingness-to-pay approaches to incorporating patient experiences into drug value with historical anecdotes about the origins of these concepts. In this manner, Neumann and colleagues make convoluted methodologies understandable to a lay audience. With the fragmented healthcare industry in the United States, this book offers solutions to overcoming the disincentives to pharmaceutical companies for innovation and investment in basic research and development while suggesting the role of value assessment to address the need for affordable drug pricing. It is high time that the United States consistently incorporate value-based schema into its decision-making processes about drug pricing and affordability; this book serves as a practical guide to implementing best practices to do so. I can honestly say that, after teaching these types of topics in schools and during consultations for the industry and the United States government for over two decades, I gleaned some valuable information that I will incorporate into my work. The authors also provide valuable context to the value-based pricing of drugs in the United States in comparison to global pricing attempts. As the authors indicate, simply slashing prices without demanding improved patient outcomes is a folly that needs to be remedied immediately. This book addresses issues that are both economically and societally important and it gives serious thought to a topic that touches all of us, often at our most vulnerable moments. It also fills a significant gap. Although there are many books on the pharmaceutical industry (including my own) and on health economics, I’m not aware of a book that addresses drug pricing in such an authoritative manner. Peter Kolchinsky’s impassioned book, The Great American Drug Deal, takes a somewhat different perspective on the same topic and the two books complement each other. In summary, this is a very good book and I commend it to you. It takes intellectual courage to address such a challenging, contentious topic and the “The Right Price” justifies the authors’ self-belief. It may be an over-used commendation, but I think this book is essential reading for anyone working in the areas of health economics or in the pharmaceutical or related sectors. The book is especially relevant to US readers but non-US readers will find it valuable too. I shall be recommending it to my students, clients, colleagues and anyone else in the field. “The Right Price” is structured into three parts. First, a primer on the economics of prescription drugs. Second, a historical and present-day consideration of value-assessment approaches in the US and elsewhere. Finally, recommendations towards the goal of making drugs pricing value-based and more affordable drugs. This structure allows a clear flow of ideas. The are many things about this book that I liked. Its style is easy to read and, for a multi-author book, consistent. It makes liberal use of relevant and often recent examples. It is also fulsomely referenced. Where detailed or technical digressions are needed, it maintains its narrative flow by judicious use of side-boxes. But it is the depth and breadth of its content that is most impressive. Written by three people who lead in their field, the depth of their experience permeates the book. Perhaps most admirable of all is that the authors refrain from polemic and maintain an even-handed perspective throughout. As the author of 7 books myself, I know that it is impossible for any human author to write a perfect book. In particular, space and time enforce difficult choices about how to limit a book and the authors’ perspectives almost invariably lead to some weaknesses. The Right Price has its weaknesses and limitations, but they are heavily outweighed by its strengths. Its primary limitation is its focus on the US situation in what is a global market. Although the US market is not isolated from other markets, the authors have chosen to focus their discussion on their home market, apart from where it is necessary to give context to the development of value assessment. I’m sure this is a deliberate choice and quite justifiable on the grounds of space. Despite this limitation, I think non-US readers would find much of the book worthwhile. The US is the largest market in the world for innovative drugs and, despite its exceptionalism, strongly influences the global pharmaceutical market. Its weakness, and one often shared by writers calling for price control, is that it does not ask the “what if” question. It makes forceful recommendations towards achieving value-based pricing of drugs in the US market and the partly implicit assumption is that this will reduce the price of innovative medicines. But the authors do not consider the full consequences of that outcome. If successfully implemented, the authors’ recommendations might be expected remove many billions of dollars from the cash flow of pharmaceutical companies and others in the pharma supply chain, leaving the money either in the hands of governments, insurers or patients to be redirected as they wish. I would like to have seen the authors consider the indirect consequences of this because there surely would be some. The system that invents, makes and provides innovative medicines is a complex, adaptive system of which drug pricing is only one important element. Pharmaceutical companies, especially big pharma, rely on internal cash flows to fund innovation. A substantial change in pharmaceutical industry cash flows, especially if sudden, might reasonably be expected to have consequences for innovation, whatever its cost benefits for payers. It is perhaps naïve to assume that value-based pricing would not have some unintended consequences that are less desirable. But the authors do not seem to consider this in any detail, which seems a significant weakness in an otherwise very good book. In this respect, I think “The Right Price” is as auspicious as it is admirable and auspicious. “The Right Price” should precede and would valuably inform a wider debate about the holistic management of the pharmaceutical innovation ecosystem.
