"The purpose of an Acknowledgment is for a signer, whose identity has been
verified, to declare to a Notary or notarial officer that he or she willingly

VERIFICATION OF OATH OR AFFIRMATION: 

"If no other format is prescribed, a Verification on Oath or Affirmation form
may be used when an individual is signing and swearing (or affirming) that
certain written statements are true."  (NationalNotary.org)

COPY CERTIFICATION: 

"This certificate may be used by Pennsylvania Notaries to certify true copies of
original documents."  (NationalNotary.org)

WITNESSING OR ATTESTING A SIGNATURE:

"The Signature Witnessing or Attesting certificate is used to determine that the
signature appearing on a document is that of the person appearing before the
