family the best benefits for the best cost. Please take time to review this
summary and make any benefits changes necessary for 2024. Benefits will be
effective until December 31, 2024.


WHO IS ELIGIBLE

 * All full-time employees working a minimum of 30 hours or more per week
 * Dependents are covered until age 26
 * If you wish to elect or change voluntary life elections, you are required to
   complete an additional enrollment form and also submit evidence of
   insurability to Lincoln Financial Group (“Health Statement”)




THINGS TO KNOW BEFORE YOU ENROLL

 * You will have 30 days from your date of hire to enroll in benefits
 * If you want to add or drop coverage for yourself or a dependent for the new
   plan year, you must do so during this open enrollment period; The only time
   you can make coverage changes outside of the open enrollment period is if you
   experience a qualifying life event
 * You must provide the Social Security number and date of birth for any spouse
   and/or dependent you enroll; If you have not received the Social Security
   number for a newborn, contact Human Resources to update upon receipt
 * If you are electing any of the voluntary life/AD&D plans, be sure to enter
   the beneficiary name and date of birth if you have not already


2024 BENEFITS SUMMARY




MEDICAL

 * Your major medical plan will be offered through Blue Cross Blue Shield of
   Illinois (BCBSIL)
 * We also offer two regional plans through Kaiser Permanente for those who
   reside in Northern and Southern California; additionally, we offer a regional
   plan for employees in Hawaii through HMSA
 * Telemedicine will be offered through First Stop Health at no cost to you
 * Through our partnership with BCBSIL, you will have access to Hinge & Livongo




DENTAL AND VISION

 * Your dental coverage is offered through Delta Dental
 * Your vision coverage is offered through VSP




VOLUNTARY BENEFITS

 * Accident and critical illness is offered through Lincoln Financial Group;
   identity theft is offered through LifeLock
 * Pet Insurance is offered through Metlife at a discounted rate.




LIFE AND DISABILITY

 * Your life and disability carrier is Lincoln Financial Group; life, short-term
   disability, and long-term disability is covered at 100% by AHEAD




FLEXIBLE SPENDING ACCOUNTS (FSA)/TRANSPORTATION

 * Your FSA/transportation will be offered through Employee Benefits Corporation
   (EBC)
