8. WEB ANALYTICS
We use web analytics tools to enhance our users’ experience by optimizing the
efficiency, design, and quality of our Website and other Properties. Our
analytics tools use cookies and similar technologies to collect and analyze
information related to user behavioral metrics on our Website and Properties,
such as mouse movements, clicks, user inputs, scrolling, access time, visit
duration, pages viewed, IP address (including approximate location), operating
system, and page reloading. These tools provide insights into what works and
what does not work for users of our Website and Properties and they allow us to,
for example, identify and repair technical errors, such as broken links that
hamper the Website’s/Properties’ functionality. These web analytics tools also
help us reconstruct the user experience to better understand, for example, where
users may be encountering frustration or impediments to using the Website and/or
other Properties.

We use a website analytics tool that provides session replay, heatmaps, funnels,
form analytics, feedback campaigns, and similar features and functionality. This
session replay software may record your clicks, mouse movements, scrolling, form
fills (keystrokes) in non-excluded fields, pages visited and content, time on
site, browser, operating system, device type (desktop/tablet/phone), screen
resolution, visitor type (first time/returning), anonymized IP address, location
(city/country), language, and similar metadata. This software does not collect
information on pages where it is not installed, nor does it track or collect
information outside your web browser.

We use this web-based analytics software because it provides us with a tool to
track the online behavior of visitors while on the Website and other Properties
and lets us replay visitors’ actions. This tool gives us the opportunity to make
the Website and Properties better and, in turn, provide you with an enhanced
online experience as well. Thanks to this tool, we are able to obtain insights
on how you and other visitors use the Website and other Properties; for example,
whether you can easily locate certain pages or whether it takes multiple clicks
to get to those pages. Information about these small choices and interactions
are of paramount value to us, as they can easily make your experience better and
far more intuitive and, more importantly, allow us to provide you with a more
seamless, customer-centric e-shopping experience while on the Website and other
Properties.

For more information about this session replay software, see FullStory’s Privacy
Policy at FullStory’s Privacy Policy.

In addition, Pixel tags (also known as web beacons and clear GIFs) may be used

provide the services you request of us, and to otherwise operate, develop and
improve the service.

transaction and provide you with the product(s) you have purchased. In addition,
we may use your vision and prescription information to contact your prescriber
for the purpose of requesting that your prescriber furnish a copy of your
prescription to us. By providing your vision and prescription information to
GlassesUSA.com, you agree and authorize GlassesUSA.com to use your vision and
prescription information for the purpose of requesting that your prescriber
furnish a copy of your prescription to GlassesUSA.com.

Payment: We may use and disclose your PHI in order to obtain payment for the
health care products and services that we may provide to you and for other
payment activities related to the services that we provide. For example, we may
contact your insurer, pharmacy benefit manager or other health care payor to
determine whether it will pay for health care products and services you need and
to determine the amount of your co-payment. The information on or accompanying
the bill may include information that identifies you, as well as information
about the services or products that were provided to you. We may also disclose
your PHI to other health care providers or HIPAA covered entities you are
associated with, who may need it for their payment activities related to their
engagement with you.

Healthcare Operations: We may use PHI to improve the Properties and Services,
for marketing activities permitted under HIPAA (such as suggesting a
health-related product that we make available to our Users, either on our
website or through personalized advertising based on your past purchases), and
for our customer service activities, quality improvement, performance
evaluation, and other internal business management purposes. We may use your PHI
to create de-identified data, which is stripped of your identifiable data and no
longer identifies you.
We may also use and disclose your PHI without your prior authorization for the
following purposes:
 * Business Associates: We may contract with third parties to perform certain
services for us, such as billing services, copy services or consulting
services. These Third Party Service Providers, referred to as Business
Associates and/or Business Associate Subcontractors, may need to access your
PHI to perform services for us and in such case we will use our best efforts
to ensure they are required by contract and law to protect your PHI and only
use and disclose it as necessary to perform their services for us.
 * To Communicate with Individuals Involved in Your Care or Payment for Your
Care: We may disclose to a family member, other relative, close personal
friend, or any other person you identify, PHI directly relevant to that
person's involvement in your care or payment related to your care. If a
person has the authority by law to make health care decisions for you, we
will generally regard that person as your "personal representative" and treat
him or her the same way we would treat you with respect to your PHI.
 * Reorders: To send you reminders for re-orders (e.g., re-orders of contact
   lenses), as described above.
 * Food and Drug Administration ("FDA"): We may disclose to persons under the
jurisdiction of the FDA, PHI relative to adverse events with respect to
drugs, foods, supplements, products and product defects, or post-marketing
surveillance information to enable product recalls, repairs, or replacement.
 * Worker's Compensation: To the extent necessary to comply with law, we may
disclose your PHI to worker's compensation or other similar programs
   established by law.
 * Public Health: We may disclose your PHI to public health or legal authorities
charged with preventing or controlling disease, injury, or disability,
including the FDA. In certain circumstances, we may also report work-related
illnesses and injuries to employers for workplace safety purposes.
 * Law Enforcement: We may disclose your PHI for law enforcement purposes as
required or permitted by law for example, in response to a subpoena or court
order, in response to a request from law enforcement, and to report limited
   information in certain circumstances.
 * As Required by Law: We will disclose your PHI when required to do so by
   federal, state or local law.
 * Health Oversight Activities: We may disclose your PHI to an oversight agency
for activities authorized by law. These oversight activities include audits,
investigations, inspections, and credentialing, as necessary for licensure
and for the government to monitor the health care system, government programs
and compliance with civil rights laws.
 * Judicial and Administrative Proceedings: If you are involved in a lawsuit or
a dispute, we may disclose your PHI in response to a court or administrative
order. We may also disclose your PHI in response to a subpoena, discovery
request, or other lawful process instituted by someone else involved in the
dispute, but only if efforts have been made, either by the requesting party
or us, to first tell you about the request or to obtain an order protecting
   the information requested.
 * Fund raising: As permitted by applicable law, we may contact you to provide
you with information about our fundraising programs. You have the right to
"opt out" of receiving these communications and such fundraising materials
will explain how you may request to opt out of future communications if you
do not want us to contact you further for fundraising efforts.

Your Health Information Rights:
 * Obtain a paper copy of the Notice upon request. You may request a copy of our
current Notice at any time. Even if you have agreed to receive the Notice
electronically, you are still entitled to a paper copy. You may obtain a
paper copy at the site where you obtain health care services from us or by
   contacting privacy@glassesusa.com.
 * Request a restriction on certain uses and disclosures of PHI. You have the
right to request additional restrictions on our use or disclosure of your PHI
by sending a written request to privacy@glassesusa.com. We are not required
to agree to the restrictions, except in the case where the disclosure is to a
health plan for purposes of carrying out payment or health care operations,
is not otherwise required by law, and the PHI pertains solely to a health
care item or service for which you, or a person on your behalf, has paid in
   full.
 * Inspect and obtain a copy of PHI. With a few exceptions, you have the right
to access and obtain a copy of the PHI that we maintain about you. If we
maintain an electronic health record containing your PHI, you have the right
to request to obtain the PHI in an electronic format. To inspect or obtain a
copy of your PHI, you must send a written request to privacy@glassesusa.com.
You may ask us to send a copy of your PHI to other individuals or entities
that you designate. We may deny your request to inspect and copy in certain
limited circumstances. If you are denied access to your PHI, you may request
   that the denial be reviewed.
 * Request an amendment of PHI. If you feel that PHI we maintain about you is
incomplete or incorrect, you may request that we amend it. To request an
amendment, you must send a written request to privacy@glassesusa.com. You
must include a reason that supports your request. If we deny your request for
an amendment, we will provide you with a written explanation of why we denied
   it.
 * Receive an accounting of disclosures of PHI. With the exception of certain
disclosures, you have a right to receive a list of the disclosures we have
made of your PHI, in the six years prior to the date of your request, to
entities or individuals other than you. To request an accounting, you must
submit a request in writing to privacy@glassesusa.com. Your request must
   specify a time period.
 * Notification of a Breach. You have a right to be notified following a breach
of your unsecured PHI, and we will notify you in accordance with applicable
   law.

Cookie Policy

We use certain monitoring and tracking technologies, including ones offered by
third party service providers to collect and process the personal information we
specified above. These technologies are used in order to maintain, provide and
improve our Platform on an ongoing basis, in order to provide a better
experience to our Users and to provide our customers and potential customers
with more relevant advertisements of our Services. For example, these
technologies enable us to: (i) keep track of and apply our Customer's and
Potential Customers’ Services and Website preferences and authenticated
sessions, (ii) better secure our Services by detecting abnormal behaviors, (iii)
identify technical issues and improve the overall performance of our Services,
(iv) monitor and analyze our ads’ performance (v) create and monitor analytics
relating to use of our Services, and (v) deliver to you targeted advertisements
that are more tailored to you based on your browsing activities and inferred
interests.

Based on the foregoing, we’ve adopted this policy ("Cookie Policy") to explain
how we use cookies and your related choices and options. In this policy we use
the term "cookies" to refer to any applicable tracking technologies described
herein.
1. WHAT ARE COOKIES
Cookies are a small text file that is stored on your device to help websites and
mobile apps remember things about you. Session-based cookies last only while
your browser is open and are automatically deleted when you close your browser.
Persistent cookies last until you or your browser delete them or until they
expire. To learn more about cookies, visit http://www.allaboutcookies.org/

Pixel Tags (also known as web beacons or clear GIFs) are transparent images,
iFrames, or JavaScript placed on a website or in marketing emails that are used
to understand how you interact with such websites or emails.

Social Media Features, such as the Facebook "like" button, may collect your IP
address and which page you are visiting on a website, and may set a cookie to
enable the feature to function properly. Your interactions with these features
are governed by the privacy policy of the company providing the relevant Social
Media Features.

Single Sign-on allows you to log in to certain websites or mobile applications
using sign-in services provided by third parties, such as Facebook or Google.
These services will authenticate your identity and provide you the option to
share certain personal information with us such as your name and email address
to pre-populate our sign-up form.
2. HOW WE USE COOKIES
The cookies we use can be classified in one of the following categories:

Strictly Necessary cookies We use these cookies to enable you to use our
Platform’s features, such as enabling movement between pages and remembering

be possible and our Website will not perform as it should. Security cookies We
use these cookies to help identity and prevent security risks. Authentication
cookies We use these cookies to help us show you the right information and
settings when you are logged-in to our website. Performance and Analytics
cookies We use these cookies to collect information about Users’ use of our
Platform to help improve the way the Platform works. An example is counting the
number of different people coming to our website or using a particular feature,
rather than the total number of times the site or feature is used. Without this
cookie, if you visited the website once each week for three weeks we would count
you as three separate users. We would find it difficult to analyze how well our
website was performing and improve it without these cookies. Functionality and
Preference cookies We use these cookies to remember the choices you make such as
which language you prefer and to provide you with personalized features.

Furthermore, we use cookies to track the items in your shopping cart and may use
that information to send you triggered email or text campaigns (e.g., sending
you personalized text messages such as shopping cart reminders). Targeting and
Advertising cookies We use these cookies to deliver you with ads when you browse
the web that are more relevant to you and your interests, by remembering
information such as what you looked at on our Platform and to track the
performance of these ads. Social Sharing cookies We use these cookies to allow
you to share content directly on the social network/sharing sites, like
Facebook, Twitter or Google+ on which you have a user account. Examples would be
if you wanted to "like" or "tweet" about a service or product advertised on our
website

Accessibility StatementCompliance status

We firmly believe that the internet should be available and accessible to anyone
and are committed to providing a website that is accessible to the broadest
possible audience, regardless of ability.

To fulfill this, we aim to adhere as strictly as possible to the World Wide Web
Consortium’s (W3C) Web Content Accessibility Guidelines 2.1 (WCAG 2.1) at the AA
level. These guidelines explain how to make web content accessible to people
with a wide array of disabilities. Complying with those guidelines helps us
ensure that the website is accessible to blind people, people with motor
impairments, visual impairment, cognitive disabilities, and more.

This website utilizes various technologies that are meant to make it as
accessible as possible at all times. We utilize an accessibility interface that
allows persons with specific disabilities to adjust the website’s UI (user
interface) and design it to their personal needs.

Additionally, the website utilizes an AI-based application that runs in the
background and optimizes its accessibility level constantly. This application
remediates the website’s HTML, adapts its functionality and behavior for
screen-readers used by blind users, and for keyboard functions used by
individuals with motor impairments.

If you wish to contact the website’s owner please use the website's form

Screen-reader and keyboard navigation

Our website implements the ARIA attributes (Accessible Rich Internet
Applications) technique, alongside various behavioral changes, to ensure blind
users visiting with screen-readers can read, comprehend, and enjoy the website’s
functions. As soon as a user with a screen-reader enters your site, they
immediately receive a prompt to enter the Screen-Reader Profile so they can
browse and operate your site effectively. Here’s how our website covers some of
the most important screen-reader requirements:

 1. Screen-reader optimization: we run a process that learns the website’s
components from top to bottom, to ensure ongoing compliance even when
updating the website. In this process, we provide screen-readers with
meaningful data using the ARIA set of attributes. For example, we provide
accurate form labels; descriptions for actionable icons (social media icons,
    search icons, cart icons, etc.); validation guidance for form inputs;
element roles such as buttons, menus, modal dialogues (popups), and others.

Additionally, the background process scans all of the website’s images. It
provides an accurate and meaningful image-object-recognition-based
description as an ALT (alternate text) tag for images that are not
described. It will also extract texts embedded within the image using an OCR
(optical character recognition) technology. To turn on screen-reader
adjustments at any time, users need only to press the Alt+1 keyboard
combination. Screen-reader users also get automatic announcements to turn
the Screen-reader mode on as soon as they enter the website.

These adjustments are compatible with popular screen readers such as JAWS,
    NVDA, VoiceOver, and TalkBack.

 2. Keyboard navigation optimization: The background process also adjusts the
website’s HTML and adds various behaviors using JavaScript code to make the
website operable by the keyboard. This includes the ability to navigate the
website using the Tab and Shift+Tab keys, operate dropdowns with the arrow
keys, close them with Esc, trigger buttons and links using the Enter key,
navigate between radio and checkbox elements using the arrow keys, and fill
them in with the Spacebar or Enter key.

Additionally, keyboard users will find content-skip menus available at any
time by clicking Alt+2, or as the first element of the site while navigating
with the keyboard. The background process also handles triggered popups by
moving the keyboard focus towards them as soon as they appear, not allowing
    the focus to drift outside.

Users can also use shortcuts such as “M” (menus), “H” (headings), “F”
(forms), “B” (buttons), and “G” (graphics) to jump to specific elements.

Disability profiles supported on our website
 * Epilepsy Safe Profile: this profile enables people with epilepsy to safely
use the website by eliminating the risk of seizures resulting from flashing
or blinking animations and risky color combinations.
 * Vision Impaired Profile: this profile adjusts the website so that it is
accessible to the majority of visual impairments such as Degrading Eyesight,
Tunnel Vision, Cataract, Glaucoma, and others.
 * Cognitive Disability Profile: this profile provides various assistive
features to help users with cognitive disabilities such as Autism, Dyslexia,
CVA, and others, to focus on the essential elements more easily.
 * ADHD Friendly Profile: this profile significantly reduces distractions and
noise to help people with ADHD, and Neurodevelopmental disorders browse,
read, and focus on the essential elements more easily.
 * Blind Users Profile (Screen-readers): this profile adjusts the website to be
compatible with screen-readers such as JAWS, NVDA, VoiceOver, and TalkBack. A
screen-reader is installed on the blind user’s computer, and this site is
   compatible with it.
 * Keyboard Navigation Profile (Motor-Impaired): this profile enables
motor-impaired persons to operate the website using the keyboard Tab,
Shift+Tab, and the Enter keys. Users can also use shortcuts such as “M”
(menus), “H” (headings), “F” (forms), “B” (buttons), and “G” (graphics) to
   jump to specific elements.

Additional UI, design, and readability adjustments
 1. Font adjustments – users can increase and decrease its size, change its
family (type), adjust the spacing, alignment, line height, and more.
 2. Color adjustments – users can select various color contrast profiles such as
light, dark, inverted, and monochrome. Additionally, users can swap color
schemes of titles, texts, and backgrounds with over seven different coloring
    options.
 3. Animations – epileptic users can stop all running animations with the click
of a button. Animations controlled by the interface include videos, GIFs,
    and CSS flashing transitions.
 4. Content highlighting – users can choose to emphasize essential elements such
as links and titles. They can also choose to highlight focused or hovered
    elements only.
 5. Audio muting – users with hearing devices may experience headaches or other
issues due to automatic audio playing. This option lets users mute the
    entire website instantly.
 6. Cognitive disorders – we utilize a search engine linked to Wikipedia and
Wiktionary, allowing people with cognitive disorders to decipher meanings of
    phrases, initials, slang, and others.
 7. Additional functions – we allow users to change cursor color and size, use a
printing mode, enable a virtual keyboard, and many other functions.

Assistive technology and browser compatibility

We aim to support as many browsers and assistive technologies as possible, so
our users can choose the best fitting tools for them, with as few limitations as
possible. Therefore, we have worked very hard to be able to support all major
systems that comprise over 95% of the user market share, including Google
Chrome, Mozilla Firefox, Apple Safari, Opera and Microsoft Edge, JAWS, and NVDA
(screen readers), both for Windows and MAC users.

Notes, comments, and feedback

Despite our very best efforts to allow anybody to adjust the website to their
needs, there may still be pages or sections that are not fully accessible, are
in the process of becoming accessible, or are lacking an adequate technological
solution to make them accessible. Still, we are continually improving our
accessibility, adding, updating, improving its options and features, and
developing and adopting new technologies. All this is meant to reach the optimal
level of accessibility following technological advancements. If you wish to
contact the website’s owner, please use the website's form

Performance Cookies

These cookies allow us to count visits and traffic sources so we can measure and
improve the performance of our site. They help us to know which pages are the
most and least popular and see how visitors move around the site. If you do not
allow these cookies we will not know when you have visited our site, and will
not be able to monitor its performance.

FUNCTIONAL COOKIES

Functional Cookies

These cookies enable the website to provide enhanced functionality and
personalization. They may be set by us or by third party providers whose
services we have added to our pages. If you do not allow these cookies then some
