PUT PREVENTION TO WORK

Welcome to Prevention4ME, which gives you and your doctor a personalized
assessment of your possible risk for developing breast and other cancers. As a
part of your routine screening and preventive care, this survey-based assessment
is a critical step to proactive early cancer detection and determining if you
are eligible for enhanced breast cancer screening and/or genetic testing.

Prevention4ME is one of the population-based screening and early detection
offerings from the Providence Genomics EDGE Program. EDGE programs help to
identify those who might have an increased risk for cancer due to genetic or
other risk factors.

READY FOR TESTING & FEEDBACK (BETA-V1)

Feedback on these topics is much appreciated:
 * Content, Questions, and Response Options
 * User experience; anything that is broken / doesn't work as expected
 * Images, layout, spacing, margins, padding etc
 * Design, colors, fonts, sizes, etc
 * User interface widgets, Form inputs, etc

Feedback on these topics is not needed (yet):
 * The Verify Appointment screen after login is not implemented yet
 * Mobile, tablet, and browser testing
 * Inputs are missing for third-degree relatives e.g. cousins etc

HOW TO CREATE A TEST ACCOUNT

When you reach the MyChart Account screen, click the `Sign Up Now` button and
then follow these steps to create a Test Account for testing. This same pattern
applies for testing in all Propel Programs.

 * You may use any Gmail, Google, or Microsoft 365 email address... including
   your Providence email address.
 * Use a "plus-address" by adding a `+` sign followed by any word or set of
   words before the `@` symbol in your email address.
 * Use the `+isbtest` plus-address to identify the account as a Test account.
 * Consider adding a number to the end of the plus-address to create infinite
   test accounts.



Putting it altogether, my test accounts are:

GET YOUR RISK CALCULATION.

After completing the assessment, you will receive a personalized risk assessment
and details summarizing the next steps, including the opportunity to receive
genetic testing if eligible.

GET YOUR MAMMOGRAM.

Go to your mammogram appointment as scheduled. Your assessment responses will
help you and your healthcare team personalize your screening.


ABOUT PREVENTION4ME

Identifying your risk for cancer helps you and your healthcare team create a
more personalized care plan to support early detection and even cancer
prevention.

Prevention4ME is a supplement to your annual mammogram. The online questionnaire
will walk you through a series of questions about your personal and family
history. Based on the results of your personal assessment, you might be eligible
for additional breast cancer imaging (like breast MRI) and/or genetic testing to
identify inherited risk for breast and other cancers.

After your assessment, you only need to provide updates or changes to your
personal/family health at future mammogram appointments. We do all the rest.
We’ll send you reminders to update your information and fine-tune your risk
assessment based on any changes you provide and the latest guidelines.


PERSONALIZED TO YOU

The Prevention4ME assessment uses your responses to calculate your Tyrer-Cuzick
(TC) score, which is an estimate of your lifetime risk of developing breast
cancer. It also assesses your eligibility for genetic testing according to the
National Comprehensive Cancer Network (NCCN) Guidelines to identify your
possible inherited risk of breast and other cancers.

Once your assessment is complete, you will receive a personalized risk
assessment, outlining steps you and your healthcare providers can take to create
a care plan just for you. For many women, their care will remain the same. For
some high-risk women, your healthcare provider might recommend more frequent
screening or other approaches to early cancer detection and risk reduction.
