Grand Rounds Health is a health benefit made available to you through your
employer. We are a guide and resource for your medical and health care-related
needs. Our services include finding you the top doctors and specialists in your
insurance network, booking doctor’s appointments on your behalf, getting you
remote second opinions from world-leading experts, answering any questions you
may have about your medical conditions, and more.
When should I use Grand Rounds Health?

Use Grand Rounds Health when:

 * You need a primary care physician or specialist for an in-person visit. We’ll
   help you find trusted and experienced doctors within your insurance network
   and can even set the appointment for you.
 * You’d like quick answers to medical questions. We can provide you with
   over-the-phone guidance and support from a Grand Rounds Health doctor.
 * You’re unsure about a diagnosis, or you’ve been recommended surgery as a form
   of treatment. We can arrange for you to get a remote second opinion from a
   leading expert specializing in your area of need.

Who can use Grand Rounds Health?

Grand Rounds Health is available at no additional cost to you and your
dependents who are enrolled in a PACCAR Premera Medical Plan. Your employer
provides this service so that you have access to the best medical care possible.

How much will this cost me?

Grand Rounds Health is fully covered by your employer and available at no
additional cost to you and your enrolled dependents. Plan guidelines will apply
for any treatments as a result of the Expert Opinion or Office Visit – Grand
Rounds Health will find the doctor, collect all relevant medical records, and
(if necessary) schedule the appointment at no additional cost.

What can I expect from Grand Rounds Health?

All Grand Rounds Health services can be easily accessed by calling our Care
Team, visiting app.grandrounds.com or by downloading the Grand Rounds Health
mobile app. For certain Grand Rounds Health services, a member of your Care Team
will reach out to you to learn about your exact needs. They’ll take care of
everything, from finding you the right doctor to collecting your medical
records. Expect a care coordinator to follow up with you after you’ve seen the
recommended doctor, to answer any questions you may have and start additional
cases if needed.
What areas does Grand Rounds Health specialize in?

Grand Rounds Health physicians span all conditions and specialty areas,
including: back and knee pain, cancer, heart disease, arthritis, behavioral
health disorders, migraines, digestive system disorders, dermatology,
pediatrics, ACL tears, and much more.
How does Grand Rounds Health select its physicians?

Grand Rounds Health selects physicians based on their education and training,
patient outcomes, and location. As a result, we’re able to match you with highly
experienced doctors and specialists who are best suited to your personal
preferences and medical needs.
How much will it cost me to see a recommended doctor?

Typically, you’ll only pay your regular copay and office visit fees for seeing
an in-network doctor. For further details, call the Grand Rounds Health Care
Team, available Monday through Friday from 8:00am - 9:00pm ET.
What will my doctor think about Grand Rounds Health reviewing my case?

Most doctors are glad to get the opinions of their peers when assessing
complicated medical issues. Their primary goal is to get you feeling better. A
good way to frame a conversation with your treating physician regarding a Grand
Rounds Health expert opinion is:

The last four digits of your SSN are needed to confirm both your identity when
creating an account and your eligibility for the Grand Rounds Health benefit.
