<p>You will receive information regarding the benefit enrollment process from the Benefits Center in<span style="text-decoration: underline;"><strong> 7–10 days</strong></span> after you have passed your pre-employment
physical, background check and drug test. This information will come to you either by email or regular mail depending on whether you have provided a benefit email address. Once you are an active employee, you can
enroll online and talk with a service representative. <br><br>You will need to enroll in your Health & Welfare benefits within <span style="text-decoration: underline;"><strong>31 days</strong></span> from your
date of hire.</p>

<p>Coverage begins on your date of hire, assuming you enroll by the deadline. If you make no elections within 31 days, your default coverage will take effect and continue through December 31. Your next opportunity to
change your benefits will be Annual Enrollment, unless you have a qualifying change in family or employment status. <br><br>If you need to seek medical care or fill a prescription prior to receiving your medical ID
card, you will need to pay in full at the time of service and file a paper claim for reimbursement with the carrier once coverage has been established.</p>

<li><strong>Health Savings Account (HSA)</strong>: No participation.</li>
<li><strong>Long-term disability (LTD) buy-up</strong>: No participation.</li>
<li><strong>AEP System Retirement Plan (Pension Plan)</strong>: Automatic enrollment after one year.</li>
<li><strong>AEP Retirement Savings 401(k) plan</strong>: Automatic enrollment after 21 days at 3% of your eligible compensation on a before-tax basis contributed to the applicable target retirement fund.</li>
<li><strong>Vacation</strong>: Automatic eligibility.</li>
</ul>
<h4>For full-time employees only:</h4>
<p>In addition to the coverage listed above, full-time employees will automatically receive coverage under certain company-paid benefits as follows:</p>
<ul>
<li><strong>Life insurance</strong>: 2x annual base pay for yourself.*</li>
<li><strong>Supplemental life insurance</strong>: No coverage. </li>
<li><strong>Dependent life insurance</strong>: No coverage.</li>
<li><strong>Accidental death & dismemberment (AD&D) insurance</strong>: 2x annual base pay for yourself.*</li>
<li><strong>Supplemental AD&D insurance</strong>: No coverage.</li>
<li><strong>Dependent AD&D insurance</strong>: No coverage.</li>
<li><strong>Long-Term Disability (LTD) Plan</strong>: Automatic eligibility for 60% base plan.</li>

REVIEW YOUR OPTIONS
View your benefit options below to help you decide which benefits are right for
you and think about who you want to cover.
If you plan on covering dependents, you will need their name, birthdate and
social security number when you enroll. You will be able to determine who you
would like to cover in each plan when you enroll.
STEP TWO
CHOOSE HOW YOU ENROLL IN YOUR BENEFITS
1. Online | Register and login to Benefits on the Home page. Our company key is
COMPANY (uppercase). You can use any device to access this site. Once you
log in, click the Start Here button to begin your enrollment.
2. Mobile App | Once you login, download the MyChoice Mobile App, and use your
Access Code to activate the Mobile App to enroll.
3. Talk | Call the Benefits Center at 1-888-123-1234, Monday-Friday 8 a.m. - 8
p.m. EST
STEP THREE
DOWNLOAD AND PRINT YOUR BENEFIT SUMMARY FOR YOUR RECORDS
You may be asked to elect beneficiaries or provide documentation to prove your
relationship with dependents before they will be added to your coverage. Be sure
to check the To Do list for any important notices.

WHEN WILL I ENROLL IN MY BENEFITS?
You will receive information regarding the benefit enrollment process from the
Benefits Center in 7–10 days after you have passed your pre-employment physical,
background check and drug test. This information will come to you either by
email or regular mail depending on whether you have provided a benefit email
address. Once you are an active employee, you can enroll online and talk with a
service representative.
You will need to enroll in your Health & Welfare benefits within 31 days from
your date of hire.
WHEN DOES MY COVERAGE BEGIN?
Coverage begins on your date of hire, assuming you enroll by the deadline. If
you make no elections within 31 days, your default coverage will take effect and
continue through December 31. Your next opportunity to change your benefits will
be Annual Enrollment, unless you have a qualifying change in family or
employment status.
If you need to seek medical care or fill a prescription prior to receiving your
medical ID card, you will need to pay in full at the time of service and file a
paper claim for reimbursement with the carrier once coverage has been
established.
WHAT IF I DON'T ENROLL?
If you make no elections within 31 days, your default coverage will take effect
and continue through December 31. Your next opportunity to change your benefits
will be Annual Enrollment, unless you have a qualifying change in family or
employment status.
DEFAULT COVERAGE:
* Medical: Coverage in the HSA Basic medical plan option, covering you only; no
coverage for your family. Your share of the cost will be deducted from your
first and second paychecks each month on a before-tax basis.
* Dental: No coverage.
* Vision: No coverage.
* Health Care Flexible Spending Account (HCFSA) and Dependent Care Flexible
Spending Account (DCFSA): No participation.
* Health Savings Account (HSA): No participation.
* Long-term disability (LTD) buy-up: No participation.
* AEP System Retirement Plan (Pension Plan): Automatic enrollment after one
year.
* AEP Retirement Savings 401(k) plan: Automatic enrollment after 21 days at 3%
of your eligible compensation on a before-tax basis contributed to the
applicable target retirement fund.
* Vacation: Automatic eligibility.
FOR FULL-TIME EMPLOYEES ONLY:
In addition to the coverage listed above, full-time employees will automatically
receive coverage under certain company-paid benefits as follows:
* Life insurance: 2x annual base pay for yourself.*
* Supplemental life insurance: No coverage.
* Dependent life insurance: No coverage.
* Accidental death & dismemberment (AD&D) insurance: 2x annual base pay for
yourself.*
* Supplemental AD&D insurance: No coverage.
* Dependent AD&D insurance: No coverage.
* Long-Term Disability (LTD) Plan: Automatic eligibility for 60% base plan.
* Sick Pay Program: Automatic eligibility.
* Personal days off (PDOs).
*If you are a rehired employee who, upon leaving on or after January 1, 2013,
elected to port or convert your group life insurance/AD&D insurance coverage,
the ported/converted coverage must be surrendered once you are again eligible
for group life insurance/AD&D insurance coverage.

WHAT BENEFITS ARE AUTOMATIC?
Full-time employees will receive company-paid basic life insurance, basic
accidental death & dismemberment (AD&D) insurance, sick pay, Long-Term
Disability (LTD) Plan and retirement plan benefits immediately upon hire.
Full-time employees will also have automatic eligibility for company-paid
vacation, holidays and personal days off.
