guarantee that a prescription will be written. The Virtual Primary Care and
TeleCounseling program does not prescribe DEA controlled substances, lifestyle
drugs and certain other drugs which may be harmful because of their potential
for abuse. The Virtual Primary Care and Telecounseling program physicians
reserve the right to deny care for potential misuse of services. The Virtual
Primary Care and TeleCounseling program operates in all states and is subject to
state regulations. QuestSelect- Not all labs that can be prescribed are included
in this program. Provider collection and handling fees may apply, and are

not provide access to all generic medications or reimburse for all
prescriptions. Brand name and Specialty drugs are specifically excluded. The

recommendations. The maximum per-claim benefit on a generic medication is
