CREDENCE INFORMATION REQUEST

To complete this Information Request you will be required to supply certain
information which, depending on the level of screening, may include details of
address history, employment details, previous education and professional
certifications. You may also be asked to provide details of criminal records or
bankruptcy.

While you may save this form and return to complete it at a later time, it is
advisable to have the required information on hand.

The application form will time-out after 20 minutes of inactivity and therefore
we would recommend that you regularly save the form. If you have not saved it,
you will be reminded to do so before it times-out. Before you 'Log In', refer to
your welcome email for your 'Access Code' as this is your password.
