When you need home care or hospice services for the first time, everything can
feel unfamiliar and overwhelming. Fortunately, we’ve been here for over twenty
years, we understand what you're feeling, and it's why we created our Care
Navigation program. Over the past couple decades, our team of Care Navigators,
advocates and activists have learned the workarounds and built the industry
relationships that lead to solutions for all people, under any circumstances. At
Integrity, we'll dedicate a point-person from our Care Navigation team to learn
your specific needs who will match you with the services, support and resources
you need to live life on your terms.

HOME CARE

When you find yourself or a loved one needing regular assistance with light
housekeeping, running errands, meal prep, self-care, or even simply seeking
companionship care, Home Care is most likely the service that will best address
your needs. Integrity's Home Care teams are also equipped to handle advanced
nursing tasks such as ostomy hygiene and catheter care, and will work with you
to design a care plan specific to your personal needs as they change over time.

HOSPICE

Often misunderstood, but truly beautiful, Hospice is a concept of care that
prioritizes comfort, peace and support for the terminally ill and their
families. Our highly skilled and compassionate Hospice teams will gently lead
you through this last chapter of life, focused on maintaining the dignity and
beauty of life in all its stages—even at the very end. Integrity Hospice
Services can begin same-day, and our team will immediately handle every
detail—from medical equipment procurement and delivery, to ongoing pain
management—with grace and a quiet strength that will allow you to let go of the
details and focus on the things that matter most.

HOME HEALTH + REHABILITATION

If you have a medical condition that requires temporary care and/or therapies to
restore your health after an injury, surgery, or illness requiring
hospitalization, Home Health & Rehabilitation services are most likely the best
fit. Utilizing Home Health & Rehab after a hospitalization will reduce your
chances of being re-admitted, while keeping you safe, healthy, and comfortable
in your own home. Our skilled Care Navigators will handle the transitions from
hospital to home, seamlessly—all you have to do is give us a call—we'll take
care the rest.

CONSUMER DIRECTED SERVICES

CDS stands for Consumer Directed Services which is a Missouri Medicaid program
that is authorized through the Department of Health and Senior Services (DHSS).
This program will provide compensation to a loved one who is caring for you at
home as long as that loved one is not a spouse.

LOVE NOTES

Sometimes it's the little things that make a big difference, and sometimes it's
the big things. Here are the things that have made a difference to our Integrity
family.

I lost a client about 7 mos ago, I was lucky to be in his life from day one and
I was around him when he was passing. He kept calling my name. I was so sad for
him, but at the same time I knew I had grown to become a part of his heart. So
if you wonder if this (type of work) is worth the pain, I say YES, YES, YES it
is! Because just knowing I made a positive difference in their life brings me
peace.

My dad is at the point where he needs more permanent home health care. Not
living in Columbia, I had no idea where to start. I had a list of places and I
even asked for Facebook recommendations. As I was looking at the list, I asked
GOD to tell me which one to call. I called Integrity Home Health + Hospice and
spoke with Dan.
Dan went to my dad's house and immediately recognized a picture of my cousin's
daughter on the mantle. Dan told us he's known my cousin Tiffany since he was 7
years old and lived close to our family farm. As Dan was finishing up the
paperwork, he told me and my dad (I participated by phone) that Integrity is a
Christian based organization and they HONOR GOD IN EVERYTHING THEY DO! I
couldn't contain my PRAISE. My dad feels good about this. This is a GOD
CONNECTION.
