El juego fue desarrollado por Square, publicado en Japón en 1999 y tanto en Norteamérica como, a diferencia del juego anterior, en regiones PAL en 2000. Es la secuela de Parasite Eve y el segundo juego de la serie del mismo nombre. Tres años después de los acontecimientos del primer juego, Aya Brea, ahora una agente del FBI que trabaja en su clandestino Equipo de Supresión e Investigación Mitocondrial (MIST), es enviada al centro comercial Akropolis Tower en Los Ángeles para respaldar al equipo SWAT de LAPD en respuesta. a informes de avistamientos de criaturas neomitocondriales (NMC). Mientras Aya y su compañero Rupert Broderick investigan la torre, descubren una nueva forma de NMC con el poder de hacerse pasar por humanos y que fue capaz de masacrar a los socorristas.
