HAVE QUESTIONS?

Where are you located?

HRC Fertility has nine locations throughout Southern California, including:
Pasadena, San Diego, Encino, West LA, Westlake Village, Newport Beach, Rancho
Cucamonga, Fullerton, and Carlsbad. We not only work with patients from
Bakersfield to San Diego, but nationally and internationally as well.

What should I bring to the initial visit?
 * In order to assist your doctor, it is preferred that you submit medical
   records or lab results in advance so there is adequate time to review your
   medical history in detail.  If needed, we can also provide you with a medical
   records request.
 * HRC will mail you a welcome packet to bring with you on the day of your
   consultation.  We include a patient questionnaire that you will need to have
   completed prior to your appointment.  You can also prepare questions for your
   doctor prior to your appointment using the You and Your Doctor form.

Planning Your First Visit
Do you take insurance?
 * In order to verify your insurance benefits, please provide us with a fax copy
   of your insurance card or you can provide the information over the phone.  We
   are currently contracted with most PPO plans and select HMO groups. If you
   have an HMO, an authorization must be submitted prior to your appointment.
    We have also created a downloadable Insurance Question form to help you when
   gathering information from your insurance carrier.

What are some causes & risks of infertility?

Some leisure activities, jobs, and the use of drugs and medications may all
increase the chances of infertility. On the positive side, some lifestyle
changes may bolster a couple’s chances of having a family. Factors that can
influence fertility include:
