TELEHEALTH INTERVENTION IMPROVES LIVER CARE IN BLACK, LATINO PATIENTS WITH
DIABETES

A telehealth intervention may help manage type 2 diabetes (T2D) and preventing
metabolic dysfunction-associated steatotic liver disease (MASLD) in patients who
are Latino or Black, according to results of a pilot study published in Diabetes
Therapy.

The pilot study included 10 participants with T2D who were part of the Duke
Healthcare System. The aim was to evaluate the feasibility and acceptability of
a telehealth program to deliver evidence-based management (EBM) of MASLD in a
Latino and Black population.

Over 3 months, participants completed at least 3 study visits, during which they
received MASLD education, diet/lifestyle counseling, T2D medication adjustment
to improve MASLD, and clinically indicated tests and referrals as needed. All
visits were conducted with a board-certified endocrinologist via Zoom or
telephone.

After the study completion, the endocrinologist contacted each participant’s T2D
provider with advice on annual testing, risk stratification, and diet/lifestyle
efforts to aid in continued MASLD prevention and management. Participants were
surveyed at baseline and 3 months, along with interviews following study
conclusion.

The study had a 100% retention rate, and all scheduled visits were completed. At
the end of the study, all participants reported improved awareness of MASLD and
its link to T2D. During their sessions, all participants had either clinically
indicated labs ordered or T2D medications adjusted to improve MASLD. Two
participants had liver ultrasound ordered, “both of whom were found to have
hepatic steatosis,” the authors noted.

One patient was referred to hepatology for elevated alanine aminotransferase
levels. Five participants were found to be non-immune to viral hepatitis A
and/or B and advised to receive vaccinations for liver protection.

“In this study, our telehealth approach to delivering EBM of MASLD was feasible
and acceptable to a small cohort of Latino/a and Black patients with T2D, and
there were opportunities to better align each participant’s care with current
guidelines,” the authors summarized. “While each component of our intervention
is based solidly in guidelines and best practice recommendations, combining
these components into a telehealth intervention to improve MASLD care is novel,
and has the potential to reach populations that are not engaging in clinic-based
care.”
