TWITTER HIT WITH $250M LAWSUIT FROM MUSIC PUBLISHERS OVER “MASSIVE COPYRIGHT
INFRINGEMENT” CLAIM

Twitter’s longstanding refusal to secure music licensing rights has come to a
head with a lawsuit accusing the company of mass copyright infringement.

The three major music conglomerates — Universal, Sony and Warner — joined by a
host of other publishers on Wednesday sued Twitter for at least $250 million
over the alleged infringement of roughly 1,700 works for which it received
hundreds of thousands of takedown notices. They allege the company “consistently
and knowingly hosts and streams infringing copies of music compositions” to
“fuel its business.” Twitter has rebuffed calls for it to obtain the proper
licenses, according to the suit.

Twitter is the lone major social media platform without music licensing deals,
which permits those sites to legally host videos and other content featuring
publishers’ music. Facebook, Instagram, TikTok, YouTube and Snapchat have all
entered into agreements that compensate creators of musical compositions for use
of their work.

The suit filed in federal court in Tennessee accuses Twitter of intentionally
fostering a user base that posts video overlaid with copyright-protected music
to “retain account holders and viewers” and “grow the body of engaging tweets”
on the platform. “Twitter then monetizes those tweets and users via advertising,
subscriptions, and data licensing, all of which serve to increase Twitter’s
valuation and revenues,” states the filing.

The complaint points to a tweet from a known repeat offender who has been the
subject of at least nine infringement notices to Twitter. The publishers say
Twitter has profited off of the infringement by posting a “promoted” tweet
directly below the post and recommending a paid “promoted” account directly to
its right.

In marketing and securities filings, Twitter has represented that video drives
high levels of engagement and that the majority of its users watch video
content.

“The availability of videos with music, including copies of Publishers’ musical
compositions, furthers Twitter’s financial interests both because it drives user
engagement, and thus advertising revenue, and because Twitter does not pay fees
to license musical compositions from Publishers and other music rights holders,”
writes the publishers’ lawyer Steven Riley in the suit. “Indeed, providing free,
unlicensed music gives the Twitter platform an unfair advantage over competing
platforms.”

In December 2021, the National Music Publishers’ Association, acting on behalf
of the publishers, began sending formal infringement notices to Twitter on a
weekly basis, according to the complaint. The company was notified of over
300,000 infringing tweets. Despite this, Twitter “often waited weeks, a month,
or even longer” before removing the posts “if it acts at all,” the suit says.

The publishers stress that Twitter failed to implement a policy to terminate
repeat infringers. In its policy, the company addresses issues concerning
copyright complaints, including what happens to accounts that receive one or
more copyright complaints, but neglects to carry a warning to users that they
may be terminated for repeat infringement despite having one prior to 2019.

“Users who repeatedly infringe copyrights do not face a realistic prospect of
permanently losing access to their accounts on the Twitter platform,” the suit
states. “Rather than terminating access to specific users brought to its
attention as infringers, which would have stopped or limited their infringement
and deterred others from infringing, Twitter has operated its platform to be a
haven for infringing activity.”

Under the Digital Millennium Copyright Act, Twitter can limit its liability over
infringement by its users but only if it took reasonable steps that provide for
the termination of accounts held by repeat offenders. Production companies have
been taking Verizon Wireless, AT&T and Comcast to court for allegedly
facilitating mass piracy of their movies, arguing that the internet service
providers aren’t protected by the DMCA, which provides for notice and takedown
of content that infringes on copyrights, because they turned a blind eye to
customers who illegally distribute and download pirated films.

Anticipating a fair use defense from Twitter, the publishers also argue that the
purpose of using their music on Twitter is to maximize views and make posts more
compelling, “not to engage in political or newsworthy speech, or commentary or
criticism.” They cite a tweet with the official music video of Rihanna’s
“Umbrella” that states, “15 years ago, Rihanna released ‘Umbrella.’” The post
got 221,000 views and nearly 15,000 likes.

Last year, Elon Musk tweeted that “current copyright law in general goes
absurdly far beyond protecting the original creator,” according to the
complaint.

The suit, which seeks $150,000 for each infringed work, claims direct copyright
infringement, contributory infringement and vicarious infringement. The 17
publishers that sued are represented by the National Music Publishers
Association. They include Anthem Entertainment, Big Machine Music, BMG Rights
Management, Kobalt Music Publishing America, Spirit Music Group, among others.

In a statement, NMPA president David Israelite said Twitter “knows full well
that music is leaked, launched, and streamed by billions of people every day on
its platform.” He added that the company can’t “hide behind the DMCA.”

Twitter was exploring the licensing of music rights from three major labels
before Musk’s takeover of the company, The New York Times reported in March.

In a letter to then-CEO Jack Dorsey sent in 2021, more than 20 members of
Congress called on the company to adopt “robust content protection technology”
to decrease the posting of infringing content. Twitter in January was hit with a
similar suit from celebrity photo agency Backgrid over the alleged infringement
of thousands of its photographs by users on the platform.

INSTRUCTIONS



TO MAKE THE CRUST:

 1. Preheat the oven to 350 degrees Fahrenheit.
 2. Cream together the butter and sugar substitute.
 3. Add the coconut, coconut flour and baking powder, mixing until thoroughly
    combined.
 4. Press evenly into a parchment lined or lightly greased 9 x 9 pan. 
 5. Bake for 8 minutes.
 6. Remove from the oven and set aside until the filling is ready.

TO MAKE THE CHEESECAKE FILLING:

 1. Beat the cream cheese and sugar substitute together until smooth.
 2. Add the egg and vanilla extract and mix until thoroughly combined.
 3. Pour mixture over the crust.
 4. In a small bowl, mash the raspberries and sugar substitute together with a
    fork.
 5. Drop by spoonfuls over the cheesecake mixture and then swirl it gently with
    the fork until distributed over the entire top – don’t over mix.
 6. Bake in a 350 degree (F) oven for 30 minutes or until set, with just a
    slight jiggle in the middle when shaken.
 7. Remove and chill for at least 1 hour before cutting and serving.

up. All the hardware is included in the packaging, so as long as you have a
place picked out to mount the camera, a screwdriver or drill handy, and a power
outlet close by, the H8 Pro 3K will be ready to scan your yard in no time.

You do, of course, need to download the Ezviz app and follow the steps to get
the H8 Pro 3K online and active. That’s just as simple of a task, though, with
the added benefit of the app prompting you to adopt security features like
two-factor or biometric authentication as you get started.


IT OFFERS 360 DEGREES OF SECURITY

At a glance, the biggest draw for the H8 Pro 3K is twofold: it boasts a 3K
resolution video feed—a rarity in the smart security camera space—and it
features pan/tilt functionality that paves the way for a full field of view. I’m
here to tell you that these aren’t just buzzwords to get the H8 Pro’s foot in
the door. They’re the real deal.

I mounted the H8 Pro in the corner of our yard, and the pan/tilt feature allowed
me to not only see the entirety of our backyard, but I could rotate the camera
around to get a clear line of sight down our side yard.

The 3K resolution comes in quite handy with keeping track of what’s happening in
the space.

If you’ve ready my past outdoor camera reviews, you know that we have a friendly
African Sulcata tortoise named Darwin roaming the yard. With some cameras, it
would be difficult to spot Darwin if he was too far away. But the H8 Pro 3K had
such depth and clarity that we never lost sight of the 60-pound dinosaur.

On a more serious note, the H8 Pro 3K has a security feature tied to its
pan/tilt functionality called Auto Zoom Tracking. This setting will detect human
motion, then follow them and zoom in as they move across the field of view.

The goal is to not have unwanted individuals moving through your yard. Should
that ever happen, though, the H8 Pro 3K won’t miss a step.


THIS EZVIZ CAMERA HAS A LOT OF USEFUL FEATURES

While pan/tilt and 3K resolution may be the marquee features, they are far from
the only tools that the H8 Pro has on its belt. Ezviz built in a number of
AI-enabled features to improve your experience with the camera.

The company says its on-board AI algorithm will automatically reduce alerts that
may be caused by leaves or insects. Elsewhere, you can enable a feature that
allows visitors to initiate a video call to your phone simply by waving their
hand at the camera.

There’s also as many as 12 customizable detection spots that you can pinpoint
through the Ezviz app, which essentially instructs this model Ezviz camera to to
focus specifically on those areas.

Like many outdoor security cameras these days, the H8 Pro also comes with
features like color night vision, a built-in microphone for two-way audio, and a
piercing siren to help deter intruders.

Finally, while you can opt into subscription-based cloud storage, the H8 Pro has
the ability to accept local storage with a microSD card.


WHAT WE DON’T LIKE

The EzViz H8 Pro 3K outdoor camera boasts a 3K resolution video feed—a rarity in
the smart security camera space – and it features pan/tilt functionality that
paves the way for a full field of view.

Plain and simple, the Ezviz app isn’t perfect. It has its fair share of
benefits, but on occasion it became extremely laggy. It didn’t happen very
often, but there were multiple instances where there was a significant delay
between signaling the camera to pan and it actually moving.

There were also situations when the camera feed refused to pull up, forcing me
to close the app out completely before it would retrieve a live feed.

Now, to be transparent, it could very well be a Wi-Fi issue, since it is in a
remote section of my backyard. But there are other security cameras within feet
of the H8 Pro, and they have significantly less issues with delays.

Plus, our experience with the Ezviz C3W Pro (in the same location as the H8 Pro)
produced similar problems.


THE POWER CABLE IS SUPER SHORT

The soapbox that I will stand on today is this: if you have an outdoor camera
that runs on 12-volt power, it should come standard with at least a 10-foot
power cable. Instead, the H8 Pro 3K has a roughly 5-foot cable that severely
limits where you can place it in your yard without the addition of an extension
cord.

Some extension cords can be cheap, but it doesn’t help that you may have to
purchase an entirely different product just to be able to use the H8 Pro in your
space.


PRIVACY

The Ezviz app itself encourages two-factor and biometric authentication to
protect from unwanted sources accessing your feeds.

Ezviz also touts that it has won awards for information security, personal data
protection and cloud security as a result of its efforts.

Ultimately, smart home tech is never 100% free from security concerns, but Ezviz
has made a genuine effort to ensure the safety of customers’ data.


SHOULD YOU BUY THE EZVIZ H8 PRO 3K?

But to really nail the value element, the H8 Pro is packed with plenty of other
features that make functionality and security a priority.

There are some scattered instances where the app can’t quite catch up with the
camera, causing delays and sometimes failing to pull up the audio feed, period.
The power cable is also very short for a security camera that is meant to be an
outdoor installation.

If you’re enamored by the H8 Pro 3K, you’re going to have to accept those
blemishes.

In the grand scheme of things, those are small hiccups in exchange for a smart
home camera that adds real utility to your outdoor security situation. That’s
what the H8 Pro 3K was built to be, and that’s precisely what it turned out to
be.

ECLAMPSIA: WHAT IT IS AND WHO’S AT RISK

Olympic sprinter Tori Bowie died of complications during childbirth at age 32,
according to her autopsy, which was released this week. She was eight months
pregnant at the time of her death.

Respiratory distress and eclampsia were listed as potential complications,
according to The New York Times.

Bowie’s death underscores the severity of the maternal mortality problem in the
United States, which has a maternal mortality rate that is double — if not more
— than other developed countries like France, Germany, the United Kingdom and
Sweden. Her death also underscores the severity of the Black maternal mortality
crisis in this country. Black women are three times more likely than white women
to die from pregnancy-related causes, according to the Centers for Disease
Control and Prevention.

The World Health Organization reports that among the causes of maternal death
are severe bleeding after childbirth, infections after childbirth and
preeclampsia and eclampsia, or high blood pressure during pregnancy.

And while you may be unfamiliar with eclampsia, you may be aware of preeclampsia
— and the two are related. “Eclampsia is a rare but serious complication of
preeclampsia,” according to the Cleveland Clinic website.

So, what is eclampsia (and preeclampsia) and what are the signs? Here’s what to
know.




WHAT IS ECLAMPSIA?



Eclampsia is when seizures occur when someone is pregnant or after they give
birth. It is usually a severe outcome of preeclampsia, which is a pregnancy
complication in which someone has high blood pressure and excess protein in
their urine, said Dr. Alison Cowan, an OB-GYN in Colorado and head of medical
affairs at Mirvie, a health technology company that is working to develop blood
tests to determine one’s pregnancy risks.

Eclampsia can also develop if a pregnant person has HELLP syndrome (which stands
for Hemolysis, Elevated Liver enzymes, Low Platelets) or gestational
hypertension, said Dr. Ahizechukwu Eke, director of research in the division of
maternal fetal medicine at Johns Hopkins University School of Medicine in
Baltimore. But preeclampsia is the most common reason why eclampsia develops.

According to the March of Dimes, preeclampsia occurs in 5% to 8% of pregnancies.
Eke said severe preeclampsia occurs in .6% to 1.2% of pregnancies in Western
countries, and eclampsia occurs in 2% to 3% of people with severe preeclampsia.

While eclampsia overall is not common, it is serious. It’s also considered a new
occurrence, meaning the person who suffers from it usually does not have a
history of seizures, according to Eke.

WHAT ARE THE WARNING SIGNS OF ECLAMPSIA?



There are a number of eclampsia warning signs. The biggest one is high blood
pressure. According to Eke, around “75% of [people] who have eclampsia will have
a history of high blood pressure.”

About two-thirds of patients with eclampsia would also have a history of severe
headaches prior to the seizure, Eke said. Additional symptoms include pain in
the upper right abdomen, less frequent urination, dark pee, vision issues like
blurred eyesight, swollen hands and feet, and nausea and vomiting, he noted.

While 75% of people who are diagnosed with eclampsia do have symptoms predating
the seizures, unfortunately and terrifyingly, 25% do not have any symptoms —
“and then all of a sudden, boom, they just have a seizure,” Eke said.

The warning signs of preeclampsia are similar, according to Cowan.

“Symptoms that can commonly occur with preeclampsia are severe headache that
doesn’t improve with Tylenol, changes in your vision … you can have pain in the
right upper abdomen,” Cowan said. Swelling is another symptom, she said, but it
can be confusing since swelling can happen in pregnancy regardless.

Vomiting, high blood pressure and shortness of breath are additional signs of
preeclampsia, according to Penn Medicine.




WHO IS AT RISK OF DEVELOPING ECLAMPSIA?



As mentioned, having preeclampsia is the most common risk factor for developing
eclampsia. Additionally, if it’s the person’s first pregnancy, they are at risk,
too, Eke said.

Chronic hypertension and chronic kidney disease are risk factors, as well, along
with your age. If you’re younger than 16 or older than 35, your chances of
developing the condition are greater, he noted. People who are pregnant with
multiple children — like twins, triplets or quadruplets — also have a higher
risk.

“And then it tends to be more common in minority racial and ethnic groups, most
especially Black women,” Eke said.

It’s important for providers to thoroughly check all patients for risk factors
and to inform them of the symptoms of preeclampsia and eclampsia, but it’s
especially crucial for providers to exhaust all potential risks and
complications when it comes to Black patients, since eclampsia is more common in
this group, Eke said.

Additionally, if you feel that you are at risk and not getting the care and
attention you need, you can find another doctor.

“For everyone regardless of their risk, they need to know the signs and symptoms
and they need to be aware of when to call their doctor or midwife for help,”
Cowan added.

“The biggest thing about prevention is early recognition of symptoms. So these
patients typically will need to call their doctors when they start having
[symptoms],” Eke said.

This is especially true for patients with a history of high blood pressure. Eke
said many doctors will encourage people to “have a blood pressure cuff at home,
and monitor your blood pressure at least once a day.”

You can find at-home blood pressure cuffs at stores like Walgreens, Walmart and
on Amazon. “Regular monitoring at home is more accessible than it ever has been
before,” Cowan said.

“I never want people to feel [anxious] or afraid of becoming pregnant because of
these stories, because the key thing is that, number one, there is a lot we can
do to prevent preeclampsia, and number two, if we aren’t able to prevent it
entirely, if we recognize it, it’s manageable — we can do everything possible to
protect mom and baby as long as we’re able to recognize that it’s there,” Cowan
said.

When it comes to the prevention and management of preeclampsia, Cowan said a
baby aspirin can be an effective tool in preeclampsia treatment, and so can
blood pressure management. Additionally, certain lifestyle behaviors like
exercise and eating well can help prevent it, too.

“If you put those all together, it’s actually quite a comprehensive list of
things that clinicians can be doing and that pregnant individuals can be doing
to decrease their risk of preeclampsia as much as possible,” she said.

As far as what Cowan wants everyone patient to know, she said it’s important to
talk to your doctor about your risk and the symptoms.

And, if you are diagnosed with preeclampsia, make sure you know what you need to
do and have an emergency plan in place with your doctor, she noted.

“I think knowledge is just that first step to prevention,” Cowan said.

INGREDIENTS

The ingredients in this keto beef stew are both simple and flavorful.

Unlike traditional beef stew, ingredients like starchy root vegetables and flour
are substituted with low-carb alternatives such as turnips, green onions, and
fewer carrots. You’ll hardly notice the difference!

 * Beef Stew Meat – use a beef chuck roast or round roast and cut it into
   bite-sized pieces for the stew. You can also buy precut stew meat. I prefer
   grass fed beef, but use whatever fits within your own budget.
 * Olive Oil – a light-tasting oil is used to brown the meat and saute perfectly
   cooked vegetables.
 * Green Onions – the whole spring is used including the white part. Green
   onions have fewer carbs than bulb onions.
 * Garlic – pre-minced or mince yourself at home.
 * Vegetables – turnips are used in place of potatoes, and only one carrot is
   needed.
 * Seasoning – Italian seasoning, bay leaves, and salt and pepper are all
   needed.
 * Beef Broth – this is a great opportunity to use beef bone broth if it is in
   your budget to purchase or make your own Instant Pot beef bone broth!
 * Tomato Paste – adds a savory tomato flavor to the stew. It will also help the
   stew thicken as it simmers.
 * Apple Cider Vinegar – adds a bit of tangy to really bring all the flavors
   together.

INSTRUCTIONS

Making this hearty keto beef stew is a simple process.

Start by prepping your meat and vegetables. You will want to peel and cut all
your vegetables in advance, except the turnips.

The turnips are added last after the stew has simmered. I recommend waiting to
prep them until the end so they don’t turn brown sitting out.

Heat the olive oil in a large pot or Dutch oven over medium-high heat. Then, add
the beef stew meat and brown on all sides, about 5-7 minutes. Remove the beef
from the pot and set aside.

Next, add the carrots, and the white part of the green onions, to the pot and
sauté until the vegetables are tender, about 5-7 minutes. Then stir in the
Italian seasoning, salt, pepper, and garlic. Saute until fragrant, about 1
minute.

Add the tomato paste, beef broth, and bay leaves. Stir to combine. Let the stew
come to a boil then cover and reduce the heat to low and simmer for 45-60
minutes, until the beef is tender

Peel and cut the turnips to 1-inch cubes. Remove the lid from your pot. Add the
turnips. Increase to medium-high and bring back to a boil. Boil for 15-20 or
until the turnips are soft and the stew has started to thicken.

Remove the stew from the heat and serve topped with the remaining green onions.
This will add flavor and give color to your stew.

Hint: Brown the stew meat in batches so that the meat isn’t crowded in the pan.
This will give the meat enough space to crisp without releasing all the juice
and just boiling it.


SUBSTITUTIONS

THE REDDIT BLACKOUT SHOWS NO SIGNS OF STOPPING

A widespread Reddit blackout affecting some of the site’s largest communities
has continued into its third day with no signs of stopping, as a number of
groups on the site vowed to remain closed off indefinitely to protest changes to
the platform’s data policies.

As of Wednesday morning, more than 6,000 subreddits remained inaccessible and in
private mode after what began as a two-day voluntary shutdown. The blackout
includes popular forums such as r/aww, r/videos and r/music, each of which
claims more than 25 million subscribers on the platform.

The extended protest highlights the commitment of some users, moderators and
developers to a long-term standoff with Reddit’s management over a decision to
begin charging steep fees for third-party data access to its platform.

Reddit didn’t immediately respond to a request for comment.

The coming fees have provoked broad outrage because of their expected impact on
independent apps and moderator tools that have grown up around Reddit and that
many users view as a critical resource. Some of the largest third-party apps,
such as Apollo and RIF, have said they cannot afford the fees and must shut
down, effectively driving users to Reddit’s native app that has been widely
panned as slow, buggy and inferior, particularly for users with disabilities.

In recent days, Reddit has said it would exempt some accessibility apps from the
price changes and allow some third-party tools to continue operating through its
application programming interface (API). But many moderators have called the
announcements little more than a “microscopic” concession.

In response to allegations that Reddit is imposing the fees and forcing
developers to shut down in a “profit-driven” move, Reddit co-founder and CEO
Steve Huffman said in a recent Q&A with users that Reddit will “continue to be
profit-driven until profits arrive.”

“Unlike some of the [third-party] apps, we are not profitable,” Huffman said.

The tensions echo how Twitter, under its new owner Elon Musk, has prompted
criticism with plans for its own paywall for data in a bid to develop new
revenue sources and to shore up the company’s struggling finances. For Reddit,
the stakes are also high to grow revenue, as the company reportedly looks to go
public later this year.

Huffman reportedly dismissed the blackout in a leaked internal memo obtained by
The Verge. According to the memo, Huffman described the protest as “among the
noisiest we’ve seen” but insisted that “like all blowups on Reddit, this one
will pass as well.”

“We absolutely must ship what we said we would,” Huffman reportedly wrote in the
memo, in an apparent reference to the API changes. Huffman also reportedly
predicted that some subreddits would end their protest after the initially
scheduled two days.

As of Wednesday morning, many groups participating in the blackout had lifted
their self-imposed restrictions. But even as some groups went public once more,
others joined the protest.

Most companies make new employees well aware that anything they create during
work hours is no longer their intellectual property but belongs to the
organization they work for. That doesn’t take the sting out of relinquishing the
rights to everything you’ve spent time on when you leave employment as one fired
employee learned the hard way.

In a Reddit post titled “I just deleted thousands of hours of work from my old
job,” later shared on a TikTok account called “@reddit_replay,” a man described
what happened when he realized his former employer was still using the work he
created, despite telling him that he was not competent enough for the job.

“I was freelancing and was on a loose contract. I was desperate for the money,”
he explained while justifying why he would put forth so much effort for so
little reward.

The workload was “insane,” according to him, but he managed to keep up with the
company’s demands for six months. On top of the low pay and crushing work
requirements, the freelancer described the office environment as “bitter” and
“snide,” and even accused management of stirring up trouble between workers for
their own entertainment.

As the six-month milestone approached, the contractor compiled the results of
the video work he had done, including a graph that measured click-through sales
that had come as a result of the content he created. He asked for a pay raise to
compensate for the traffic he had delivered and hard work he had put in
relentlessly.

The man even compared his salary to industry standards to demonstrate that he
was being underpaid in hopes that managers would understand and correct his pay.
He said that although he could make more money elsewhere, he would love to keep
working for the company in the same capacity.

Although he says he had laid out clear data connecting his social media
campaigns to increased sales, the worker was told that his content had not

Naturally, the man was beside himself with anger. To make matters worse, he

Three years after being tossed aside by a business he had given his all to, the
former employee was browsing his personal Google Drive and came across a folder
he had created and shared with the company during his tenure. He found that 18
staff members were still actively using his templates, adjustments, and presets
to produce content for their social media accounts.

use the cloud service that he was paying for. So, he saved all of his files to a
local drive on his computer and deleted the online folder, leaving no video
assets behind for his former employer, including projects already in progress.

REVENGE CAN BE SWEET, BUT DELETING COMPANY FILES CAN LAND YOU IN COURT IF YOU
AREN’T CAREFUL.

An inventions agreement releases all of your rights to things you created while
employed. This means that you cannot intentionally destroy proprietary files and
data when you are leaving. The Computer Fraud and Abuse Act offers employers
some recourse when they lose information due to malicious actions. The resentful
worker would be subject to civil and criminal liability for their actions.

On the other hand, it is totally irresponsible for a company to continue to use
cloud storage that they have no ownership over to keep any work-related
documentation. The ex-employee had no obligation whatsoever to pay for and
maintain their company records once he was let go. A comprehensive offboarding
process would have helped to transition the employee out in an organized fashion
while repossessing organization-specific information.

NEW OBESITY DRUGS COME WITH A SIDE EFFECT OF SHAMING

Eileen Isotalo was always able to lose weight, but always gained it back. Now
66, her first diet was with Weight Watchers at age 14. She went on to try one
diet after another and bought so many books on weight loss that she thinks she
has more than the public library.

In desperation, she finally went to a weight management clinic at the University
of Michigan. She had sleep apnea and aching knees, but could not curb her
appetite.

“It’s just this drive to eat,” said Ms. Isoltalo, a retired interior design
coordinator. “It’s almost like this panic feeling when you start craving food.”

“My mental shame was profound,” she said.

Now, though, since she started taking Wegovy, one of a new class of drugs for
obesity that was prescribed by her doctor at the clinic, those cravings are
gone. She has lost 50 pounds and jettisoned the dark clothes she wore to hide
her body. Her obesity-related medical problems have vanished along with much of
the stigma that caused her to retreat from family and friends.



But like others at the clinic, she still struggles with the fear others will
judge her for receiving injections to treat her obesity rather than finding the
willpower to lose weight and keep it off.

Yet the drug, she said, “changed my life.”

Wegovy and drugs like it make this “a very exciting time in the field,” said Dr.
Susan Yanovski, co-director of the office of obesity research at the National
Institute of Diabetes and Digestive and Kidney Diseases.

About 100 million Americans, or 42 percent of the adult population, have
obesity, according to the Centers for Disease Control and Prevention. For the
first time, people with obesity, who faced a lifetime of medical jeopardy, can
escape the ruthless trap of fruitless dieting and see their obesity-related
health problems mitigated, along with the weight loss.

But there is still the taint.

“There’s a moral component to it,” Dr. Yanovski said. “People really believe
that people with obesity just need to summon their willpower and they think that
taking a medicine is the easy way out.”

Unlike other chronic diseases, obesity is on full public display, Dr. Yankovski
said. “No one looks at you and knows you have high cholesterol of high blood
pressure,” she said.



Obesity, she added, “is one of the most stigmatized conditions out there. “



Wegovy and a similar but less effective medication, Saxenda, are the only ones
in their class of drugs so far to be approved for the treatment of obesity —
others like Ozempic and Mounjaro are diabetes drugs but also spur weight loss.

Novo Nordisk, Wegovy’s maker, reports that doctors in the United States have
written about 110,000 prescriptions for the drug. Citing a huge demand, the
company recently put its advertising for Wegovy on hold.

“We can’t make enough,” said Ambre James-Brown, a Novo Nordisk spokeswoman.
Supplies are so limited that the company is only selling the drug in the United
States, Norway and Denmark, the company’s corporate headquarters. Its high list
price of $13,492 a month puts it out of reach for most whose insurance will not
cover it. But increasingly many insurers do.

The drugs have arrived at a time when researchers have documented the risks of
obesity and the futility of prescribing only diet and exercise as a treatment.
Decades of studies have consistently shown that very few people can lose excess
weight and keep it off with lifestyle changes alone.



People with obesity are at risk for a variety of serious medical conditions,
including diabetes, hypertension, high cholesterol, sleep apnea and nonalcoholic
fatty liver disease, a leading reason for liver transplants in the United
States.

Losing weight can make some of these complications vanish.



Yet the belief persists — fed by diet gurus, influencers and an industry selling
supplements and diet plans — that if people really really tried, they could shed
pounds.

So those who take a drug like Wegovy often end up in uncomfortable situations
that are influenced by the common view that obesity is a lifestyle choice.

At the University of Michigan clinic there are those like Ms. Isotalo whose
reluctance to admit to taking Wegovy stems from her conviction that those who
take it are often thought to be cheating.



Another patient, though, Katarra Ewing of Detroit, readily tells anyone who asks
that she takes the drug. She tried diets, but it was Wegovy that allowed her to
lose 90 pounds.

She came to the weight management clinic after her all-night shift at a Ford
factory, ebullient and vibrant, wearing a vivid green sweater. She has more
energy now that she lost the weight, her mood is brighter, her high blood
pressure gone.

But she discovered an unintended social consequence to weight loss, as many
longtime friends fell away.

“Only my genuine friends are left and that’s a very small number,” Ms. Ewing
said.

Obesity medicine specialists say they are not surprised — they see the same
thing after people lose weight with bariatric surgery.

Relationships shift because obesity is such a defining condition. People of
normal weight may feel superior to a friend with obesity and that helps define a
relationship — until the friend loses weight. Other friends who themselves have
obesity may use the condition as a bonding factor in the relationship. Now that
is gone.



Another issue is the drugs’ reputation as vanity medications, which has been
amplified by comedians’ punchlines at the Oscars and in other high-profile
settings.

But when Samuel Simpson came to the weight management clinic, he considered
losing weight to be a matter of life or death.

Mr. Simpson was terrified he’d face the fate of his mother, brother and sister,
all of whom had obesity and diabetes. They all developed kidney failure that
ultimately killed them, each dying at the age of 59.



His first appointment with Dr. Amy Rothberg at the clinic was nearly two years
ago, when he was 58. He had obesity and diabetes. Although he was taking high
doses of insulin to lower his blood sugar, his kidneys were starting to fail.



“I was so afraid,” he said. “Was I going to end up on dialysis like everyone
else? I’d be history.”

He began with a diet and then Dr. Rothberg added Mounjaro, a drug by Eli Lilly
that appears to be even more powerful than Wegovy in eliciting weight loss, but
is, so far, only approved for people with diabetes.

Now he’s lost 44 pounds, 20 percent of his original weight, and his diabetes is
in remission. The weight loss, he said, “turned my life around.”

He will tell those who ask how he lost the weight,

“I’m not like the roadside preacher but when someone asks me how I did this I
will tell them,” he said.

Art Regner had a different issue. A garrulous color commentator for the Detroit
Red Wings hockey team, he said he was not ready to resort to medication. But
when he came to Dr. Rothberg’s clinic he was chagrined. He’d regained 22 of the
76 pounds he lost by dieting.

Dr. Rothberg, who is also the medical director of Rewind, a company that
counsels diabetic patients, suggested Wegovy or Mounjaro. But Mr. Regner felt he
should have enough willpower to do it on his own. He knows his blood sugar is
high and is aware of the consequences of diabetes.



Dr. Rothberg gently explained to him that it was not his fault he kept regaining
weight every time he lost some.

“I think biology is conspiring against you,” she said. “I don’t think it’s a
matter of willpower.”

Mr. Regner was not swayed. “I believe in myself,” he said. “I wake up in the
morning and look in the mirror and say, ‘Are you going to do it or aren’t you?’”
