At Firm Growth Forum, CPE credit will be awarded for the in-person event in
which you participate by attending and participating in. Please note that CPE
credits are not awarded for viewing archived recordings. All attendance will be
monitored and recorded.

In order to receive credit:

 * Attendees must sign the sign-in sheets that will be placed outside of each
   eligible session
 * Complete and submit this CPE and evaluation form no later than May 10, 2023,
   to receive your credits
